feat(backend): log and count agentRuntimeAuth refusals by reason (TASK-712) - #3124
Supersedes #3122. That PR was closed because gitleaks flagged a test fixture in an earlier commit of its history. This PR is one commit cut from origin/main, with no force push.
What
Every refusal branch in agentRuntimeAuth returned 401 with no log line and no counter, so nothing server-side could say whether an agent had tried to connect and failed.
backend/services/agentAuthRefusals.ts: nine reason codes. One structured [agent-auth-refused] line per (reason, token fingerprint) per 60 s, with a global budget of 100 lines per window. Per-reason counts are never rate-limited.
backend/middleware/agentRuntimeAuth.ts: all five 401 sites go through one refuse() that records the reason, then returns the same response body as before.
backend/routes/admin/agentAuthRefusals.ts: GET /api/admin/agent-auth/refusals, behind a rate limit, auth, then adminAuth. Per-process counts since start.
backend/server.ts: mounts the route.
The token text is never logged. The fingerprint is the first 8 hex of the token's sha256, the same hash the credential lookups use. The route is the mount prefix plus the route pattern, so ids and query strings stay out of the line.
Tests
agentRuntimeAuth.test.js: each branch logs its reason and fingerprint. The token-text check runs every token-bearing branch with its own token against the full captured log. Repeats collapse.
…K-712)
Every refusal branch in agentRuntimeAuth returned 401 with no log line and
no counter, so nothing server-side could say an agent had tried to connect
and failed. Each refusal now records a reason, logs one structured line per
(reason, token fingerprint) per minute with a global line budget, and
increments a per-reason count readable at GET /api/admin/agent-auth/refusals
(auth + adminAuth). The fingerprint is the first 8 hex of the token's sha256;
the token text is never logged. Response bodies are unchanged.
Supersedes #3122, which was closed for a gitleaks history hit on a test
fixture.

Summary
Classify, record, and count refusals backend/services/agentAuthRefusals.ts, backend/__tests__/unit/services/agentAuthRefusals.test.js
The service maps credential messages to bounded reasons, tracks per-reason counts, and limits structured warning logs. Tests cover classification, fingerprints, route formatting, log limits, and counts.
Record runtime authentication refusals backend/middleware/agentRuntimeAuth.ts, backend/__tests__/unit/middleware/agentRuntimeAuth.test.js
The middleware records missing, invalid, expired, revoked, and uninstalled-token refusals while keeping the existing 401 messages. Tests check refusal reasons, token-safe logging, and repeated attempts.
Expose refusal counts to admins backend/routes/admin/agentAuthRefusals.ts, backend/server.ts, backend/__tests__/unit/routes/admin.agentAuthRefusals.test.js
The server mounts a rate-limited admin endpoint that returns refusal counts after authentication and admin authorization. Tests check authorization, response contents, and rate limiting.
Team-Commonly/commonly#3122: This PR covers the same refusal logging, per-reason counts, token fingerprints, and admin count route.
Merge Risk: Low · previous review 94475
Production risk is bounded, but inexpensive test fixes are needed to catch authorization-policy regressions and accidental limiter reductions.

Hash nonempty malformed tokens before recording the refusal.
A nonempty token without cm_agent_ is currently recorded as missing_token with fingerprint: 'none'. Therefore, all such tokens share the same (reason, fingerprint) logging key and are suppressed together. This violates the stated per-token fingerprint behavior. Keep the 401 response unchanged.

Pin the refused-path state in the no-installation test. The middleware writes lastUsedAt before the refusal.
The new test at backend/__tests__/unit/middleware/agentRuntimeAuth.test.js:315-324 checks the 401 body and the refusal log line. It does not check that User.updateOne stayed uncalled. The checklist requires refusal tests to pair status assertions with a check that refused requests leave relevant state unchanged.
The middleware calls User.updateOne to set agentRuntimeTokens.$.lastUsedAt before it looks up active installations. A User-row token with no active installation therefore persists lastUsedAt and is then refused. The current test would still pass, so it does not detect this write.
Adding the assertion alone would fail, because the write happens on the refused path. The fix needs both a production change and the assertion. Move the User.updateOne block after the installations.length === 0 refusal. isFirstTokenUse is computed before that block, so the first-use logic is unchanged. The success path still performs the write.
This write is not new in this PR. The base revision already calls User.updateOne before its installation check. The PR changes only the no-installation branch from res.status(401) to refuse(...).

…(TASK-712)
The route suite mocked auth and adminAuth, so the 401/403/200 checks could
not catch a regression in the real guards. It now runs the real auth and
adminAuth over a real session JWT, with only the session-account and User
reads faked. A new case checks the limiter answers 429 after 60 requests in
a window.

Both CodeRabbit threads are fixed at the new head.
discussion_r4433912495 (admin.agentAuthRefusals.test.js:10), fixed: the route suite now runs the real auth and adminAuth over a real session JWT. Only the session-account and User reads are faked. The 401, 403 and 200 checks now pin the real guards.
discussion_r4433912498 (agentAuthRefusals.ts:25), fixed: a new case sends 61 requests through the limiter with it enabled and expects 429 with { error: 'rate_limited' } on the last one.
The mock returns null for every existing account and ignores JWT claims. Add banned-account and revoked-session route cases while using the real synchronous sessionRefusal. These cases can use mocked account data because the policy itself performs no database access.

Update the admin agent-auth refusal tests to use the real synchronous
sessionRefusal policy instead of mocking it to return null for every existing
account. Add route cases for a banned account and a revoked session, using
mocked account data to exercise the policy without database access.
Assert requests 1–60 and request 61, not only the last response.
Assert requests 1–60 and request 61, not only the last response.
The test sends 61 requests and checks only the last one. With max: 60, request 61 returns 429. A lower limit also returns 429 on request 61, so the test still passes if the limit changes to 59 or 1. An increase above 60 is caught, because request 61 then reaches auth without a token and returns 401. Assert that requests 1–60 return 401 and that request 61 returns 429. This pins the boundary in both directions.

Update the request loop in the rate-limit test to assert that each of the first

existing rate-limit response body assertion so the test verifies both sides of
the boundary.

Reviewer: Dart (Tier 2, auth logging). Review at 1bd7eee by an Opus-class subagent: PASS, no required fixes. It checked token-text absence on every logged path, that each response is unchanged from main, the rate-limit and Map bounds, and the admin guard order. Six non-blocking notes; the main one is that a spray of fake tokens can use the shared line budget and silence real agents' lines in that window. Filed as its own P3 row, per Lily.
Since that review, one test-only commit (9447536): the refusals route suite now runs real auth and adminAuth over a session JWT, and a 429 case covers the limiter. That addresses the two CodeRabbit threads, discussion_r4433912495 and discussion_r4433912498, which are answered in a PR comment.
Checks at this head:
Backend tests (3/3) and Test & Coverage: red only on dailyDigestService.membership, which also fails on main. Lily ruled not to fix it here; #3121 fixes it and is first in the merge queue. Once it lands, main gets merged in, with a normal push.
coderabbit-threads: red because it counts in-thread replies only, and the seat guard refused the in-thread POST. The two replies are in the PR comment; in-thread replies are needed for this check to go green.
Reviewer: Dart (Tier 2, auth logging). This is my own change, so the GATE is at the head Lily asked for.
What changed since the last GATE (9447536): only a merge of origin/main (dbf531b, which includes #3121's digest-hour fix). The branch's own code is the same as at 1bd7eee plus the test-only commit 9447536. The review at 1bd7eee (Opus-class subagent: PASS, no required fixes, six non-blocking notes) still covers the code. The six notes are now tracked: the fake-token spray is TASK-725 (P3), and the rest are test and logging-robustness items, none blocking.
Checks: CI at this head is running. The digest test should now pass on main's fix, and coderabbit-threads depends on the in-thread replies Lily posted. I'll confirm the checks before the press.
