fix: A review-repair job that fixes nothing is counted as a successful repair - #954
Treat no-op review-repair jobs (exit 0, head unchanged, threads open) as failed attempts toward the repair cap, and block item done when local commits never reached the remote.
Opened by cursor-cli on i-flare-51bb8de6c33b (flared:51bb8de6c33b) for item #393 via agentflare.
Summary by CodeRabbit
Bug Fixes
Completion now checks whether a tracked pull request’s branch has diverged from its target and may stop completion when it has.
Repair attempts that finish without changing the pull request can now count toward the repair limit, helping prevent repeated no-op attempts. Once the limit is reached, a needs-human-review gate is applied.
Repair dispatches are tied to the pull request’s current revision to avoid applying repair decisions to a different revision.
Enable usage-based reviews in Billing to review now. Otherwise, wait until the next included review is available.

Walkthrough
Item completion now checks remote branch publication before returning a push outcome. The supervisor also tracks successful no-op CodeRabbit repairs against the self-repair cap, using the PR head SHA recorded at dispatch.
The new worktree helper reads the origin task-branch tip. The completion helper uses that tip and branch divergence to determine whether to post a marked comment and return an error. item_done consumes the helper’s outcome.

Summary
Pass the PR head SHA to repair dispatch src/supervisor/merge.rs, src/supervisor/review_bots.rs, src/supervisor_coderabbit_tests.rs
The merge path passes the CI-judged head SHA to repair dispatch. Dispatch records or clears the head metadata, and existing test calls provide the new optional argument.
The sweep checks repair state, findings, head SHA, and success comments before crediting a void repair. A regression test checks the self-repair cap and needs-human gate.

getappz/agentflare#960 — Its changes to push_and_open_pr outcomes affect the outcome that this completion guard evaluates.

Count each repair dispatch exactly once.
The initial dispatch already contributes one marker to coderabbit_repair_prior_attempts. Line 255 adds a silent attempt for that same job when it succeeds without moving the head. The next retry then skips its own attempt increment because its success was credited. If that retry fails without another WORK_SUCCESS comment, later retries keep skipping the increment. The repair cap can therefore fail to stop repeated dispatches. Count each enqueue once, and track credited successes separately from the attempt count.

Keep the publication check active when push is false.
When an existing, unmerged PR has new local commits, push: false skips the remote-tip check. item_done can then mark the item completed without publishing those commits. Compare the local and remote task-branch tips before completion even when the caller disables the push.

Use the dispatched finding set for void-credit fingerprints.
coderabbit_repair_or_gate receives review.to_dispatch, which includes optional findings when a blocking finding starts a repair. maybe_credit_void_coderabbit_repair filters those findings with blocks_merge(), so a mixed set gets a different fingerprint and a no-op success may not be credited. Pass the same dispatched set to both paths, and add a mixed-finding cap test.

L55: ### Description
L56:
L57: `tokio::io::unix::AsyncFd` provides safe constructors (`new`, `with_interest`, `try_new`, and `try_with_interest`) that accept any `T: AsRawFd` rather than requiring proof of ownership or validity of the file descriptor. Because `RawFd` implements `AsRawFd`, safe code can construct an `AsyncFd` from an arbitrary or closed file descriptor, or close/replace the underlying file descriptor while the `AsyncFd` is still alive.
L58:
L59: This violates I/O safety in two ways:
L60: 1. `AsyncFd<T>` implements `AsFd` for all `T: AsRawFd` by calling `BorrowedFd::borrow_raw(self.as_raw_fd())`, allowing safe code to obtain an arbitrary `BorrowedFd<'_>` without owning the file descriptor.
L61: 2. Tokio's I/O driver stores a pointer to an internal allocation in the `epoll`/`kqueue` user data for registered file descriptors.
L62: If the underlying file descriptor is duplicated and then closed while an `AsyncFd` is alive, and the OS reuses that file descriptor number for a another IO resource on the same runtime, dropping the first `AsyncFd` deregisters the reused file descriptor number and frees the first while the original file description remains registered in `epoll`, leading to a use-after-free when a readiness event fires.
L63: Starting in Tokio 1.51.6, 1.53.3, and 1.54.0, `AsyncFd::new`, `AsyncFd::with_interest`, `AsyncFd::try_new`, and `AsyncFd::try_with_interest` are deprecated in favor of the `unsafe` constructors `AsyncFd::register` and `AsyncFd::register_with_interest`. Callers must ensure that the file descriptor remains valid, open, and unchanged for the lifetime of the `AsyncFd`.

Remove the stale Tokio advisory ignore.
The lockfile contains only tokio1.54.1, which RustSec lists as patched. Remove this unused ignore.

Compare the local task-branch tip with the remote tip before rejecting done.
The gate tests only whether a remote tip existed before the push. It never compares that tip with the local branch head. This causes two errors:
False rejection: When .worktrees/task/<seq> is missing, push_and_open_pr returns NothingToPush before it checks divergence. If the branch is already pushed, remote_tip_before is Some. branch_diverged then reports true for an unmerged branch, so done fails with "unpushed commits remain". Retries fail the same way.
Missed rejection: If origin/<branch> does not exist, remote_tip_before is None and the gate is skipped, even when local commits were never published. A missing remote ref means "not published", not "ok".
Reject only when the branch diverges and the local head differs from the remote tip. Read the remote tip at the check, after the push attempt:
Add local_task_branch_tip beside remote_task_branch_tip in src/worktree/mod.rs. It must return the head of the local refs/heads/<branch> ref for the same branch resolved by resolve_item_task_branch.

Review comment at @src/mcp_server/item_merged_pr.rs around lines 155 - 156:
Update the `done` rejection gate to require both `branch_diverged` and
inequality between the local and remote task-branch tips, reading the remote tip
after the push attempt so a missing remote ref also counts as different. Add
`local_task_branch_tip` beside `remote_task_branch_tip` in `worktree/mod.rs`;
return the local `refs/heads` tip for the branch resolved by
`resolve_item_task_branch`.

This PR now conflicts with its base branch and can't be merged automatically. Rebase or merge the base branch in and resolve the conflicts, then push.
(Set auto_resolve_conflicts = true under [review_sweep] in .agentflare/config.toml -- project or user-home -- to have an agent attempt this automatically instead.)
Extract push/PR validation into item_merged_pr; allow repeat done when the
PR branch is already on the remote; void-credit only when the announced
findings fingerprint still matches open blockers; count attempts as
max(dispatch markers, silent bumps).
