Avoid advancing rotation for an undispatched account
When pool.kernel is enabled with the generic OAuth round-robin strategy and a combo-level 429 arrives after the request has exhausted its account-rotation/send budget, this call uses rotateGenericOAuthAccountOn429 only to test whether a healthy sibling exists and then discards the returned account. That rotator calls the mutating pickRoundRobinAccount, so it advances/sticks the pool cursor even though no request was dispatched to that account; subsequent requests can therefore skip or remain sticky to the wrong account. Record the failed account's cooldown and check sibling eligibility through a non-committing path (for example, a peek/pure eligibility helper) instead.

Use a separate recovery kind for Codex 402.
The status sealing is correct, but oauth-account-429 remains in recoveryKinds. causeForRecoveryKind maps that value to rate-limit, so request metrics report a Codex 402 quota recovery as rate_limit instead of quota. The dashboard also renders the value as an OAuth 429 recovery.
Add a value such as oauth-account-402, map it to quota-exhausted, and add it to the cooldown and dashboard consumers.

Preserve provider attribution for identical combo targets.
When two combo targets use the same model ID and context window, the local refusal details are identical. The final response exposes only generic error fields and the repeated message. It does not identify the refused provider or target. The existing test uses different windows, so it does not cover this case.
This is a narrow diagnostic improvement, not a material workflow failure. Add the target key as optional attribution.

Hosted GitHub Actions were unreliable and the maintainer ruled out local test runs for this round, so the evidence is remote and exact-tree:

Summary
Preserve healthy OAuth combo targets when one account is refused, make zero-send combo refusals actionable, keep private replay ownership on the account that actually served the turn, and report recovery/route-health evidence from physical sends.
Carried as separate commits, all by @vadymhimself:
combo_unavailable.No assigned source PR is excluded. Maintainer #7153 is outside this lane and is not carried. The stacked-parent conflict preserves #7191 and adds only #7204's unique intent. Structure documentation covers the combined behavior; no GUI or CLI-capability changes.
Verification
Local tests, typechecks, builds, lint and package installation were not run, as explicitly required for this round. All execution below is on lidge, using Bun 1.4.3. Hosted required checks are not reported for this PR head; these are remote lane results, not hosted-CI certification. The coordinator owns final security acceptance and integration.
