fix(review): validate repair routing and credit no-op attempts - #965
Review repair now checks PR-to-item identity, marks findings outside the diff for out_of_scope reporting, and credits fully reported no-op runs while keeping lost jobs bounded by the repair cap. Explicit adoption preserves source identity, and scope changes refresh retry briefs. Validation: 208 supervisor tests, 31 discovery/routing tests, and all 10 item724 regressions passed; formatting, diff checks and independent review passed. The full workspace suite was not run. Older adoptions may need pr adopt again; historical attempts remain charged.

Summary by CodeRabbit
Bug Fixes
Pull requests are matched to the correct work item before review or repair actions proceed.
Findings outside the pull request’s changed files are identified and excluded from repair instructions.
Repair attempt limits account for fully reported attempts with no fixes, while still counting incomplete or legacy reports.
Pull request discovery and adoption preserve item identity when matching existing work.
Tests
Added coverage for pull request identity matching, out-of-scope findings, and repair attempt accounting.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
The PR adds PR-to-item identity validation for adoption, discovery, and live reviews. It marks review findings outside the PR diff and gives fully reported no-op repair attempts credits against retry counts.
Identity checks compare PR metadata, item identity, branch, and normalized title. Discovery skips mismatched PRs. Adoption reuses an item with matching PR metadata or creates a tracking item, then records the adopted identity.
Live review validation and finding scope src/supervisor/review_bots.rs, src/supervisor/review_findings.rs, src/supervisor/tests/review_routing_tests.rs, src/supervisor_tests.rs, src/supervisor.rs, docs/superpowers/plans/*
Live review checks PR identity before processing threads. When findings are queued, it verifies changed-file data and marks findings outside the diff, including renamed paths. Repair briefs tell bots not to edit those findings, and fingerprints include their scope.
Dispatches store job, thread, and round data. Matching reports update thread outcomes. A completed attempt earns a credit when every thread outcome is non-Fixed; charged attempts subtract credits from gross attempts with saturation.

Possibly related issues
getappz/agentflare#947: This issue concerns self-repair dispatch for PRs owned by another instance. This PR adds instance-aware identity validation during discovery and live review.
getappz/agentflare#973: This issue concerns duplicate-PR matches when sequence IDs are reused. This PR validates PR-to-item identity, but does not address the separate label creation problem.
Possibly related PRs
getappz/agentflare#968: Both PRs change supervisor repair-cap handling; this PR changes attempt charging, while #968 adds comments for cap and blocked self-repair outcomes.
getappz/agentflare#954: Both PRs change repair-attempt accounting. This PR tracks outcomes by dispatch owner, thread, and round when calculating no-op credits.
getappz/agentflare#955: This PR uses the changed-file API and current and previous filenames to validate finding scope.

Review repair can dismiss a finding that has entered the PR diff, and editing an untagged adopted PR’s title can halt repair for human mapping. Resolve these issues and confirm the reported Clippy failure is cleared before merging.

existing title-based safeguard for items without the marker; add a regression
