Summary

Fix account recovery and usage reporting without changing healthy request routing. A combo's eligibility-only sibling check no longer advances its round-robin pool; Codex HTTP 402 account moves get a quota-specific Logs label and metric classification; refused-then-served usage rows include their measured cost while client cancellations remain unknown. Oversized Anthropic server-tool input fragments stay within the existing buffer cap.

Carried with original authorship preserved, in this order:

Structure contracts and the dashboard guide describe the combined behavior. No file-size caps or import-cycle baselines were raised; no test files were added.

Excluded: #7205 by @vadymhimself. Re-reviewed current source head f6e0f74c5e7cf83f43051ff7a22bf43168459aef, including occurrence anchors ee3d9727e, appendPreface ordering 3f66646a5, and the newer legacy fetch_result compatibility fix. A concrete counterexample remains: an earlier assistant message has string content Bun shipped., followed later by an assistant array containing two identical server searches, repeated Bun shipped. text anchors, identical Read calls with distinct IDs, and interleaved signed thinking. Capture skips the earlier string message at inbound-content-options.ts:104 and assigns the later runs text:Bun shipped.#0 and #1. Translation does preserve the string at inbound.ts:301-304, so adapter replay:742-746 consumes run 1 in the earlier turn and run 2 before the later turn's first text. appendPreface preserves the chosen run's local order but cannot repair its wrong owner. The repetition regression:303-312 uses array-form earlier content and misses this case. This is a static counterexample, not a runtime reproduction. Capture/replay must enumerate identical normalized anchors or carry source message/block provenance before this PR can be carried.

#7232 is independent of #7205: it caps input_json_delta accumulation before result parsing in both implementations. Its dev-side regression passes in this bundle; #7205's result/replay changes are absent.

Exact tested and pushed head: b158e8cc10545ad3827d5ec0557301f65dbaa91b.

Local tests, typechecks, builds, and lint were NOT RUN by delegation rule. All agent-run executable validation is on lidge, Bun 1.4.3; hosted CI is reported separately below.

  • VC-b158e8c-focus: ssh lidge '~/bin/ocx-lane-launch.sh 61 b158e8cc10545ad3827d5ec0557301f65dbaa91b VC-b158e8c-focus tests/claude-integration/claude-translated-server-tools.test.ts tests/oauth/generic-oauth-combo-account-cooldown.test.ts tests/lib/failure-stage-model.test.ts tests/responses/responses-account-label.test.ts tests/usage/usage-partly-measured-cost.test.ts tests/ci-workflows/import-cycle-ratchet.test.ts tests/test-layout.test.ts tests/test-layout-tooling.test.ts tests/ci-workflows/file-size-ratchet.test.ts tests/ci-workflows/structure-ssot.test.ts' — typecheck exit 0; test exit 0, 147 pass / 0 fail across 10 files.
  • Remote setup completed frozen installs and the GUI production build successfully.
  • VC-46117b3-extra: remote bun run privacy:scan and GUI bun run lint:i18n both exit 0; GUI bun test --isolate tests/i18n-locales.test.ts tests/logs-cost-lower-bound.test.ts tests/logs-cost-plain-dollar.test.ts exits 0, 53 pass / 0 fail.
  • VC-b158e8c: ssh lidge '~/bin/ocx-lane-launch.sh 2 b158e8cc10545ad3827d5ec0557301f65dbaa91b VC-b158e8c' — completed with typecheck exit 0; test exit 1; 49,278 pass / 29 fail / 147 skip, 666 seconds including lock wait. ssh lidge '~/bin/ocx-lane-cmp.sh VC-b158e8c R2-L4-b4d6376' reports 0 new failures against primary baseline (49,272 pass / 29 fail / 147 skip). Its tree 7e7c71321b6689c12d7e7acaa0dcd164b23bd809 is identical to base 4ed45e9516. ssh lidge '~/bin/ocx-lane-cmp.sh VC-b158e8c dev-7a42860' also reports 0 new failures (one baseline failure no longer observed). No baseline test-lane labels are missing. The full suite is not green; its 29 failures match the primary baseline.
  • Static union review: all 11 locale dictionaries contain the new key; no production imports changed; file-size and import-cycle baselines unchanged; no new tests require layout registration; edited structure documents are within their 600-line budget. git diff --check passed.
  • No live provider request or new browser UI smoke was run. The screenshot is supplied evidence from fix(codex): record a 402 account move as a quota recovery #7231. The final-head hosted documentation build passed.

Hosted review follow-up: commit b158e8cc10 corrects structure/transports/responses.md:437 to distinguish 402 quota recovery from 429 rate-limit recovery. The routing-analytics cost comment describes retained pre-existing behavior: src/routing/analytics.ts:162-175 and its aggregate-cost estimator are unchanged from base 4ed45e9516; this carry fixes usage-summary pricing only. Broadening routing analytics is deferred to the coordinator.

The final-head hosted CI run passed all four test shards, docs build, structure gate, storage policy, API usage, keyring, npm-global and Docker smokes. Its gates job failed four assertions in gui/tests/models-keep-native-v1-placement.test.ts. Running that exact file on lidge against primary baseline R2-L4-b4d6376 reproduced 2 pass / 4 fail. The test, gui/src/pages/Models.tsx, and gui/src/styles-models-workspace.css are unchanged from base. This is a retained base failure, not a passing CI check.

The superseded VC-46117b3 full run had 49,277 pass / 30 fail / 147 skip: one additional five-second timeout in plaintext-v2-agent-messages-server. VC-b158e8c-timeout reran that file with the required union guards on the final head: typecheck exit 0, 134 pass / 0 fail across 6 files. It did not recur in the final full run. These results support a transient timeout, not a source fix.

The coordinator retains merge, closure, and the explicit maintainer security-review decision for the account-pool changes.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults; maintainer security review remains a coordinator gate before merge.

Summary by CodeRabbit

  • New Features

    • Request logs distinguish Codex account-quota recoveries after HTTP 402 from rate-limit recoveries after HTTP 429. Quota recoveries are included in recovery metrics and labeled across supported languages.
  • Bug Fixes

    • Usage summaries account for measured attempts even when other attempts are unreported. Client-cancelled requests and certain incomplete upstream attempts are not automatically classified as having spent nothing.
    • Oversized streamed server-tool inputs are capped, and excess input is omitted from client output.
    • Checking for an eligible alternate account no longer advances account rotation.

📝 Walkthrough

The changes distinguish Codex HTTP 402 quota recovery from HTTP 429 rate-limit recovery, add non-committing OAuth sibling probes, revise usage-cost classification for measured attempts and HTTP 499, and limit streamed Anthropic server-tool input to 4,096 characters.

Changes: Codex Account-Quota Recovery

Layer / File(s) Summary
Classify HTTP 402 account moves
src/usage/telemetry-contract.ts, src/server/responses/core-codex-account.ts, src/lib/request-failure-model.ts, tests/lib/failure-stage-model.test.ts
Codex account moves now record HTTP 402 as oauth-account-402, while HTTP 429 remains oauth-account-429. The new recovery kind maps to quota-exhausted.
Record and display quota recoveries
src/routing/analytics.ts, gui/src/pages/Logs.tsx, gui/src/i18n/*, tests/responses/responses-account-label.test.ts, structure/dashboard-and-usage.md, structure/transports/responses.md, docs-site/src/content/docs/guides/web-dashboard.md
Analytics and request metrics recognize quota recoveries. Logs map the new kind to localized labels. Tests and documentation describe the recovery record and quota metrics.

OAuth Account Probing

Layer / File(s) Summary
Probe eligible OAuth siblings without advancing the pool
src/oauth/generic-account-failover.ts, src/server/responses/core-combo.ts, tests/oauth/generic-oauth-combo-account-cooldown.test.ts, structure/providers/generic-account-pool.md
The rotation functions accept a commit flag. Combo sibling checks pass false to peek at the round-robin pool; the default remains committed selection. The test checks that the sibling is not sent a request and the pool position does not remain on it.

Usage Cost Accounting

Layer / File(s) Summary
Classify priced, unpriced, and unmetered requests
src/usage/summary.ts, tests/usage/usage-partly-measured-cost.test.ts, structure/dashboard-and-usage.md, docs-site/src/content/docs/guides/web-dashboard.md
A measured attempt prevents a row from being counted as unmetered, even when aggregate usage is unreported. HTTP 499 no longer qualifies as proof that an attempt spent nothing. Tests cover measured retries, client cancellations, missing prices, and rows containing only known-zero refusals.

Anthropic Server-Tool Input

Layer / File(s) Summary
Limit streamed server-tool input
src/adapters/anthropic.ts, tests/claude-integration/claude-translated-server-tools.test.ts, structure/providers-and-adapters.md
The adapter retains at most 4,096 characters of streamed server-tool input. A test checks oversized input, and documentation describes the limit.

For requests combining a priced attempt with a measured attempt whose model has no price, Usage & Logs can present only the known portion as a priced total, understating the displayed estimate. This does not change provider billing, but the cost-reporting contract should be resolved before merging.

P2 Badge Apply known-zero retry pricing to routing analytics

When a successful request has an unreported pre-output refusal followed by a measured send, this branch now prices it in /api/usage, but src/routing/analytics.ts::successCostUsd still passes the aggregate usageStatus: "unreported" to estimateRequestCost, which returns null. Consequently, /api/routing-analytics continues to underreport cost and price coverage for the same now-known-cost request; use the attempt-aware computeEntryCost result or apply the same known-zero-attempt logic there.

Fixed on source PR #7233 in 52b623b (routing analytics successCostUsd now reuses computeEntryCost, so a usage-less 429 before a measured send is priced in /api/routing-analytics as in /api/usage; test: tests/routing/routing-analytics.test.ts "a refused retry before a measured send is priced, matching /api/usage"), ready to cherry-pick.

P2 Badge Update the Responses contract for 402 recoveries

This changes Codex HTTP 402 moves to oauth-account-402, but the applicable structure/transports/responses.md:437 contract still says Codex quota-recovery moves record oauth-account-429. Update that mapped transport document so it distinguishes 402 quota recovery from 429 rate-limit recovery instead of leaving the architecture contract contradictory.

@vadymhimself Thanks for the verification details. The successful build at 9ea4be7a6 confirms source PR #7231 builds, with only the pre-existing chunk-size warning.

That does not confirm the combined state in #7236, so this PR’s GUI build remains unverified. No change to the French translation is needed.

Maintainer review and security disposition before landing (head 069c1da):

  • Independent review passed all carried commits. The one finding, usage pricing, was resolved by keeping dev's partial-cost contract and pinning refusal invariance: a row with a leading usage-less refusal must summarize identically to the same row without it. The attempted stricter pricing change was reverted because it contradicted existing dev tests.
  • Security: the 402 move is recorded as quota recovery within the existing dispatch permits and send bounds. Cooldown fencing keeps the physical credential/destination ownership checks from fix: charge key cooldown to the credential sent #7240. The server-tool input buffer is bounded at 4,096 code units per incoming fragment. Usage accounting keeps client cancels at unknown cost. No blocker found.
  • Validation: the lidge full suite shows 0 new failures against the dev baseline, and the routing timeout passed on 3 head reruns and the baseline. mini Windows focused: 48/48. Hosted PR CI is green on the exact head (run 38920919111).
