feat(mentions): say once in the room when a never-connected agent is mentioned (TASK-655, supersedes #3087) - #3101
Supersedes #3087 (Kai's draft; same commit, carried on my branch). TASK-655 item 3.
A person who @mentions a Bring your own agent that has never polled now gets one system line in the room, as a reply to their mention: the agent isn't connected yet and won't see it until it runs. At most one per agent per pod per UTC day. The mention still queues and delivers when the agent connects.
"Never connected" is deriveAgentState returning never-connected, the same verdict Your Team and the stalled-connect nudge use. No second rule.
Silent for bot senders, inactive installs, and a missing PG pool (no line beats a repeat).
…mentioned (TASK-655)
A person who @mentions a Bring your own agent that has never polled got
silence. The mention still queues; now the room gets one system line,
placed as a reply to the mention, saying the agent isn't connected yet.
At most one per agent per UTC day. The never-connected verdict is
deriveAgentState's, the same one the Your Team row and the stalled-connect
nudge use. Bot senders get nothing.

Walkthrough
Direct human mentions of never-connected agents can now trigger a system reply in the mention thread. The service checks eligibility, suppresses concurrent attempts, and avoids posting another notice for the same agent on the same UTC day.

Summary
Not-connected system line backend/services/systemActionLineService.ts
Adds a persisted system reply with agent and mention identifiers. A UTC-day query checks for an existing line for the same pod and agent.
Notice eligibility and posting backend/services/neverConnectedMentionService.ts, backend/__tests__/unit/services/neverConnectedMentionService.test.js
Checks sender, installation, and agent state. Suppresses concurrent attempts in-process and tests eligible, suppressed, and concurrent outcomes.
Direct mention integration backend/services/agentMentionService.ts
Calls the notice service for direct mentions from human senders. Logs notice errors and continues to enqueue the mention.

Team-Commonly/commonly#3087: Implements the same notice flow in the same services; this PR adds in-process suppression for concurrent mentions.
Team-Commonly/commonly#3050: Introduces the shared system-line persistence and placement path used by this notice.
Team-Commonly/commonly#2811: Adds thread-root propagation that this notice uses to place the reply in the mention thread.
Merge Risk: Moderate · up to 499ce
Some notices will be absent from the mention thread, and others can give recipients an unusable instruction or promise delivery that was not queued. Resolve these issues before merging.

Own review (Sonnet subagent), TASK-655 item 3. Read from the diff; tests not run by the reviewer (CI covers them).
SQL columns and payload keys match what insertSystemLine writes; deriveAgentState usage matches the other three callers; no owner-only fix command leaks into the copy.
Bot senders, non-active installs and a null sender fail closed; a failed notice never blocks the mention.
Medium: check-then-insert dedupe can double-post on concurrent mentions (e.g. @nova @nova). Fixing on the fix head with an in-flight guard.
Medium: V2MessageRow has no branch for this action, so it renders as a plain agent row. Readable; follow-up in TASK-709.
Low: no tests for the new SQL or the call-site gating; the in-flight guard gets one.

Do not instruct every sender to run the connect command.
A pod member who does not own the installation can trigger this notice. deriveAgentState provides the fix command only to the owner, so that member may not have the command shown on the agent’s page. Remove the instruction or direct the member to the owner. (raw.githubusercontent.com)
As per coding guidelines, fixCommand is an owner-only instruction in the shared agent-state contract.

Post the notice only after the mention queues.
If AgentEventService.enqueue fails, this call has already posted a line promising that the agent will see the mention when it runs. No event was queued in that case. Move the notice call after a successful enqueue so the line describes a durable delivery.

Review comment at @backend/services/agentMentionService.ts at line 1965:
Move the noteNeverConnectedMention call to after AgentEventService.enqueue
succeeds, so the notice is posted only when the mention has been queued;
preserve the existing behavior when enqueue fails.
Add caller-level coverage for the notice side effect.
The new tests call noteNeverConnectedMention directly, so they still pass if enqueueMentions stops calling it. Add an enqueueMentions test that asserts a human direct mention both posts the notice and queues the mention. Also assert that a bot sender queues the mention without posting a notice.
