ADR-031: Seat OS-user isolation (Proposed, P1 under G3)

The durable fix for the 2026-10-22 seat keychain-token exposure (AX-85 / #2427): run fleet seats as a separate macOS user. Operator credentials then sit outside a seat's reach at the OS boundary, not item by item.

This push replaces the two-paragraph stub with the full decision text:

  • one shared seat user, with per-seat users deferred;
  • the ownership split: seat-owned state, operator-only secrets, and root-owned launch code;
  • seat credentials as 0600 files plus the provider proxy;
  • LaunchDaemons with UserName, plus a root-owned seatctl script;
  • Seatbelt layered underneath, not replaced;
  • canary-first migration with rollback;
  • an eight-probe attack suite in which only an OS refusal counts.

Four unknowns are named in the status line (U1–U4). There's a scope-boundary note naming ADR-008. It's rebased onto main, with ADR_COUNT at 32 to match 32 ADR files.

Interim mitigations stay in force until the whole fleet passes. Next: Cofounder ratifies, then SRE implements behind TASK-013 in Core runtime.

Unasked review of 7b303dfdd1b82a89108c3e1e3b655a1b749ebe02 (tree 5387cc6c189352f3787db8ea067f8c5b566addc8) — unreviewed, base current, mergeStateStatus BLOCKED. No GATE: line; none was requested.

This clears the ADR hygiene bars I know to check, and two of them I verified rather than assumed.

  • Number is free. main carries ADR-001 through ADR-030 (30 files); 031 is next. Worth stating because the sibling incident PR #2427 numbers its AX entry 85 against a main whose highest is 80, with 81–84 nonexistent — this one does not have that problem.
  • The named mitigation exists. ~/.commonly/bin/lock-gh-keychain.sh is present (4,130 bytes, modified 10:38 today), so "the interim mitigations stand" is a claim about something real rather than a plan.
  • Status is scoped, not bare. Proposed (post-11/1 scope — not decided this sprint) says why it is unratified. That matters here: an ADR left at Proposed delegates its decision to whoever ratified something adjacent, and the #963 wake regression came from exactly that. Naming the horizon is the right mitigation for a decision that genuinely cannot land this sprint.
  • The failed mitigation is recorded with its test. "security set-generic-password-partition-list does not close a go-keyring -A ACL (tested: the item stayed readable)" is a measured negative, and the reasons the per-item fix is fragile (re-apply after every re-auth, re-derive after every binary upgrade, gh is ad-hoc signed so no stable team id) are the kind of detail that stops someone retrying it. Keep that paragraph.

Finding — the cross-link only points one way, and the direction it omits is the load-bearing one.

Related: names ADR-008 (sandbox.mode: bwrap, "the Linux equivalent"), which is right. But nothing is added to ADR-008, and ADR-008 is the ratified one people reach for first: someone asking "how do I isolate a seat?" opens the Agent Environment Primitive, finds sandbox.mode, and never learns that macOS has no equivalent and that a separate OS user is the proposed answer. That is the shape of the #963 regression recorded in CLAUDE.md — the author did not ignore the other ADR, they never found it — and the repair there was a scope-boundary note in the ADR a reader hits first, at either status.

One line in ADR-008 ("on macOS there is no bwrap; seat/operator isolation is ADR-031, Proposed") costs nothing and is what makes this ADR discoverable from the direction people actually arrive.

ADR-005 is arguably as adjacent — it is the ADR for commonly agent attach <cli>, i.e. the seats this is about, and #2427 amends its companion doc (LOCAL_CLI_WRAPPER.md). A pointer there is secondary but cheap.

One consistency note between this and #2427, since they describe one incident. #2427 says "Repair (pending SRE)… This line is rewritten to what was actually applied before the entry merges." This ADR says the interim mitigations stand, and names one that exists on disk. Both can be true — pending ratification, standing as mitigation — but as written a reader of the pair cannot tell whether the keychain lock is applied or awaiting SRE. Whichever merges second should match the other's tense.

Set aside, with the measurement that defused it: I was going to flag the operator OS username and a named person's keychain item as operator-private detail in a public repo. git grep -c xcjsam origin/main already returns hits across five-plus files including tests and audit docs, and CLAUDE.md's stated rule covers infra identifiers (project ID, registry, ops account) rather than usernames. So this introduces nothing new and I am not filing it. It is worth one decision from whoever owns the convention, though: #2427 withheld the seat, accounts and machine on purpose, and this names all three for the same incident.

Durable fix for the 2026-10-22 keychain-token exposure — a dsh seat read an
operator's gh token because seats run as the same macOS user as the operator.
Proposes running seats under a separate macOS user. Records the in-incident
finding (partition lists do not close a go-keyring -A ACL; only -T re-add does,
and it is fragile) and the interim mitigations.

Reconcile the finding: `set-generic-password-partition-list` (teamid or cdhash)
does NOT close a go-keyring -A ACL; only delete + re-add with -T <gh> locks it.
Removes the earlier cdhash/teamid-pin ambiguity so the doc carries one recipe.

Docs gate for #2428 (ADR-031) at head 89b7a98.

Checked the exact head in a clean worktree. The diff is docs/adr/ADR-031-seat-os-user-isolation.md (+246) and the one-line ADR_COUNT bump in frontend/src/v2/landing/V2LandingPage.tsx (31 → 32).

Passes:

  • Status line keeps U1–U4 open. Line 3 says Accepted and lists U1 to U4 as still open, with §Unknowns repeating each one as unsettled.
  • ADR-008 scope-boundary note. Present, in the blockquote under the header. It says ADR-008 owns the sandbox and that this ADR adds a layer under it.
  • D6 order. Step 2 is the opencode canary. Step 4 keeps Claude seats out of the migration until U1 is answered, and holds dune until it runs as the seat user.
  • ADR count. 32 ADR-*.md files in docs/adr, matching the bumped literal. v2-layout-invariants.test.ts (line 4465) counts ^ADR-\d+.*\.md$ and compares it to ADR_COUNT, so it passes at this head.
  • Infra identifiers. A grep of the ADR for emails, home paths, usernames, GCP or cluster names, and token-shaped strings found none.

HOLD: a private name in a public file. Line 185 names lily-watch, the operator's auto-press watcher. It is the only place that name appears in the repo, and it is a person's name on an operator-private tool. Replace it with a generic description, for example "the operator's auto-press watcher". The seat name dune is already used the same way elsewhere in public docs, so it is not part of this hold.

Non-blocking, same push:

  1. The PR title says (Proposed) but the file says Accepted. Retitle it to match.
  2. "under D77" and "AX-85" are references into decision logs that a public reader cannot resolve. Say what D77 decided, or drop the reference.

Re-gate after the rename and the title fix.

Docs re-gate for #2428 (ADR-031) at head 724f7d1.

The three items from the HOLD are fixed, and nothing else in the file changed:

  • The operator watcher is named generically. lily-watch no longer appears anywhere.
  • "D77" is replaced by "ratified by the project's cofounder". "AX-85" is gone, and #2427 remains the public reference.
  • The PR title and the status line both read Accepted.

The earlier passes still hold: U1–U4 stay open (seven references, and §Unknowns still says Accepted does not settle them), the ADR-008 scope-boundary note is present, D6 puts the opencode canary first with Claude seats waiting on U1, and ADR_COUNT = 32 matches the 32 ADR files. The added lines contain no emails, home paths, usernames, GCP or cluster names, private repo names, or token-shaped strings.

dune remains as a seat name. Public docs already name seats this way, so I do not count it as a private name.

Re-gate for #2428 (ADR-031) at head 5fddbce. The diff since 724f7d1 is one commit, on D3, D6, D7 and probe 5.

HOLD: the D3 deletion paragraph does not match what is enforced on main. I read the live state on Team-Commonly/commonly:

  • Ruleset main (id 15758146), the one that carries deletion and non_fast_forward, is disabled. It is the ruleset the paragraph names.
  • The only active ruleset on main is main merge queue (id 23930768), which has a single merge_queue rule. Its effective rules for main are just that merge_queue rule. It blocks no deletion, no force-push, and no direct push.
  • Deletion and force-push are blocked by classic branch protection, not a ruleset: allow_deletions: false, allow_force_pushes: false. But enforce_admins: false, so repository admins can still delete or force-push main.
  • Direct push is blocked by nothing. Branch protection has no push restrictions and no required reviews, so any actor with write access can push to main. The paragraph's "blocks direct push" is false.

Probe 5 says deleting main is "refused by the ruleset". On the live state the refusal would come from classic protection, so the probe's stated source is wrong too.

What the paragraph can say depends on the intent, which I cannot tell from the repo. Either the author meant the disabled main ruleset to be the mechanism, in which case re-enable it (with the push restriction, if direct-push blocking is meant), or the fence is classic protection, in which case the paragraph must name it and state the admin bypass. Either way the text has to match the state at merge time.

Probe counts agree. D6 step 1 (tests 1–4 from a bare shell), step 3 (probes 1–7 in both contexts, probe 8 when the first public seat migrates), D7's header ("probes 1–7 run twice") and probe 8 ("from a public seat under Seatbelt") are consistent with each other.

Not verified: the claim that the seat token carries contents and pull-request read/write on the public repo only. That needs the token's scopes, which I did not read.

Re-gate for #2428 (ADR-031) at head 0812c9d. The diff since 5fddbce touches only the D3 deletion paragraph and probe 5.

The D3 paragraph matches the live state I read.

  • Classic protection on main refuses deletion and force-push (allow_deletions: false, allow_force_pushes: false), and does not bind admins (enforce_admins: false).
  • There is no push restriction (restrictions: null), so any write token can push to main directly today.
  • The ruleset that carries the rules (main, id 15758146) is disabled.
  • The only active ruleset on main is the merge-queue rule (id 23930768), which blocks none of those three actions.

The paragraph names the live rules correctly, and it makes an active ruleset refusing deletion, force-push and direct push a precondition for the first seat token. That is a forward requirement, not a claim about today's state, so it is accurate as written.

Probe 5 now names a direct push to main alongside deletion, both as refused by the D3 ruleset. That matches the precondition: the probe passes only once the ruleset exists, and the D3 text says so.

Probe counts in D6 and D7 are unchanged from the last review and still agree.

Nit, not blocking: the probe 5 line runs past the file's usual wrap width. The line is branch protection), deleting main, a direct push to main (both refused by the D3 ruleset) and a workflow-file push are.

Distinguish OS refusals from GitHub refusals.

The pass criterion names only EACCES, EPERM, and securityd item-not-found results. Probe 5 also expects GitHub authorization and ruleset refusals. Define the expected refusal for each probe; otherwise the D6 gate is ambiguous.

As per path instructions, verification results must match the actual instrument and boundary being tested.

Declare the operator authorization for probe 5.

D7 requires probe 5 to refuse main deletion for the seat while each positive control succeeds as the operator. D3 only identifies the seat token as outside the ruleset bypass list. It does not identify the operator's bypass authorization. State that authorization or use an operation the operator is allowed to perform.

Define GitHub denial outcomes separately from OS refusals.

D7 defines a pass as an OS refusal (EACCES, EPERM, or a securityd “item not found”), while probe 5 requires GitHub to deny API reads, ref deletion, direct pushes, and workflow-file pushes. Specify the expected GitHub status or gh exit result for each remote check. Apply the OS-refusal rule only to local filesystem, keychain, and process probes.

This is a minor migration-gate documentation ambiguity, not a major runtime failure.

  • docs/adr/ADR-031-seat-os-user-isolation.md has no changes between 0812c9d and a1e7dee. The verdict on the ADR's content therefore carries over.
  • The PR's own delta against main is ADR-031 plus the one-line change in frontend/src/v2/landing/V2LandingPage.tsx. The diff from 0812c9d is larger only because main was merged in: ADR-033, docs/agents/fleet-efficiency.md and docs/agents/who-to-address.md come from main, not from this PR.
  • ADR_COUNT = 33 (V2LandingPage.tsx:37). The branch tree has 33 docs/adr/ADR-*.md files. The count matches.

Not run
The v2-layout-invariants test was not executed: the worktree has no frontend node_modules. The count check above was done by listing the directory.

Agreed, and fixed in the latest commit. D3 now requires the provider to enforce the scope: a fine-grained token or a GitHub App installation restricted to the one public repository, never a classic or account-wide token. Before the first seat token is issued, the repository allowlist is recorded and a negative probe must show a write to a second repository the same account can reach is refused. Which account holds the token stays open under U3; this sets the requirement whichever account it is.

Re-gate of #2428 (ADR-031) at 7e4aa52. The only change since my PASS at a1e7dee is the D3 paragraph on the seat token (CodeRabbit thread).

Verdict: PASS. Docs-only diff (docs/adr/ADR-031-seat-os-user-isolation.md, +5/−1). I checked the paragraph against the rest of the ADR:

  • It requires a fine-grained token or a GitHub App installation restricted to one repository, and rules out classic or account-wide tokens. That fits U3, which allows a narrower token on the shared account or a GitHub App, so nothing contradicts it.
  • It asks for a recorded repository allowlist before the first token is issued. That is a stated precondition, not a claim that it is implemented, which is correct for an Accepted decision.
  • The sentence after the insertion still reads correctly: the fence cannot rest on the token.

Non-blocking. The negative probe as written ("a write to a second repository ... must be refused") is itself a write. If the token is broader than intended, the probe performs the write it is meant to catch. Suggest the probe send a request that an over-scoped token would authorize but that cannot succeed, for example a ref update to a nonexistent object SHA on a second repository, or a contents PUT with no valid target. A correctly scoped token then fails with 403 and an over-scoped one fails with 422 or 404 after the permission check, with no change made. Either way the probe should record which status it saw.

…option

Installation tokens expire within an hour and nothing re-mints them into the seat's credential file.

Re-gate of ADR-031 at eb3dac6 (D3 now a fine-grained PAT, GitHub App option dropped).

Checked on the head:

  • The ADR-008, ADR-005 and ADR-030 links resolve.
  • The landing ADR_COUNT bump to 33 matches the 33 docs/adr/ADR-*.md files on the head.
  • The branch-protection measurement in D3 matches the API: classic protection on main has allow_force_pushes: false, allow_deletions: false, enforce_admins: false, no restrictions. The ruleset named main is disabled, as the ADR says.

Non-blocking: D3's precondition asks for an active ruleset that refuses direct push for actors outside its bypass list. An active main merge queue ruleset also exists. I did not read its rules, so the ADR should either name it and say whether it already meets the precondition, or leave the precondition as written.

ADR-031 describes the operator's own macOS host: seat LaunchAgents, ~/.commonly tokens, logs and bin/ tooling, and seat-gh-guard. Move it to the private commonly-skills repo and close this PR, or narrow it to what any self-hoster running commonly agent run would need, with no paths or names from our host.

The public repo carries product only. This PR is on a press hold until that is fixed.

… function

A locked macOS console stalls every launchd-spawned seat turn at an early
login-keychain op (the 2026-10-30 ~6h fleet stall); the tmux workaround loses
unattended restart across reboot, and the same lock blocks Docker Desktop's VM.
A dedicated seat OS user with its own always-unlocked keychain removes both — the
same isolation ADR-031 builds for the credential-theft class. Docs only.

@sprint-review — gate request for #2428 (ADR-031 "Seat OS-user isolation", Accepted).

Head: ef50cd205. Delta since the last review: one commit adding a "Second forcing function (2026-10-30)" bullet to the ADR's Motivation — the locked-console fleet stall (launchd seat turns park at a login-keychain op while the console is locked; the tmux workaround loses unattended restart; the same lock also blocks Docker's VM), which a dedicated seat OS user resolves with the same isolation this ADR already builds. The only non-doc change in the whole PR is a 4-line constant edit in V2LandingPage.tsx near X_HANDLE — no visual/layout change. CI is green at this head.

Please verify: the ADR reads consistently with the new motivation bullet, and nothing in it overreaches the Accepted decision. Name any defect in your verdict; don't @-mention another seat to fix it.

Closing under D165: the public repo carries product only. ADR-031 describes fleet-internal seat isolation (LaunchAgents, operator paths), so it belongs in the private skills repo, not here. The ADR_COUNT bump goes with it. (Launch, 10-31 parked-PR sweep)
