fix(agents): agent state reads listener check-ins, so an MCP call alone is never-connected (TASK-721) - #3130
Part of TASK-721. deriveAgentState and its callers now read lastListenedAt (a listener check-in), not lastUsedAt (any authenticated call). A seat that has only made MCP calls reads never-connected, so the stalled-connect nudge fires for it, and the BYO page and the "is listening" state no longer flip on an MCP call.
HELD: do not merge or deploy until SRE confirms the backfill apply (#3120, dry-run id set then --apply --ids). Prod order: check-in writer #3114 (live), backfill, re-run the backfill right before this deploy, then this change. Without the backfill every old seat reads never-connected and is nudged.
What changes:
agentStateService: latestListen reads lastListenedAt. AgentStateRow.lastUsedAt keeps its wire name and now carries the last listener check-in.
agentListenerCheckIn stamps both copies of a token (the User row and AgentInstallation.runtimeTokens) and works with no agent user id. The WebSocket connect stamps an installation-only token too.
POST /events/renew is a check-in. The CLI runner polls /events only between turns, so during a turn over 5 minutes only renew traffic flows, and without this a live seat would read gone-dark. (Opus review pass 1 found this; fixed here.)
A renew is a check-in only when it names an event this agent owns and that is delivered or already checkpointed (AgentEventService.holdsEvent), so made-up ids do not count and a seat that checkpointed mid-turn still does. A valid pong from a live WebSocket is a check-in too.
Selects switched in routes/pods.ts (/agent-states), routes/registry/install.ts, stalledConnectService.botTokensFor, neverConnectedMentionService.
kernelWorkSweepService comment corrected: an MCP-only holder gets no deferral.
Runbook and agent-status-honesty.md updated.
Tests: a real-model service test for /agent-states (User-token and installation-token check-ins over HTTP), plus unit tests for the check-in stamps (installation, no user row, retry), derivation (MCP-only never-connected, check-in beats a later MCP call, old seat after backfill stays gone-dark), the nudge (MCP-only nudged, backfilled old seat not), and renew as a check-in (mutation-checked). Pointing latestListen back at lastUsedAt turns 13 tests red. Touched suites green here, tsc clean, 0 lint errors.
Known follow-ups, not in this PR:
deriveAgentConnection (the Your Team card) and the activity bucket still read lastUsedAt. For an MCP-only seat the card can read connected while the room says not connected.
No /agent-states round-trip test.
No index on tokenHash (fine at current scale).
A legacy installation-only token stamps only the installation that holds it, so a sibling installation of the same agent identity reads never-connected. Same on main with lastUsedAt; not widened here.
Push-webhook agents never poll, so they read "waiting".
Review: Tier 2, writer's own Opus subagent over two passes, filed as a PR review. The first merge of main into this branch (43af74e) brings in only main plus a runbook conflict resolved to this branch's text.
…ntime tokens, dry run first (TASK-721)
Agent state is about to read listener check-ins. Tokens that predate
lastListenedAt have no value, so without this every long-gone seat would
flip from gone-dark to never-connected on deploy. The script stamps
lastListenedAt = lastUsedAt per token element, insert-only against live
writers, and its apply run touches only the id set a dry run froze.
Installation tokens gain the field so the script (and a later stamp) can
write it.

…nd also stamps explicit nulls (TASK-721)
The dry run now counts used tokens whose subdocument has no ObjectId _id
instead of listing them as ids that apply would fail on. The write guard
is lastListenedAt: null, which matches an absent field and an explicit
null but never a live Date, so it agrees with the listing. An
unacknowledged write is a failure, not "already stamped". The runbook
says what to check before the derivation switch ships.

Check the id's type, not its shape, so a 24-hex string id is counted as
unaddressable instead of being listed and silently skipped. List with
`!= null` so the listing and the write guard agree for every stored
value. The runbook and the script header name the current guard.

Move the --out write, the --ids read and the report into runBackfill so
they can be tested without a second connection. A failed stamp now fails
the run after the whole set has been tried, and the message says to re-run
the same ids file. The runbook names the installation-token live stamp as
a deploy gate for the derivation change.

deriveAgentState and its callers now read lastListenedAt, not lastUsedAt. A seat
that has only made MCP calls reads never-connected and the stalled-connect scan
nudges it. The check-in writer stamps the installation copy of a token as well,
and the WebSocket connect stamps an installation-only token, so a token that
lives only on AgentInstallation no longer reads never-connected.

…t inside a long turn stays listening (TASK-721)
The runner polls /events only between turns. During a turn the only seat traffic
is POST /events/renew, which stamped lastUsedAt alone, so a turn past five
minutes would have aged a working seat into gone-dark once state read
lastListenedAt. Renew now stamps the check-in. Also asserts the stalled-connect
projection in its test mock and corrects two comments and the status doc.

Tier 2 review (schema and wake), run by the writer with an Opus subagent over two passes. This is the derivation switch for TASK-721: deriveAgentState and its callers read lastListenedAt, not lastUsedAt.
Pass 1 (98dfff2) held on one blocker: the CLI runner polls /events only between turns, and during a turn the only seat traffic is POST /events/renew, which stamped lastUsedAt alone. A seat inside a turn longer than five minutes would have read gone-dark, shown an offline banner, and had its lapsed lease rescued without the #1080 deferral. Fixed: a delivery renew now counts as a listener check-in (routes/agentsRuntime.ts, with a route test I mutation-checked).
Pass 2 (c95d7cd) found the fix correct. A renew from an MCP-only seat or a spawn-child token stamps nothing, and the check-in still fires before the nonce is confirmed, which gives a token holder nothing they could not already get from GET /events.
Also fixed from pass 1: the stalled-connect test mock now asserts its projection, the kernelWorkSweepService comment and docs/design/agent-status-honesty.md are corrected, and the runbook says to re-run the backfill right before the deploy.
Known and not in this PR (follow-ups):
deriveAgentConnection (the Your Team card) and the activity bucket still read lastUsedAt, and the credential ledger has no lastListenedAt. For an MCP-only seat the card can read connected while the room banner reads not connected.
No /agent-states round-trip test for the pods.ts projection. Both projections were read and are correct.
No index on runtimeTokens.tokenHash or agentRuntimeTokens.tokenHash; the auth middleware already scans the same paths.
A WebSocket listener checks in once, at connect.
Deploy order: check-in writer (#3114), the backfill (#3120, run by SRE against the frozen id set), then this change.

Walkthrough
This change records listener check-ins in user and installation runtime-token stores. Event renewals and active WebSocket pongs provide check-in signals. BYO liveness and related status checks use lastListenedAt instead of lastUsedAt.
The check-in service updates matching user and installation tokens with the same timestamp. Event renewals and active WebSocket pongs record check-ins. Legacy installation-token WebSocket authentication updates both lastUsedAt and lastListenedAt. Tests cover token matching, installation-only tokens, renewal outcomes, retries, and replaced sockets.
BYO state uses the latest valid lastListenedAt across user and installation token stores. State queries and mention handling select that field. Tests distinguish listener check-ins from MCP use and cover installation-only tokens and backfilled timestamps. The design document describes the check-in signals.
Apply check-ins to stalled-connection checks backend/services/stalledConnectService.ts, backend/services/kernelWorkSweepService.ts, backend/__tests__/unit/services/stalledConnectService.test.js, docs/runbooks/listener-check-in-backfill.md
Stalled-connection scans use lastListenedAt as evidence of a listener check-in; lastUsedAt alone does not prevent a nudge. The runbook describes writer, backfill, and derivation deployment order and identifies remaining readers of lastUsedAt.

Add a real-model round-trip test for the installation check-in.
The test at lines 46-48 asserts the AgentInstallation.updateMany call against a mock. It does not show that the installation runtimeTokens entry persists lastListenedAt, or that the state derivation reads it back. A dropped write, or a path the schema does not declare, would still pass this test. A live seat could then read as never-connected and receive the stalled-connect nudge.
Add a test that calls recordListenerCheckIn against the real AgentInstallation model. Read the document back, then pass its runtime tokens to deriveAgentState. Add the same coverage for the User.agentRuntimeTokens store. backend/__tests__/unit/scripts/backfill-listener-check-in.db.test.js already uses real models for these fields. It covers the backfill writer, not the check-in writer or the state read path.

Tier 2 re-review (schema and wake), writer's own Opus subagent, third pass over the CodeRabbit fixes. Earlier PASS was at c95d7cd; this covers 6656346 and 3506c7e.
Renew check-in: first version stamped only when a delivery renewed. The pass found that breaks the long-turn case, because the MCP checkpoint tool acks mid-turn (row becomes acked, nonce cleared) and every later renew then returns false while the runner keeps working. Fixed with holdsEvent (this agent's own delivered or acked event). Made-up ids, pending rows and other agents' events do not count.
WebSocket: a valid pong from the live socket is a check-in; JWT sockets are a no-op; a replaced socket cannot stamp.
/agent-states has a real-model round-trip service test (User token and installation token).
Non-blocking, left as is: holdsEvent has no age bound; the pong test sets tokenHash by hand; the sibling-installation case for legacy installation-only tokens is unchanged from main and listed as a follow-up in the PR body.
Mutation checks: guard, pong call, projection and the checkpoint case each turn a test red. Touched suites green, tsc clean.

Wait for the check-in write, not a fixed delay.
recordListenerCheckIn starts asynchronous database writes. If a write takes more than 150 ms, states() reads the old value and the test fails. Poll the expected token’s persisted lastListenedAt with a bounded deadline before requesting /agent-states.

Replace the fixed 150 ms wait in the check-in test with bounded polling for the
expected token’s persisted lastListenedAt value, and request /agent-states only
after that value is observed. Use the recordListenerCheckIn write outcome as the
condition rather than relying on elapsed time.
The pong test assigns mockSocket.tokenHash directly, so it does not exercise namespace authentication or prove the assignment in backend/services/agentWebSocketService.ts. Add a named test that authenticates a cm_agent_ socket, emits pong, and asserts that recordListenerCheckIn receives the derived hash. This is a testing-requirement gap, not a demonstrated production failure.
