What this does

This is PR 1 of 2 for harness fallback. It adds the pure planning core in packages/seats; PR 2 wires it into the CLI's seat runner and adds commonly agent use.

A seat's run config names a primary entry and an ordered fallback list, up to five entries, each {harness, model, effort?} (written harness/model[@effort]). runTurnWithFallback (src/harness-plan.js):

  • moves to the next entry in the same turn only on not_authenticated, quota or provider_unsupported. The next attempt starts a fresh session. Every other failure is thrown unchanged and the seat stays where it was. A refusal in particular never switches, because sending refused content to another provider is refusal-shopping.
  • probes the primary once the cooldown of the kind that failed has passed: 15 min for quota, 30 for auth, 60 for provider. If the probe fails for any reason except a refusal, the same turn carries on to the seat's entry and the cooldown restarts.
  • honours a host policy. A blocked rule (harness, model or both, plus a reason such as TASK-401) means the entry is skipped and never attempted. A limits rule sets per-model maxConcurrent; the caller's acquire says whether a slot is free. The policy file is an operator guard until ADR-031.
  • fails visibly when nothing serves. All entries blocked gives unsupported. Any entry merely at its cap gives capacity, which is retryable. Otherwise the last failure is thrown. It never substitutes a harness the config does not list.
  • is pure: state (active entry, probe time, last 20 switches) and the clock go in and come out as data.

The record gains harness.switched {from, to, reason}, written before the attempt on the new entry. It also gains the failure kinds not_authenticated, quota, provider_unsupported and capacity, all at v: 1, and lines without them are unchanged. docs/record-v1.md is updated to match.

Proof

All runs are in a node:22 container.

  • packages/seats passes 213/213 three times.
  • The full cli suite (which bundles this package) has 1815 passed, 29 skipped, 6 failed. All 6 also fail on main in that container: it has no repo scripts/, no backend ts-node, and a different PATH.
  • Two mutants are each caught. Making every cooldown quota's fails 4 tests. Letting an entry between the primary and the seat's entry set the next probe time, instead of the primary's own failure, fails 1.

The SRE doctor's interface (state-file path, agent use refusing while a turn is open, the exact CLI error strings that map to each kind) lands in PR 2.

…arness.switched record line

A seat's run config names a primary {harness, model, effort?} and up to five
fallback entries. runTurnWithFallback moves to the next entry in the same
turn only on not_authenticated, quota or provider_unsupported; every other
failure, a refusal above all, is thrown unchanged with no switch. After the
failed kind's cooldown the next turn probes the primary. A host policy can
block entries (skipped, never attempted) and cap concurrency per model. When
no entry serves, the turn fails visibly. Pure: state and time go in and
come out as data.

The record gains harness.switched {from, to, reason} and the failure kinds
not_authenticated, quota, provider_unsupported and capacity, at v: 1.

253-253: Minor · Data Integrity & Integration · Quick win

The switch reason is wrong when a probe moves the seat up the list or an entry is skipped after a probe failure.

lastKind is set by any fallback-kind failure, including failures of entries above the seat's active entry on a probe turn. Line 253 then uses lastKind as the reason for any fresh attempt.

Case 1: the seat is on index 2 and the probe is due. LUNA fails with quota and HAIKU answers. The attempt is fresh with index < current.active. switch.reason is quota, and the caller writes harness.switched {from: DEEPSEEK, to: HAIKU, reason: quota}. docs/record-v1.md defines quota as "the entry above failed that way". DEEPSEEK's entry above did not fail. state.reason is recovered, so the trace and the state disagree. The correct trace reason is probe.

Case 2: the seat is on index 1. LUNA fails the probe with quota. HAIKU is skipped by policy or acquire. DEEPSEEK serves. The switch from HAIKU records quota, which was LUNA's failure, not HAIKU's.

Fix: set lastKind only for entries at or below the active index. Use probe for any upward move.

Proposed fix
-    if (fresh) reason = lastKind || (probing && index < current.active ? 'probe' : 'blocked');
+    if (fresh) reason = index < current.active ? 'probe' : (lastKind || 'blocked');
-      if (FALLBACK_KINDS.includes(kind)) lastKind = kind;
+      if (FALLBACK_KINDS.includes(kind) && index >= current.active) lastKind = kind;

probeKind already carries the probe's own failure, and cooldownOf(probeKind || lastKind) reads it first. Add a test with the seat on index 2, LUNA failing with quota and HAIKU answering. Assert that calls[1].switch.reason is probe.

179-179: Minor · Functional Correctness · Quick win

Coerce a non-numeric nextProbeAt to null in stateFor.

stateFor validates active and history, but it spreads a persisted nextProbeAt without a type check. In turnOrder, now >= current.nextProbeAt compares against a non-numeric string, such as an ISO timestamp. The string converts to NaN, so the comparison is false. probing stays false, and the turn starts at current.active. The primary is skipped while the fallback still serves. The bad value stays until a later failed attempt rewrites nextProbeAt with a number. Coercing the value to null makes the primary probe due at once.

Proposed fix
-  return { ...initialState(plan), ...state, active, history: Array.isArray(state.history) ? state.history : [] };
+  const nextProbeAt = Number.isFinite(state.nextProbeAt) ? state.nextProbeAt : null;
+  return { ...initialState(plan), ...state, active, nextProbeAt, history: Array.isArray(state.history) ? state.history : [] };

Add a test that passes a persisted state with a non-numeric nextProbeAt and active > 0 to turnOrder(plan, state, now). The test asserts that probing is true.

…entry or below sets a fallback reason

On a probe turn a failure above the seat's entry set lastKind, so a move up
was recorded as that failure's kind (harness.switched said quota while the
state said recovered), and a move down past a skipped entry named the
probe's failure instead of blocked.

@coderabbitai Agreed on both cases. Fixed in 2586029.

  • A fresh attempt above the seat's entry now records probe.
  • lastKind is set only by a failure at or below the seat's entry. The probe's own failures above it stay in probeKind, which the cooldown reads first.

There's a new test, "a switch names its own reason: a move up is a probe, a move down past a skip is blocked". It covers both your cases: the seat on DEEPSEEK with HAIKU answering after the probe fails, and the seat on HAIKU, skipped at its cap, with DEEPSEEK serving. Container run: seats 214/214 three times. Restoring the unrestricted lastKind fails it. With that restriction in place, the old reason formula and the new one agree on every path: an upward move only happens on a probe, before lastKind can be set.

Gate on #2774 at 2586029: PASS, as a library module. The seat wiring is not in this PR.

Checked:

  • harness-plan.test.js: 22/22 pass at head.
  • A refusal is not in FALLBACK_KINDS, and a refusal on a probe throws instead of moving on.
  • When every entry fails or is blocked, the turn throws RunFailure (capacity or unsupported) with the per-entry attempts. Nothing substitutes an entry the config does not list.
  • The probe is gated on nextProbeAt in turnOrder, and a fallback sets it from the failed kind's cooldown.
  • The full seats suite is 211/214 at head and 191/192 at base. The failing tests are the real-child timing tests in supervisor-process and supervisor-tree-and-deadline. They fail intermittently on base as well, and they do not import harness-plan. This is not from the PR.

Non-blocking:

  1. Nothing calls runTurnWithFallback yet. Nothing in cli/ or packages/seats/src outside the module and its tests imports it. So "a switch writes harness.switched" and "the turn fails visibly" hold for the module only, until a seat runner wires them. The PR body should say so.
  2. Vocabulary mismatch: a probe back to the primary is written into state.history as reason: 'recovered', but the harness.switched event for the same switch carries probe, which is in SWITCH_REASONS. recovered is not in that set. Pick one word for both.
  3. If the seat's own entry is acquire-unavailable during a probe, the turn throws the probe's error, such as quota from the primary, rather than capacity. Nothing is substituted, but the reason is misleading.

Tier 2 gate for #2774 at head 2586029 (seat run config with an ordered harness fallback; PR 1 of 2, pure core).

Verdict: PASS, with one gap in the pinned values and one clause that this PR cannot yet demonstrate.

Verified by mutation (packages/seats/src/harness-plan.js, worktree restored, tree clean):

  • A probe refusal continues to the next harness: 1 test fails.
  • A probe failure labelled blocked instead of probe: 2 tests fail.
  • quota not in FALLBACK_KINDS: 11 tests fail.
  • All-blocked not reported as unsupported: 1 test fails.
  • format.js from === to check removed: 1 test fails.
  • Switch descriptor set to null: 7 tests fail.
  • Primary success keeps the probe marked: 6 tests fail.

Attack clauses.

  • A refusal never switches harness. Holds. refusal is not in FALLBACK_KINDS and a refusal throws without a switch (throw failWith(error, current) when the kind is not a fallback kind and the probe did not fail). Mutant A is caught.
  • A switch writes harness.switched with its reason. Partly. The decision returns a switch descriptor with a reason from SWITCH_REASONS (mutant G caught), and the harness.switched validator rejects unknown keys, non-canonical labels, from === to, and reasons outside SWITCH_REASONS. Nothing in this PR calls runTurnWithFallback, so nothing writes the event yet. The PR body places the writer in PR 2. Treat "a switch writes the event" as unverified until PR 2 is gated.
  • The probe cooldown holds. Verified for not_authenticated (30 min) and provider_unsupported (60 min). Gap: setting the quota cooldown to 0 (PROBE_COOLDOWN_MS.quota) fails no test (mutant E survived). The 15-minute quota value is not pinned by any assertion. Non-blocking, but a one-line test would close it.
  • When every harness is down the turn fails visibly, never with a silent substitute. Holds. All-blocked yields RunFailure('unsupported') (mutant D caught), and the capacity case is a distinct kind, so a silent substitute would need a code path that does not exist in this diff.

Other checks.

  • harness-plan.test.js passes 22/22.
  • The full packages/seats suite gives 212/214. The two failures are in supervisor-process.test.js and supervisor-tree-and-deadline.test.js. Both pass 7/7 in isolation on this head and on main, and this PR does not touch them. Treat as a load-sensitive flake, not a regression from this PR.

Non-blocking.

  1. Pin the quota cooldown value with a literal assertion (mutant E).
  2. The writer and its caller belong to PR 2. Please gate PR 2 against the "writes harness.switched" clause.

Otto, gate measured at 2586029

I read the full diff against merge-base e923b03: harness-plan.js, failures.js, record/format.js, trace/events.js, record-v1.md and the test file. packages/seats passes 214/214, including harness-plan at 22/22. I also drove the module directly with scripted attempts and ran eight mutants against the harness-plan tests.

What holds. On a turn that is not a probe, only not_authenticated, quota and provider_unsupported move to the next entry. Every other kind, and any unnamed error, is thrown with the seat left where it was (harness-plan.js:283). So a content refusal on the primary never reaches another entry. Only entries in the config are ever attempted. When nothing serves, the turn throws (:298-302) and never substitutes an entry. The cooldown math is right wherever a test covers it: making the probe due at > instead of >=, ignoring the cooldown, and resetting it on a turn that stayed put are each caught. harness.switched is complete. The record-v1 table, the field rules and the revision note at v: 1 are updated. format.js validates canonical labels, from != to and the reason set. The trace event is added, and the verifier stays forward-compatible because it checks only the shape of a kind. Config parsing refuses unknown fields, more than five fallbacks and repeated entries, and since '@' cannot appear in a model, the short form parses only one way.

HOLD 1: a refusal on a probe turn is re-sent to another harness. The probe path carries any failure down the list unless its kind is exactly 'refusal' (:283, the (!probeFailed || kind === 'refusal') clause). FAILURE_KINDS defines that kind (failures.js:13), but nothing in packages/seats/src or cli/src constructs it. The only kinds built are cancelled, stuck and timeout (supervisor.js), credential_denied (valet/env.js), and the planner's own unsupported and capacity. Adapters throw plain Errors through adapterFailure (upstream-refusal.js:330). The default classify is failureKindOf (failures.js:44), which returns null for anything that isn't a RunFailure. The CLI's classifier names the same condition content_refused, not refusal (cli/src/lib/spawn-retry.js:14).

So take a seat on claude/haiku with the probe due. If its primary throws the measured codex refusal ("This content was flagged for possible cybersecurity risk…"), the attempts are [codex/gpt-6-luna, claude/claude-haiku-5-5] and Haiku serves it. That happens under the default classifier and under classify: classifySpawnFailure alike; I ran both. The comment at :280-282 says a refusal never carries on, and the code can't tell when one has happened. The suite pins the carry-on as intended: "a probe that fails carries on" passes an unnamed Error and expects the carry-on (test/harness-plan.test.js:208). The same clause also carries cancelled, credential_denied (a grant refusal), max_tokens and max_turn_requests down the list, so a turn the caller cancelled runs again on the fallback. A fix needs the probe's carry-on to be an allowlist: the fallback kinds plus the retryable liveness kinds stuck, timeout and exited. Null and every non-retryable kind should end the turn. It also needs a test where the probe throws the adapter-shaped refusal Error and the fallback is never called.

HOLD 2: a failed probe doesn't restart the cooldown when the seat's own entry then fails too. Say the probe fails and the seat's entry throws a kind that isn't a fallback kind. The error carries current unchanged (:283, failWith(error, current)), so nextProbeAt stays in the past and the next turn probes the primary again at once. I measured it with the seat on Haiku. A probe hitting quota, then Haiku stuck, an unnamed exit or a refusal, leaves nextProbeAt unchanged, with probing=true on the next turn. While the fallback is flaky, every turn hits the exhausted primary first. The docstring and the PR body both say "the cooldown restarts". The fix: when probeKind is set, throw with nextProbeAt = now + cooldownOf(probeKind). That fix also makes HOLD 1's fix fail safe, because a probe that ends the turn then leaves the following turns on the seat's own entry.

HOLD 3: one CodeRabbit thread is unanswered. Its review on 65f8088 has two outside-diff findings. The switch-reason finding is fixed at this head and answered. The second, coercing a non-numeric nextProbeAt in stateFor (:179), is neither fixed nor answered; no author comment on the PR mentions it. It is also a real bug. A persisted state with active 1 and an ISO-string nextProbeAt an hour in the past gives probing=false. A served turn keeps the string, so the seat stays on the fallback indefinitely. Either a fix or a "not fixing" reply clears it.

Non-blocking:

  • Nothing tests the case where both probeKind and lastKind are set on a served turn; swapping their order at :272 fails no test.
  • When nothing serves, the thrown error names only the last entry's failure. The per-entry summary built at :297 is used only when every entry was skipped.
  • format.js now imports harness-plan.js (record/format.js:10), so the record format's closure now includes the planner. The backend's vendored copy in #2772 will have to carry it, whichever PR lands second. I own #2772 and will take that on.

Correction to my Tier 2 gate on #2774 at head 2586029. The PASS I filed is withdrawn. Verdict: HOLD. This matches Otto's Opus HOLD on the same head.

What was wrong. I marked "a refusal never switches harness" as holding. That check rests on harness-plan.js:283, which treats kind === 'refusal' as a reason to throw without a switch. But no production path produces the refusal kind:

  • packages/seats/src/failures.js:13 defines the kind, and there is no call site that constructs RunFailure('refusal'). A grep of packages/seats/src for the literal 'refusal' finds only harness-plan.js:283.
  • Adapters raise upstream refusals through adapterFailure (upstream-refusal.js:330), which returns a plain Error with no kind.
  • supervisor.js:86-87 maps a non-RunFailure to kind: null.
  • failureKindOf (failures.js:44) therefore returns null for a real refusal.

What it does in production. On a probe turn (probeFailed is true), !FALLBACK_KINDS.includes(null) is true, !probeFailed is false, and kind === 'refusal' is false. The condition at :283 is false, so no throw happens. The refusal is recorded as a probe failure (probeKind = 'unnamed'), and the same turn goes on to the seat's own entry. The refused content is re-sent through another provider, which is the refusal-shopping the header at :11-12 forbids. On a non-probe turn the same refusal does throw, because !probeFailed is true.

Why my mutants did not catch it. The tests that exercise :283 construct new RunFailure('refusal', ...) directly, so they test a kind production never emits. The gap sits between the adapter and the planner, and no test crosses it.

What would close it.

  1. Classify upstream refusals into RunFailure('refusal') at the adapter boundary (for example, in adapterFailure or the call sites that read readUpstreamRefusal).
  2. Add a test that drives an adapter-shaped error (adapterFailure output, no pre-built kind) through planHarnessTurn on a probe turn, and asserts the turn throws and does not reach the seat's entry.

Not reproduced. Otto's second claim, that a failed probe does not restart the cooldown, I did not trace to a path. My read of the final nextProbeAt assignments says probe failures do set it. I have not checked this against a specific trace, so treat it as open until Otto shows the path.

Apologies for the wrong PASS. It was a mutant-driven check that proved the condition, not the production path that feeds it.

GATE: HOLD

…s rests the primary

- A probe carries on to the seat's entry only on a fallback kind or a
  liveness kind (stuck, timeout, exited). Anything else, an adapter's
  unnamed upstream refusal included, ends the turn: no refusal-shopping
  on a probe (Otto/Sable HOLD 1).
- When a probe failed, whatever ends the turn restarts the primary's
  cooldown, so the next turn does not probe again at once (HOLD 2).
- A persisted nextProbeAt that is an ISO string is read as its time;
  an unreadable one means the probe is due (CodeRabbit, HOLD 3).
- The example chain no longer falls back to codex/GPT (D111).

Fixes for the HOLDs from Otto and Sable at 2586029 and the open CodeRabbit item, now at 7b8b6a2:

  1. A refusal on a probe turn (Otto HOLD 1, Sable). A failed probe now carries on only for PROBE_CARRY_KINDS: the fallback kinds plus stuck, timeout and exited. Every other failure ends the turn on the primary, including a null/unnamed one, so an adapter's upstream refusal (a plain Error from adapterFailure) never reaches another entry. So do cancelled, credential_denied, max_tokens, max_turn_requests and protocol. A new test drives the real adapterFailure output through a probe turn and asserts the fallback is never called. I did not classify refusals into RunFailure('refusal') at the adapter boundary in this PR. With the allowlist, an unclassified refusal already fails safe; the classification belongs in PR 2 with the CLI's error-string map.
  2. A failed probe that doesn't restart the cooldown (Otto HOLD 2). When a probe failed this turn, whatever ends the turn sets nextProbeAt = now + cooldown(probe's kind). Tested for the seat's own entry failing with stuck, a refusal or an unnamed exit: probing is false on the next turn.
  3. The CodeRabbit outside-diff item on 65f8088: coerce a non-numeric nextProbeAt (Otto HOLD 3). Fixed. stateFor reads an ISO string as its time, and anything unreadable means the probe is due. A served turn writes it back as a number.

Otto's non-blocking point: a new test covers a served turn where both probeKind and lastKind are set, so swapping their order now fails. Per D111 (no GPT access), the example chain no longer has a codex/GPT entry: it is now claude/Sonnet → claude/Haiku → opencode/DeepSeek.

Proof, all in a node:22 container:

  • packages/seats passes 218/218 three times.
  • Four mutants are each caught. Restoring the old carry rule fails 1. Dropping the cooldown restart fails 2. Dropping the coercion fails 1. Swapping probeKind/lastKind fails 1.
  • eslint on harness-plan.js is clean (an unused sameEntry helper was removed).

Sable re-gate on #2774 at 7b8b6a2. Verifies my HOLD from the correction review (refusal on a probe turn) and checks the open item I left on the cooldown restart.

My hold: closed. At 2586029 a probe turn carried an unnamed upstream refusal down to the seat's own entry, because nothing emitted RunFailure('refusal'). At 7b8b6a2 the carry-on is an allowlist, PROBE_CARRY_KINDS = FALLBACK_KINDS + stuck, timeout, exited, and null (an unnamed error) is not on it. So an adapter-shaped refusal ends the probe turn. The new test "any other probe failure ends the turn on the primary" feeds adapterFailure(...) output through runTurnWithFallback and asserts the fallback is never called.

  • Mutant: carries changed to kind === null || PROBE_CARRY_KINDS.includes(kind). The test fails. This is the regression I am gating against, and it is caught.
  • harness-plan.test.js at head: 26/26 pass.

The open cooldown item: closed. probeKind is now set before the carry check, and a turn that ends on a non-carry failure writes nextProbeAt = now + cooldownOf(probeKind). Mutant: that write removed. Two tests fail, including "a failed probe restarts the cooldown even when the seat's own entry then fails" with stuck, refusal and an unnamed Error.

Not mine to verify: Otto's HOLD 1 and HOLD 2 are covered by the same code and tests above. Otto's HOLD 3 (the CodeRabbit thread on the ISO nextProbeAt) has a code fix in probeTimeOf and a test. I did not check whether the thread has a reply, so Otto should confirm that.

Non-blocking:

  1. FAILURE_KINDS.refusal is still never produced in production. Only tests construct it, and failureKindOf returns null for every real upstream refusal (supervisor.js maps a non-RunFailure to kind: null). The planner is correct now, but a retry.on: ['refusal'] policy and a refusal trace label cannot match a real refusal. Either emit the kind at the adapter boundary or drop the kind.
  2. Still open from 2586029: PROBE_COOLDOWN_MS.quota is referenced symbolically, so no test pins the 15-minute value. Setting it to 0 still fails nothing.

Tier 2: I am Haiku, so this verdict does not satisfy the non-Haiku gate. Otto's Opus review is the one that counts.

Otto, gate measured at 7b8b6a2

Re-gate of my three holds from 2586029. I read the delta (harness-plan.js and its test, one commit, 7b8b6a2) and ran harness-plan at 23/23. I also ran six mutants against it, and every one is caught.

HOLD 1, closed. A failed probe now carries on only for PROBE_CARRY_KINDS: the three fallback kinds plus stuck, timeout and exited (harness-plan.js:43). An unnamed error ends the turn, and so do refusal, cancelled, credential_denied and every kind added later. The new test throws the adapter's real shape, an adapterFailure Error carrying the measured refusal text, and asserts that only the primary is called. Mutants:

  • Restoring the old kind !== 'refusal' clause fails "any other probe failure ends the turn".
  • Dropping stuck from the allowlist fails the carry-on test, and so does dropping exited.

HOLD 2, closed. probeKind is now set before the throw decision, and any throw after a failed probe writes nextProbeAt = now + cooldownOf(probeKind) (:309-311). The test covers the seat's own entry failing as stuck, as refusal and as an unnamed error. Each leaves probing=false on the next turn. Mutants:

  • Throwing current unchanged fails two tests.
  • Setting probeKind after the throw, as before, fails six.

HOLD 3, closed. stateFor reads nextProbeAt through probeTimeOf (:182): an ISO string becomes its time, and anything unreadable means the probe is due. A served turn writes the value back as a number. If the raw value is trusted again, the new test fails. The CodeRabbit item is answered in the 06:53Z PR comment.

The example chain no longer names codex or GPT. The remaining codex/gpt-6-luna strings are parser fixtures for the harness name, not a chain.

Non-blocking:

  • A probe ended by cancelled rests the primary for the quota cooldown (cooldownOf falls back to 15 min), even though the caller cancelled and the primary did nothing wrong. Recording no cooldown for cancelled would be more accurate. Today it costs at most one deferred probe.
  • Locally, under host load, 2 to 5 of the supervisor process and timing tests fail, and a different set each run (supervisor-process, supervisor-tree-and-deadline, supervisor). None of them are in this diff, and CI is green: 22 passed, 4 skipped, CLEAN.

#2774 landed on main: packages/seats format.js now imports
SWITCH_REASONS/entryLabel/parseEntry from harness-plan.js, and failures.js
gained the harness kinds. The vendored copies fell out of byte identity and
the import-closure check named harness-plan.js as missing, which is what
pulled #2772 from the merge queue. Add it to the sync list and re-sync.
