- Line 725: Update the row button’s aria-label in the ProviderAuthPanel
component to include the key’s accessible label when a disabled entry uses the
resume action. Preserve the resume announcement and use entry.label with
entry.masked as the fallback.
- Line 592: Extract a shared pause-tooltip helper that translates each pause
reason code, formats the paused date using the active app locale, and avoids a
hardcoded separator. In
gui/src/components/provider-workspace/ProviderAuthPanel.tsx at lines 592 and
731, replace the OAuth and API-key badge titles with the helper; in

pass the locale from useI18n().
Review comments at @gui/src/hooks/useProviderAccountPools.ts:
- Around line 559-563: Update the disabled-entry resume flow in the provider key
switch handler to track when the resume PUT succeeds, then report that partial
success if the subsequent activation PUT fails. Keep the existing failure
notification for cases where resuming itself fails.
Review comments at @src/cli/account-api.ts:
- Line 421: Add a brief comment beside the pauseDetails parsing in each affected
row-construction path to clarify whether invalid or newer pause details are
intentionally omitted when accountPauseDetailsSchema.safeParse fails.
Review comments at @src/cli/account-extended.ts:
- Around line 848-850: In the API-key pause branch using apiJson, handle status
0 by passing result.transportError to proxyUnreachable before the general
apiError check. Format its JSON success output with two-space indentation,
matching the neighboring branches; retain the existing text-output wording.
- Line 854: Update the fallback usage error message that interpolates `${verb}`
to include API-key pools alongside the OpenAI Codex account pool and generic
OAuth providers.
- Line 847: Update the API-key error branch that checks `"error" in target` to
include the `Error:` prefix in the message passed to `usage`, while preserving
the existing OAuth-to-API-key wording.
Review comments at @src/config/schema/config-schema.ts:
- Line 302: Update codexAccountPauseState in the config schema to catch invalid
values as undefined, and add a brief comment explaining that this display
metadata can be safely dropped because pausedCodexAccountIds remains the
authority for pause eligibility.
Review comments at @src/providers/api-key-pause.ts:
- Line 16: Update setProviderApiKeyPaused to accept the captured refusal
threshold and route, then compare both against the freshly persisted provider
inside the mutation before pausing. Replace reliance on the request process’s
stale policyCurrent() check so a changed threshold or route prevents the pause.
Review comments at @src/server/management/oauth-account-routes.ts:
- Around line 1120-1121: Validate the result of readManagementJsonBodyOr with

gui/src/hooks/useProviderAccountPools.ts.
Review comments at @structure/providers-and-adapters.md:
- Line 9: Remove the unrelated auto-pause link appended to the sentence about
Opus progress updates and Anthropic account thresholds; keep that sentence
focused on its two existing contracts.
Review comments at @structure/runtime.md:
- Line 117: In the Orca import section, remove the unrelated permanent-refusal
auto-pause link or add a brief sentence explaining its relevance to runtime;
keep the link attached to that explanation rather than the Orca import sentence.
Review comments at @structure/transports/responses-failover.md:

the failover documentation; leave the existing auto-pause reference at the later
section unchanged.

Held out of this round's batches: the refreshed head (16c1381) passes remote validation on lidge, but a security re-review left one open item on a recovery path that needs a deliberate maintainer fix before landing. Details are tracked privately. |
…view Persist a per-provider accountAutoPausePolicyRevisions UUID, minted only by persistConfigUnlocked from the pre-write file when a tracked provider's effective policy fingerprint changes. Attempts capture revision, local generation, threshold and route; OAuth, API-key and Codex pauses commit only while the in-memory and fresh persisted fence still match, with the OAuth automatic publication serialized under the config mutation lock. Also preserve key pause fields across dual-auth OAuth upserts, keep an omitted accountAutoPause on full provider saves, accept the policy as a canonical-provider overlay, reject non-object pause bodies, degrade malformed pause display metadata, and fix CLI/dashboard pause messages, translations and accessible names.

…he wire sendWithConnectionPolicy now reports the final destination and headers, after plugin rewrites and queued rebuilds, through an init symbol. Auto-pause observers count a response only when that wire credential and destination match the captured attempt; a Codex WebSocket send, which has no HTTP final-send report, counts only while no plugin upstream rewriter is registered. Native Chat, native Messages and the Responses providerFetch path all report the final send.
…ttribution The Responses oauthDispatch send now attaches its auto-pause observer's final-send report. A send without an HTTP report (Codex WebSocket) counts only when no upstream rewriter was registered at capture and the rewriter registration epoch is unchanged at the response, so a rewriter that registers or unregisters mid-send can no longer make a rewritten dial look attributable.
A providerFetch observer and the dispatchOverride observer inside it see the same physical response. Count it once per account key, and never let an observer without a final-send report attribute a response that another observer reported as sent with a different credential.
A loadConfig() snapshot now adopts only the disk revision; fingerprints and local generations start there only for providers not yet observed, so a detached snapshot that differs from the live config can no longer keep invalidating in-flight fences. API-key quota rows also keep the pause flag and known pause details, matching the plain key listing.
Track the last observed disk fingerprint separately from the live one. A loadConfig() snapshot advances the local generation when the disk policy differs from the previously observed disk policy, so an external change followed by a restore still invalidates older attempts, while a snapshot that merely differs from the live config does not.
… reclaim deleted-provider epochs Codex pool and main-pool attempts now own a response only when the final wire request carries the captured bearer, the context's chatgpt-account-id and unchanged organization/project selectors, at capture and in the final-send report. Policy revision records are kept only for providers that exist after disk-only preservation, observed records for deleted providers are dropped, and every new record or advance draws from one process-wide generation counter so a recreated provider cannot revive an older fence.
…ale capture Once a config path has been observed, captureAccountAutoPauseFence no longer seeds a missing provider from the caller's config object; only load, reload or save adoption can enroll it. A capture taken from a stale live config while its provider is observed deleted therefore cannot survive an identical restore.
…aders With opt-in gzip request compression enabled, the auto-pause final-send report sees the compressed send's headers (content-encoding set, stale content-length removed) and still attributes a refusal to the credential actually sent.
Summary
Closes #7101.
Add
providers.<name>.accountAutoPause, default off, with a configurable consecutive permanent-refusal threshold (default 2, range 1–100). Exact account-scoped HTTP 401/403 organization/OAuth, subscription and plan refusals can pause the sending OAuth account, Codex pool account or API-key entry. Transient/network failures, quota windows, 429/5xx, and model/request permission errors never trigger the policy.Reuse durable OAuth
paused, Codex pause ids and key-pooldisabled; record a fixed reason and timestamp, show them in account listings/dashboard badges, and clear them throughocx account resumeor dashboard Resume. Scalar API keys acquire a pool row on pause. Count decisions serialize per account, reset on successful HTTP responses, and fence credential replacement, re-login, resume and policy opt-out. Counters are process-local and reset on restart; pauses persist.A threshold-triggering refusal can retry an eligible sibling inside the same logical request through the existing funded recovery owner. Shared and provider-specific send ceilings, fixed Codex selectors and uploaded-file ownership remain binding.
Policy-change fence (this refresh). A refusal from a request sent under an earlier policy can no longer pause an account after the policy was disabled and re-enabled, its threshold changed, the provider disabled, or its endpoint changed.
src/config/account-auto-pause-revisions.tskeeps a per-provider UUID revision (accountAutoPausePolicyRevisions) that onlypersistConfigUnlockedmints, from the literal pre-write file, when a tracked provider's effective policy fingerprint changes; caller-supplied values are overridden and providers that never opted in leave no map. Each attempt captures revision, local generation, threshold and route; OAuth, API-key and Codex pauses commit only while both the in-memory and the fresh persisted fence still match. Live reconciliation and disk-to-disk changes seen by any load advance the local generation; a detached snapshot that only differs from the live config does not. Revision records exist only for providers present after disk-only preservation: deleting a provider reclaims its record (persisted and observed maps stay bounded), every new record or advance draws from one process-wide generation counter, and a stale live config can no longer re-enroll an observed-deleted provider, so a recreated provider never revives an older fence. The OAuth automatic publication runs its final check and auth write under the config mutation lock; contention means no pause and the upstream response is preserved.Wire ownership (this refresh). A refusal counts only for the credential that actually reached the upstream.
sendWithConnectionPolicyreports the final destination and headers (after plugin rewrites and queued rebuilds) to the attempt's observer; native Chat, native Messages, the ResponsesproviderFetchpath andoauthDispatchall report. A Codex WebSocket send, which has no HTTP report, counts only when no upstream rewriter was registered at capture and the rewriter registration epoch is unchanged at the response. Nested capture sites that see the same physical response count it once per account, and an unreported observer never attributes a response another observer reported. For Codex pool and main-pool accounts, ownership binds the bearer, thechatgpt-account-idworkspace and any organization/project selector, so a rewrite that keeps the bearer but changes or deletes the workspace never pauses the selected account (including__main__).This refresh also resolves all review threads: key pause fields survive dual-auth OAuth upserts (sanitizer extracted to
src/oauth/api-key-pool-preserve.ts), a full provider save that omitsaccountAutoPausekeeps the stored policy, the canonicalopenaiprovider accepts the policy as an operator overlay, non-object pause bodies return 400, malformed pause display metadata degrades per entry, CLI pause errors/JSON match the other branches and quota rows keep pause fields, and dashboard tooltips are translated (11 locales) with locale dates, accessible Resume names and a distinct partial-success notice.Umbrella #3375 is outside this PR.
