Retry failed PR-comment posts on later blocked sweeps.

A failed post_pr_merge_block_once call stores no fingerprint. The current first_time_gated(...) guards still consume their keys, so later blocked sweeps do not retry the missing-approval, GitHub-review, or dispatch-gate comment.

Keep the notifications inside first_time_gated(...), but call each idempotent PR-comment helper on every blocked sweep. The PR number and labels are already available in handle_ci_green. A retry performs a metadata read and, when no fingerprint exists, attempts the GitHub comment.

Review comment at @src/supervisor/merge.rs at line 183:
Update the blocked-sweep handling in handle_ci_green so idempotent PR-comment
helpers run on every sweep: keep notifications inside first_time_gated checks,
but move post_missing_approval_pr_comment, post_github_review_block_pr_comment,
and post_item_dispatch_gate_pr_comment outside those checks while retaining each
blocker’s condition. This allows failed comment posts to retry on later sweeps
without repeating notifications.
