feat(apl): execute fetch before policy decisions - #287
Authorization sometimes needs request-time facts that are not present in the inbound request, such as an account state or risk result. Those facts must be acquired at an explicit point in policy execution and must be visible to every later authorization step in the same invocation.
This PR implements issue #241 on top of the typed metadata.fetch hook introduced by #281.
What changes
APL effect and compilation
Adds fetch(name) as a dedicated APL effect.
Allows it only in authorization.pre_invocation.
Rejects fetches inside parallel, because publication mutates request-scoped policy state.
Rejects ambiguous instance names unless they match [A-Za-z_][A-Za-z0-9_]*.
Rejects more than one syntactic fetch of the same source per effective route, including:
mutually exclusive conditional branches;
nested effect trees;
duplicates introduced by global/default/group/route stacking.
Load-time source validation
Each referenced source must:
exist in the plugin registry;
declare the metadata.fetch hook;
use mode: sequential, so its typed result is awaited and may modify the payload;
use on_error: fail or on_error: ignore.
Invalid source configuration now fails during policy load instead of becoming a request-time denial.
Runtime execution
Resolves each fetch to a dedicated metadata.fetch dispatch-plan entry.
Invokes only the named source through the normal PPE executor, preserving capability filtering, timeout handling, and route-level overrides.
Validates the returned AcquiredFacts before applying error policy.
Stamps trusted source provenance from the registered plugin name and kind.
Installs the immutable record in request extensions.
Refreshes the APL attribute bag immediately, so the next predicate or PDP call sees metadata.<instance>.* in the same invocation.
Failure semantics
on_error: fail:
stops authorization;
publishes no metadata record;
preserves the normalized source error, such as metadata.upstream_error, as the machine-readable violation code.
on_error: ignore:
publishes only the validated, source-owned error record;
exposes $outcome and $error_code to later policy steps;
publishes no partial acquired value;
continues evaluation.
A conditional fetch whose predicate is false publishes no instance. This remains distinguishable from an ignored source error.
The change adds named metadata fetching to APL pre-invocation policies. The runtime resolves and invokes configured metadata sources, validates and installs acquired facts, and makes returned attributes available to later policy steps. Fetch sources are subject to route, phase, duplicate-use, and error-policy validation.

Adds the typed metadata.fetch hook and request/result contract. Dispatch selects the named source, and the hook is registered and documented.
Metadata validation and publication crates/ppe-core/src/extensions/*, crates/ppe-core/src/executor.rs
Adds acquired-fact validation and authorizes metadata publication for qualifying APL builtins after extension validation succeeds.
APL fetch syntax and evaluation crates/ppe-apl-core/src/{attributes,evaluator,parser,rules,step}.rs, docs/content/apl/*
Parses and compiles fetch(name), invokes the source during pre-invocation evaluation, and adds successful results to the attribute bag. Rejects invalid names, unsupported phases, duplicates, and fetches in parallel effects.
Route source resolution and validation crates/ppe-apl-runtime/src/dispatch_plan.rs, crates/ppe-apl-runtime/src/visitor.rs, crates/ppe-apl-runtime/tests/{cmf_invoker_dispatch,fetch_metadata_e2e}.rs
Resolves fetch sources and route overrides. Validates source registration, hook compatibility, error policy, execution mode, duplicates, and reachability.
Metadata invocation and policy input refresh crates/ppe-apl-runtime/src/{cmf_invoker,route_handler}.rs, crates/ppe-apl-runtime/tests/fetch_metadata_e2e.rs
Invokes the selected hook, validates and installs accepted facts, returns attributes to APL, and detects metadata changes in extensions. End-to-end tests cover success, errors, and conditional fetches.

Explanation
Issue [#241] requires fetch(name) in authorization.pre_invocation, named-source validation and dispatch, duplicate rejection, result validation and installation, policy-input refresh, and defined fail, ignore, and conditional-skip behavior. The changes add the APL parser and compiler support, route dispatch entries, source and mode validation, recursive duplicate detection, typed metadata.fetch dispatch, framework-controlled validation and provenance, extension installation, APL bag refresh, and metadata-only extension propagation. The end-to-end tests cover later predicate and PDP visibility, failure and ignore behavior, conditional skips, incompatible sources, duplicate stacking, unsupported modes, and invalid error policies.

Explanation
The metadata hook contract, capability restrictions, executor support, documentation, and tests support the implementation of [#241] and its dependency contract. The changes do not add metadata/http network behavior, post-invocation acquisition, or evaluator-specific structured projections. No unrelated change is demonstrated.

Count fetch sources as referenced in the unreferenced-override lint.
collect_plugin_names excludes Effect::Fetch, but RouteDispatchPlan::build applies config, capabilities, and on_error overrides while building metadata.fetch entries. A valid fetch-source override can therefore trigger the false “override has no effect” warning.

Replace the _ => {} arm in collect_fetch_names with an explicit variant list.
The sibling methods count_elicits and contains_mutation match every Effect variant. This method uses a wildcard instead. Suppose a future variant holds nested effects. The wildcard will skip it without a compile error. The duplicate-fetch check in CompiledRoute::duplicate_fetch_name and the post-invocation fetch rejection in compile_apl_blocks would then miss fetches inside it. Both checks enforce the fetch-once rule. List the leaf variants so that a new variant fails to compile until someone classifies it.

Update Effect::collect_fetch_names to replace its wildcard match arm with
explicit arms for the leaf variants that contain no nested effects, following
the exhaustive classification in count_elicits and contains_mutation. Preserve
the existing recursive handling of fetch-bearing variants so any new Effect
variant requires explicit classification at compile time.

Require a registered metadata.fetch handler at load time.
If a factory registers account without a handler, engine.get_plugin("account") still succeeds. The declaration check also accepts hooks: [metadata.fetch]. The dispatch plan then has no metadata entry, so every request that executes fetch(account) is denied instead of rejecting the configuration at load time. Check the registered hook entries as well as the declaration and mode. (raw.githubusercontent.com)

Why
Authorization sometimes needs request-time facts that are not present in the inbound request, such as an account state or risk result. Those facts must be acquired at an explicit point in policy execution and must be visible to every later authorization step in the same invocation.
This PR implements issue #241 on top of the typed
metadata.fetchhook introduced by #281.What changes
APL effect and compilation
fetch(name)as a dedicated APL effect.authorization.pre_invocation.parallel, because publication mutates request-scoped policy state.[A-Za-z_][A-Za-z0-9_]*.Load-time source validation
Each referenced source must:
metadata.fetchhook;mode: sequential, so its typed result is awaited and may modify the payload;on_error: failoron_error: ignore.Invalid source configuration now fails during policy load instead of becoming a request-time denial.
Runtime execution
metadata.fetchdispatch-plan entry.AcquiredFactsbefore applying error policy.metadata.<instance>.*in the same invocation.Failure semantics
on_error: fail:metadata.upstream_error, as the machine-readable violation code.on_error: ignore:$outcomeand$error_codeto later policy steps;A conditional fetch whose predicate is false publishes no instance. This remains distinguishable from an ignored source error.
Malformed records, missing results, dispatch failures, and provenance/install failures always fail closed.
