feat(backend): log and count agentRuntimeAuth refusals by reason (TASK-712) - #3122
A failed agent connect left no trace. Every refusal branch in agentRuntimeAuth returned 401 with no log line and no counter, so nothing server-side could say whether an agent had tried and failed. This logs each refusal with its reason, and keeps a per-reason count an admin can read.
Change
backend/services/agentAuthRefusals.ts: nine reason codes. The credential gate's fixed messages map to bounded codes.
backend/middleware/agentRuntimeAuth.ts: all five 401 sites go through one refuse(). The status and the message text are unchanged.
backend/routes/admin/agentAuthRefusals.ts, mounted at /api/admin/agent-auth: GET /refusals returns per-reason counts since this process started. It sits behind a rate limiter ahead of auth and adminAuth, the same shape as routes/admin/pods.ts.
Log line
One [agent-auth-refused] line per (reason, fingerprint) per 60 s, with repeatsSinceLastLog for what was suppressed. It carries reason, route pattern, method and fingerprint. The fingerprint is the first 8 hex of the token's sha256. A global budget of 100 lines per window caps a spray of distinct tokens. Counts are never rate-limited.
Route is the mount prefix plus the route pattern. It never uses originalUrl, so ids and query strings stay out.
Never logs a credential
The token text never reaches the log or the response. The tests assert its absence on every branch.
Verification
agentRuntimeAuth.test.js: one case per refusal branch, plus the no-token-text assertion and the repeat-collapse assertion. New agentAuthRefusals.test.js covers mapping, fingerprint, route, the per-fingerprint window, the global budget and counts.
22/22 across the two suites. The route guard and user-shape contract suites pass.
Full middleware and routes unit run: 213 suites pass. tasksApi.leaseState and users.social are timing-sensitive under parallel load. Both pass 40/40 when run alone, and the same timing failures reproduce on base.
Typecheck: no errors in the touched files. The remaining errors are missing @openuidev/lang-core and implicit any in uiCardValidation.ts and agentComputerBroker.ts, present on base.
Review
Sonnet subagent review (the Opus window is closed until Nov 5): PASS, no required fixes. It checked token exposure, unchanged responses, the route field, the rate-limit ordering and the admin gate.
Known limits
Counts are per replica, like the rate-limit watermark. A fleet total is a sum across replicas.
The counts are per reason, not per agent. The fingerprint in the log is the per-client handle, and it can't name an agent for a token that never matched one.
…K-712)
A failed agent connect used to leave no trace: every refusal branch returned
401 with no log line and no counter, so nothing server-side could say whether
an agent had tried and failed.
Every 401 in agentRuntimeAuth now goes through one refuse() that records a
reason, the route pattern and method, and a token fingerprint (the first 8 hex
of the token's sha256). The response is unchanged.
- One structured [agent-auth-refused] line per (reason, fingerprint) per 60 s,
with the repeats it suppressed reported on the next line. A global budget of
100 lines per window caps a spray of distinct tokens. Counts are never limited.
- The token text never reaches the log or the response; the tests assert it.
- GET /api/admin/agent-auth/refusals returns per-reason counts since this
process started (per replica, like the rate-limit watermark). Admin-only,
behind a rate limiter ahead of auth, same shape as routes/admin/pods.ts.

Agent authentication refusals now record bounded reasons and request metadata. The middleware preserves existing 401 responses. An authenticated, rate-limited admin endpoint returns process-local refusal counts.
Classify refusals and maintain bounded counts and logs backend/services/agentAuthRefusals.ts, backend/__tests__/unit/services/agentAuthRefusals.test.js
The service maps credential messages to refusal reasons, fingerprints tokens, derives routes, and tracks per-reason counts. It rate-limits warning logs by reason and fingerprint and applies a global log cap. Tests cover classification, logging, rate limits, and count snapshots.
Record refusals in agent authentication middleware backend/middleware/agentRuntimeAuth.ts, backend/__tests__/unit/middleware/agentRuntimeAuth.test.js
Existing 401 paths now record refusal metadata while retaining their response status and message. Tests cover refusal reasons, token-safe logs, and repeated refusals.
Expose refusal counts through the admin API backend/routes/admin/agentAuthRefusals.ts, backend/server.ts, backend/__tests__/unit/routes/admin.agentAuthRefusals.test.js
The new endpoint applies an IP-based request limit before authentication and returns refusal counts as JSON. The server mounts the router at /api/admin/agent-auth. Route tests cover authentication and returned counts.
The endpoint’s real authorization and refused responses need integration coverage. No current authorization failure was established.

…te (TASK-712)
The token-text check ran only the invalid_token branch. It now runs each
token-bearing refusal (invalid, not installed, expired, revoked) with its
own token and checks the full captured log.
The admin refusals route had no request-level test. A new suite sends real
requests through the router and pins 401 (no auth), 403 (non-admin), and
200 with the per-reason count snapshot.

Answers to the two CodeRabbit threads, at head 89f8b21:
discussion_r4433833204 (agentRuntimeAuth.test.js:345), fixed: the token-text check now runs each token-bearing refusal branch (invalid_token, agent_not_installed, session_expired, token_revoked) with its own distinct token, asserts all four reasons were logged, and checks the full captured log for each token's text.
discussion_r4433833209 (agentAuthRefusals.ts:35), fixed: new request-level suite __tests__/unit/routes/admin.agentAuthRefusals.test.js sends real requests through the router and pins 401 (no auth), 403 (non-admin), and 200 with the per-reason count snapshot.
Local: the three TASK-712 suites pass (25/25) and eslint is clean on the changed files apart from two pre-existing max-len warnings.
Not part of this PR: dailyDigestService.membership.test.js ("a scheduled run takes only the people for whom it is the hour") fails on the current date because its fixture uses a fixed 15:01Z instant and expects 08:00 in Los Angeles. On 2026-11-01 Los Angeles is already on standard time, so the local hour is 07:01. This PR does not touch the digest code (no diff against origin/main), so it fails on main too.

Exercise the refused cases through the production guards.
auth and adminAuth are mocked, so body assertions would only reassert the mocks. A production guard could return counts or since with a 401/403 while both tests still pass.
The refusal-case rule also applies to this read-only GET: use the real guards, seed a nonzero snapshot, and assert that the refused response omits the snapshot and leaves refusal state unchanged. Keep the mocked tests only for route-chain status behavior.

The Supertest requests use mocked auth and adminAuth, so they do not exercise bearer authentication or production adminAuth against a non-admin User row. Add a request-level case that uses the real guards and a MongoMemoryServer-backed user. Keep the mocked cases if they cover guard ordering.
