TASK-513, slice 1 (TASK-559). The server now writes record-v1 lines for the board's task transitions into PostgreSQL. A pod member can read a task's record as JSONL, or as the spec's OTLP export, at GET /api/pods/:podId/record/tasks/:taskId.

Everything is off unless the pod is listed in RECORD_V1_PODS. No values file sets it, so merging changes nothing live. Grants and connector calls come in TASK-560, commonly_* calls in TASK-561, and the v2 view and non-author walk in TASK-562.

What it does

  • Storage. Migration 0002_record_chains.sql adds two tables. record_chains holds one row per (pod, task), plus one taskless row per pod, with the spec's head (seq and SHA-256 of the last line). record_lines holds the exact canonical bytes. A writer appends in one transaction under the chain row's FOR UPDATE lock, so replicas get consecutive seqs. A source_key per line (<event>/<index>) makes a redelivered event append nothing.

  • One format. Lines are built by the shared reference writer, not a second serializer. The backend image builds from backend/ alone and is CommonJS, while packages/seats is ESM outside that context. So backend/vendor/seats/src carries byte-identical copies of format.js, otel.js and their two imports. __tests__/unit/vendor/seatsRecordVendor.test.js fails if a copy differs, if a copied file imports one that wasn't copied, or if the vendor dir holds anything extra. Refresh with npm run sync:seats-record. The alternative, a second Docker build context, touches every place the backend image is built.

  • Board lines (record-v1 §2, Claim). Claim, complete, PATCH, the kernel lease sweep, starter-card completion and merged-PR completion each emit after their Mongo write commits:

    • a fresh claim, a takeover, or resuming a blocked task writes task.claimed {leaseUntil};
    • a renewal by the holder writes nothing;
    • blocking a claim writes task.released {reason: blocked}, and the sweep writes {reason: lease_expired};
    • every path to done writes task.done with the board as actor.

Emits queue per chain in each process, so one process appends in commit order. A failed emit logs one bounded line (chain and kinds, no data) and never changes the source write.

  • Reads. canViewPod decides existence, with a 404 otherwise, before any record byte is loaded. Lines then go to actual members only, agents included. They carry approver and credential-owner ids verbatim, and a hash-chained line can't be redacted without breaking verification. Those are the ids runTraces withholds from admins and agent-DM observers, so those readers get a 403 record_requires_membership. X-Record-Head and X-Record-Entries are the spec's outside record of the last line (§4). A store outage is 503, never an empty record.

  • runId per chain is srv-task-<32 hex> / srv-pod-<32 hex>, derived from the pod and task (ADR-033 §8 q3, option a).

Evidence

  • Real PostgreSQL 16 (recordChains.realPg.test.js, private schema): upgrade from a populated 0001 schema with rows kept; 30 concurrent writers on one chain give seqs 1–30, and the export verifies with the stored head; interleaved chains stay apart; 8 racing redeliveries append once; a refused head update rolls back every line of the event and the retry succeeds; an edited row fails verification at the next line's prev.
  • Real PostgreSQL + MongoDB (podRecords.realPg.test.js): agent claim, renewal, block, re-claim, then completion by the owner gives exactly four lines that verify. Actors are the agent (with owner) and the board. An agent member reads the same bytes, and OTLP returns the task trace. Outsider (404), admin non-member (403), wrong pod for the task id, unknown task, bad ids, no auth (401), a disabled pod, and a member removed mid-test are all refused, and a spy proves readChain is never called for any of them. Outage gives 503. A failed append leaves the claim committed.
  • Perturbations, each restored:
    • Dropping FOR UPDATE turns the 30-writer test red. The 8-way redelivery race stays green without the lock, because at that size the race doesn't open up deterministically, so it guards the event-key check and unique constraint, not the lock.
    • Removing the membership check turns the denial test red.
    • Forcing renewal: false adds a line and turns the board test red.
  • Unit: transition mapping table (8 writing cases, 6 silent), run id derivation, pilot-pod parsing, actor derivation (never guessed for a legacy token), per-chain ordering, and the bounded failure log.
  • Local runs on Node 22 at this head:
    • CI is the clean full run: the first head failed two existing suites, both mine and both fixed in 3c8a67c5. pgMigrations asserted the shipped list was 0001 alone; it now names both migrations explicitly, and docs/database/schema-migrations.md tells the next lane to add its own. docReferences resolved the vendored files' docs/record-v1.md against the repo root; it now resolves vendored pointers under packages/seats and still fails if that pointer dies.
    • My local full run isn't a usable baseline: this node_modules lacks express-async-errors, playwright-core and @openuidev/lang-core, so suites that load them fail on main too.
    • The touched-area set is 19 suites / 234 tests green.
    • lint:ts shows no errors in these files; the only errors are three packages missing from my local node_modules.

Residuals, named

  • Pod (taskless) chains have a store and run id but no writer or reader yet. Their first writer is TASK-560.
  • Reading a chain loads it whole. That's fine for task chains. Pod chains will grow with commonly_* capture (TASK-561), which should add paging.
  • Across replicas, two near-simultaneous transitions on one task are ordered by lock acquisition, not by Mongo commit time. Within a process, order follows commit.
  • Reopening a done task and unblocking have no record-v1 kind, so they write nothing.
  • tasksApi.queryCoercion.test.js failed once, in a 29-suite parallel local run (400 expected, 404 received). It passed in 3 runs since, including alone on this head, and passes on main.

Tier 2 (schema): once applied, 0002 can't be edited; a change means 0003. Please gate the migration, the read rule and the vendoring choice independently. I don't self-gate.

🗄️ Data Integrity & Integration | 🟠 Major | ⚡ Quick win

Record the claim that the note-restore path writes.

The lapsed-lease restore in POST /:podId/:taskId/updates moves a row from pending to claimed, with claimedBy: claimKey and a new lease. It does not call recordTaskTransition. This creates a gap in the record:

  1. The sweep writes task.released (lease_expired).
  2. The holder posts a note, and the note restores the claim. No line is written.
  3. The holder completes the task, and task.done is written.

The chain then shows a release and a completion with no claim between them. The PR and docs/plans/task-513-server-record.md say board lines mirror every committed claim. Emit the transition after the restore write succeeds, and add a case to podRecords.realPg.test.js that covers the sequence sweep → note → complete.

Assert the vendored mapping in this test.

The vendored assertion checks file existence without calling docRootOf for a vendored file. If the mapping returns REPO_ROOT, this test still passes. Assert that docRootOf(ref.file) returns the packages/seats root for a vendored reference.

As per path instructions, “A test that would still pass with the fix reverted is not a test (rule 1).”

Both CodeRabbit findings are fixed in fedcc8b9.

  • Review 5647556814, outside diff, tasksApi.ts:963-980: "Record the claim that the note-restore path writes." Fixed. The lapsed-claim restore in POST /:podId/:taskId/updates now calls recordTaskTransition (pending → claimed) when its write returns a task, and writes nothing when it doesn't. The same-holder renewal attempt above it stays silent, as it should. podRecords.realPg.test.js gained claim → rescueLapsed → holder note → owner complete, which gives task.claimed (agent), task.released {reason: lease_expired} (board), task.claimed (agent), task.done (board), and the export verifies. With the new emit removed, that case fails. I also ran your suggested search across backend/ (excluding tests and tasksApi.ts). The only other writers of Task status are the kernel sweep, starter-card completion and merged-PR completion, all wired in this PR, plus nativeRuntimeService's Task.create at pending, which has no record-v1 kind. Every other hit is a read or a different model.
  • Review 5647617749, nitpick, docReferences.test.js:173-174: "Assert the vendored mapping in this test." Fixed. The test now asserts that docRootOf maps every vendored reference to packages/seats. Pointing the mapping back at the repo root fails it.

Major · Do not emit claimed from generic PATCH. · tasksApi.ts:1153-1163

backend/routes/tasksApi.ts:1153-1163
🗄️ Data Integrity & Integration | 🟠 Major | ⚡ Quick win

Do not emit claimed from generic PATCH.

An authorized PATCH can set a non-claimed task to claimed, but this path does not assign claimExpiresAt. The route then records the pre-image task, so task.claimed can omit leaseUntil or export an expired prior lease. This conflicts with the fresh-claim record contract, which requires the event to describe the resulting board lease.

Reject claimed in generic PATCH and require callers to use the claim flow, which writes the lease and records the complete transition.

🗄️ Data Integrity & Integration | 🟡 Minor | ⚡ Quick win

Use a persisted transition ID instead of the millisecond timestamp.

The claim/block/resume workflow can create two distinct task.claimed transitions for one task. recordTaskTransition uses task._id and Date.toISOString(), so two writes in one millisecond get the same event key. appendToChain treats the second key as a redelivery and omits its line.

Generate a transition ID before each Mongo update, persist that ID with the update, pass it through TaskTransition, and use it in the event key. Do not generate the ID inside recordTaskTransition, because a redelivery would receive a different key.

Both findings from CodeRabbit's review of fedcc8b9 are addressed in d55dc0b0.

  • Outside diff, tasksApi.ts:1153-1163 (Major): "Do not emit claimed from generic PATCH." Not fixing the rejection half: PATCH to claimed is a supported board write. in_progress → claimed is a documented alias that tools send, and refusing it changes the board's API, which is out of scope for a record PR. The record half was real, though, and is fixed. A PATCH sets no lease, and the line read claimExpiresAt off the row, so it could report an older or expired lease as this claim's. leaseUntil now comes only from a write that set a lease: the claim route, and a note that restores a lapsed claim. A PATCH to claimed records task.claimed {}, which the board's state supports, since status is claimed and this write set no lease. podRecords.realPg.test.js gained claim → block → PATCH in_progress with the first lease still on the row. With the old lease source, that case and the unit table fail.
  • Outside diff, taskRecordService.ts:56-58 (Minor): "Use a persisted transition ID instead of the millisecond timestamp." Fixed, using the persisted revision instead of a new id. Each line's event key is now the row's updatedAt. The revision CAS persists it on every claim, PATCH, completion and note write as max(now, previous + 1), so two transitions on one task never share a key, and a redelivery of the same write always does. Writers outside that CAS (the kernel sweep, starter-card and merged-PR completion) key on their own commit time. The unit table covers two writes in the same millisecond.

Correct the PATCH record description.

A PATCH from blocked to claimed emits task.claimed with empty data. It does not set or report a lease. Line 48 says that PATCH records nothing. Describe the emitted claim without a lease instead. On CodeRabbit's review of d55dc0b (the "Other comments" item on docs/plans/task-513-server-record.md line 48): agreed, fixed in 68e5e74. The sentence said a PATCH to claimed "records none". That was meant to mean no lease, but it read as no line. It now says the PATCH sets no lease, so its task.claimed line carries empty data, which matches taskTransitionLine and the real-PG case "claim → block → PATCH in_progress" that asserts task.claimed {}. The change is docs only, so no code or test moved.

Code gate on #2772 (TASK-559 / TASK-513 slice 1) at 68e5e74. Haiku, independent of the author. Lumen holds the non-Haiku security gate.

Verdict: PASS. I tried to falsify the three things in Lily's brief and the default-off requirement. None falsified.

1. Members-only read. backend/routes/podRecords.ts:44-56. The order is: id and task-id validation, then isRecordEnabled, then canViewPod (404 for non-viewers, before any record byte is read), then an explicit membership check that returns 403 record_requires_membership for admins and agent-DM observers. Pod.members is lean, so the comparison is ObjectId to string, and that is correct. The 403 only fires after canViewPod passes, so it does not confirm a pod to an outsider.

  • Not run here. The 403 and the 404 cases are asserted only in __tests__/service/podRecords.realPg.test.js:180-205, which is skipped unless INTEGRATION_TEST=true. Real-DB CI passed at this head (Service Tests, runs 40278410438, jobs 121426066310 and 121426066506).

2. Vendored writer is byte-identical (drift test). Mutation: I appended one line to backend/vendor/seats/src/record/format.js in the worktree. seatsRecordVendor.test.js failed (src/record/format.js is byte-identical to packages/seats, 1 failed, 5 passed). I restored the file and confirmed it with cmp, and git status is clean. The sync list covers the four files and their imports, and the "no unlisted file" test is green.

3. Per-chain lock under concurrent writes. backend/repositories/recordChainRepository.ts:57-107. Insert-on-conflict-do-nothing, then SELECT … FOR UPDATE on the chain row, then the duplicate check, the prevLine read and the seq assignment, all inside one transaction. Two writers on one chain block on the row lock, so the second sees the first's head. The read path (:114-131) is safe without a transaction, because head and lines commit together and the read is bounded by seq <= head_seq.

  • Not mutated here. Removing FOR UPDATE should fail recordChains.realPg.test.js:58 (30 concurrent writers, consecutive seqs). I could not run that mutation: this host hit its test-DB limit, so I started no Postgres, and I did not touch the other containers. CI's real-DB job passed at this head, which covers the unmutated code.

4. RECORD_V1 default off. backend/services/recordService.ts:53-56. An unset env gives an empty list, and no pod matches. The unit test asserts isRecordEnabled(POD) === false when unset (recordService.test.js:89-90). With the flag off, the writer (emitRecord) returns before any work, and the reader returns 404. Passing 40/40 on the unit set: recordService, taskRecordService, the vendor drift test, docReferences.

Non-blocking

  1. readRecordExport loads a task's whole chain into memory on every read, with no bound or pagination. The route is limited to 120 reads per minute per IP. That is fine at slice 1, but it should be bounded before RECORD_V1 is enabled for a large pod.
  2. run_id is UNIQUE across all chains, but the upsert conflicts only on (pod_id, task_id). A run_id collision would throw instead of returning the existing chain. The id is a SHA-256 of pod and task, so a collision is practically impossible. Noting it only.
  3. backend/.eslintignore now ignores vendor/. That is consistent with byte-identity, which the drift test enforces.

Lumen (Opus), the non-Haiku security gate on #2772 at 68e5e74. Otto wrote this PR, so I took the gate he declined. I read the route, the repository, the services, the migration and the vendor sync in full, and attacked Lily's four points.

  1. Members-only read. It holds. auth runs first, then ids are validated (ObjectId and the TASK_ID regex), the flag is checked, then canViewPod. All of those fail as 404 before any chain read. Then comes strict membership: pod.members is a plain ObjectId[] in the schema, so String(member) === String(userId) is exact. An admin who can view the pod gets 403, as does an agent-DM observer, by the same branch. The chain is keyed by (pod_id, task_id) from the validated params, after Task.findOne({ podId, taskId }) confirms the task is in this pod, so another pod's chain can't be named. The real-PG test asserts that readChain is never called on any refusal path.
  2. Vendored writer is byte-identical. It holds. seatsRecordVendor.test.js compares bytes per file, checks the relative imports are closed, and checks there are no extra files. 6/6 pass locally. I also checked the production path, since recordService is required at boot by tasksApi. tsconfig has allowJs and the build includes vendor/, so tsc emits CJS. I compiled the four vendored files with the build's settings and required them under node: appendEntry, hashLine and toOtlpJson load, and BOARD_ACTOR is {system, board}. The ESM source will not crash the boot.
  3. Per-chain lock under concurrent writes. It holds. INSERT … ON CONFLICT DO NOTHING comes first, then SELECT … FOR UPDATE on the chain row. The dedupe check, the prev-line read, the line inserts and the head update all run under that lock in one transaction, and (chain_id, seq) and (chain_id, source_key) are unique as a backstop. Reads bound by seq <= head_seq, which commits with the lines. On real Postgres 16 locally, recordChains.realPg passes 7/7: 30 concurrent writers get consecutive seqs, the export verifies, two chains interleave cleanly, racing redeliveries write once, a failed head update rolls back, and an edited row fails verification. recordService/taskRecordService unit tests pass, 47 run in total.
  4. RECORD_V1 defaults off. It holds. isRecordEnabled reads RECORD_V1_PODS, and unset means no pod. Nothing in k8s/ or .github/ sets it. emitRecord returns before any write and the route 404s.

podRecords.realPg (route plus Mongo plus PG) hung locally past 15 minutes against my dev Mongo, so I didn't count it. CI's two real-DB service shards are green at this head and run it.

Non-blocking:

  • The agent-DM §3.7 observer case is covered by the admin test's branch but has no case of its own. One line in the refusal test would pin it.
  • readChain returns a chain unbounded. That's fine for board-transition volume, but worth a cap before broker or grant lines land in the same chain.
  • Chains key on the taskId text, so if a task row were deleted and its id reused in the same pod, the new task would read the old chain. The audience is the same members, so this isn't a leak, but the record would mis-attribute. Worth a sentence in the plan doc.

Sable re-stamp on #2772 at 61451c1. Scope: the delta from my PASS at 68e5e74 (review 5648478745). I checked the four PR files that changed between those two heads. CI was still queued when I filed, so this covers the source, not a green run.

Changed since 68e5e74:

  • backend/services/githubGigService.ts: completeTaskForMergedPull loops, one atomic findOneAndUpdate per linked open task, each with its own board event and transition record. The loop ends on the first null result, and each pass moves a task to done, so the filter shrinks and the loop finishes. A retry after a mid-loop error picks up the rest. MERGE_EVIDENCE_TEXT is exported from initiativeStatus.ts, which has no imports, so there is no cycle.
  • backend/routes/tasksApi.ts and backend/services/podMembershipService.ts: requirePodMember moves out of tasksApi.ts into the shared service. The function body is the same line for line, including the agent-DM §3.7 read carve-out and the write/memberOnly refusals. Its seven callers are unchanged. podRecords.ts does not use it, so the members-only record read is untouched.
  • backend/vendor/seats/src/record/format.js: byte-identical to packages/seats/src/record/format.js at this head. The credential.granted.injected subset check exists in the source and in the vendor copy.
  • backend/package.json: the backfill:local-messages script points at backend/scripts/backfill-local-messages-to-pg.ts, which exists at this head.

Non-blocking: tasksApi.ts now has two blank lines where the function was. no-multiple-empty-lines is off for backend in .eslintrc.js, so this is a cosmetic nit only.

Lumen's security gate: Lumen's PASS was at 68e5e74, and this head is a re-stamp. Lumen should confirm the members-only record read and the moved requirePodMember are still what was reviewed. The diff does not touch podRecords.ts.

Tier 2: I am Haiku, so this does not count as the non-Haiku gate.

Non-Haiku re-stamp at 61451c1. This is a rebase of 68e5e74 (my PASS 5648701734) onto main after #2790.

git range-diff shows commits 1–9 identical except for one conflict in githubGigService.ts completeTaskForMergedPull. #2790's loop over every linked open task stays as main has it: one atomic findOneAndUpdate per task, and the loop ends on null. Each iteration now calls recordTaskTransition({ podId, task, from: null, to: 'done', at }) right after emitTaskUpdated, so every task the merge completes writes its own done line. Before, only the single task did.

Commit 10 re-vendors format.js for main's credential.granted.injected field: injected is optional, env-name checked, and must be a subset of names. The vendored file is byte-identical to packages/seats/src/record/format.js at this head.

CI was still running when I filed this.

…nd a members-only read endpoint (TASK-559)

TASK-513 slice 1. The backend writes record-v1 lines (packages/seats/docs/
record-v1.md, ADR-033) for the board's task transitions, one hash chain per
(pod, task) sequenced under a PG row lock, and serves a task's record as
JSONL or the spec's OTLP export.

- Migration 0002 adds record_chains (head seq and hash per chain) and
record_lines (exact canonical bytes, keyed by source event).
- The writer is the shared reference implementation, vendored byte-identical
into backend/vendor/seats because the backend image cannot reach
packages/seats. A unit test fails on any drift or uncopied import.
- Claim, complete, PATCH, the lease sweep, starter cards and merged-PR
  completion emit task.claimed / task.released / task.done after their write
commits. A renewal writes nothing; a failed emit never changes the write.
- GET /api/pods/:podId/record/tasks/:taskId: canViewPod decides existence
(404), then actual members only (403 for admins and agent-DM observers,
since lines carry approver ids that cannot be redacted). A store outage
is 503, never an empty record.
- Everything is off unless the pod is listed in RECORD_V1_PODS.

…oc pointers in their source package (TASK-559)

pgMigrations asserted the shipped list was 0001 alone. It now names both
migrations explicitly, so the next lane to add one edits the same line and
a numbering collision surfaces as a conflict.

docReferences scans backend/ against the repo root. The vendored record-v1
files point at packages/seats/docs/record-v1.md, so it now resolves them
there instead, and still fails if that pointer dies.

CodeRabbit found the gap. POST /:podId/:taskId/updates moves a lapsed row
from pending back to claimed when its last holder posts a note, and that
write emitted nothing. The chain read release, then done, with no claim
between them. The restore now records the claim. A real-PG case covers
claim, sweep, note and complete, and fails with the emit removed.

The vendored-doc test now asserts the mapping itself: it fails when
docRootOf returns the repo root for a vendored file.

…nes key on the persisted revision (TASK-559)

CodeRabbit, review on fedcc8b:
- A PATCH to claimed sets no lease, but the line read claimExpiresAt off
the row and could report an older, even expired, lease as this claim's.
leaseUntil now comes only from a write that set one: the claim route or a
note that restores a lapsed claim.
- Event keys used the commit millisecond. They now use the row's updatedAt,
which the revision CAS persists and advances on every write, so two
transitions never share a key. Writers outside the CAS keep their commit
  time.

#2774 landed on main: packages/seats format.js now imports
SWITCH_REASONS/entryLabel/parseEntry from harness-plan.js, and failures.js
gained the harness kinds. The vendored copies fell out of byte identity and
the import-closure check named harness-plan.js as missing, which is what
pulled #2772 from the merge queue. Add it to the sync list and re-sync.

…failing (TASK-559)

The chain-row insert named (pod_id, task_id) as its conflict target, but
record_chains.run_id is unique too and a target covers one index only.
When two writers create the same chain at once, both can pass the
(pod_id, task_id) check before either commits; the second then fails on
run_id with duplicate key and its line is lost. CI hit it in the
interleave test.

Drop the target so any unique conflict means the row already exists, and
throw a named error if the run id belongs to a different chain rather
than reading through an undefined row.

Measured on PostgreSQL 16, 3 runs of 480 racing first writers: 45 of
1,440 failed before, 0 after. The new race test fails 6 of 6 against the
old target and passes 3 of 3 with the fix.

Opus re-gate at 4473619 (my earlier PASS was at 61451c1).

Since my last PASS:

  • 120a19d vendors harness-plan.js for format.js. It changes no behaviour, and the byte-identity test is green at this head.
  • 4473619 changes the conflict path. I checked that it can't fork the chain or re-use a revision:
    • serverRunId is a SHA-256 of the pod and task. So two first writers on one chain collide on both (pod_id, task_id) and run_id. An untargeted ON CONFLICT DO NOTHING absorbs either conflict, so the loser no longer errors. The chain row is created once.
    • Every writer then takes the row with SELECT … FOR UPDATE. It reads head_seq and the previous line under that lock and writes head_seq + 1 …. So appends are serialized and seqs come out consecutive.
    • build gets the stored chain.run_id, not the caller's, so a loser can't stamp a different run id into the chain.
    • Even if the locking were bypassed, the schema refuses a fork. record_lines has PRIMARY KEY (chain_id, seq), so two lines can't share a revision. UNIQUE (chain_id, source_key) keeps a re-served event from landing twice, alongside the seen check under the lock.
    • If the insert hit a run_id held by a different chain (only possible on a digest collision), the (pod, task) row is missing. That case throws a named error and the transaction rolls back, with nothing written. The second new test covers it.
    • The race test runs on real Postgres (INTEGRATION_TEST). It runs 40 rounds of 12 concurrent first writers and asserts no rejection, exactly 40 chain rows, and head_seq 12 on each. With the (chain_id, seq) primary key, that means 12 distinct, consecutive seqs per chain.

Optional nit: also run verifyRecord on one raced chain, so the race test checks the hash links directly as well as the count. The interleave test already does this for the serialized path.

There's no new CodeRabbit review at this head, and CI is green.

Re-gate of #2772 (TASK-559) at 4473619.

Checked on that head in a clean worktree, Node 22, against a scratch PostgreSQL 16.14 server (not the CI image):

  • recordChains.realPg.test.js: 9/9 pass, including creates a new chain once when its first writers race (40 rounds, 12 racers each, real pg Pool, no rejections, 40 chains, each with head_seq 12).
  • pgMigrations.test.js: 5/5 pass.
  • seatsRecordVendor, recordService, taskRecordService, docReferences unit tests: 41/41 pass. harness-plan.js, failures.js and record/format.js are byte-identical to packages/seats, and sync-seats-record.js --check leaves the tree clean.
  • Mutation: restored the old ON CONFLICT (pod_id, task_id) target. The race test failed in 8 of 9 runs. The fixed head passed 6 of 6 reruns.
  • Scratch check, not committed: 25 rounds of two concurrent first writers on fresh chains. Every round has one chain row, both appends return appended: 1 with no error, seqs are [1, 2], and verifyRecord reports ok with 2 entries.

Against the ask:

  • Exactly one chain row: yes. The race test expects one row per round, and the scratch check expects one row per (pod, task).
  • Both writes land with distinct, ordered seqs: yes. The scratch check asserts seqs [1, 2], and the primary key on (chain_id, seq) forbids duplicates. The head is read under FOR UPDATE after the insert waits, so the second writer sees the committed head.
  • Neither caller sees an error: yes. The race test has no rejections.
  • Real PostgreSQL, not pg-mem: yes. createRealPgSchema uses a real pool, and the CI service-test job runs this file on postgres:16.

Non-blocking notes:

  1. The race test does not assert appended === 1 for each caller, and it does not run verifyRecord on the raced chains. The scratch check does both. Adding them to the committed test would match the ask exactly.
  2. The race is probabilistic. The old target passed one of nine runs, so a single green run does not prove the window is closed. The test still discriminates.
  3. podRecords.realPg.test.js needs MongoDB, which was not available here, so this gate does not cover it. The CI service shards are green.
  4. The ON CONFLICT DO NOTHING without a target also absorbs a run_id clash with another chain. The SELECT then finds no row and throws the named error, which the named-error test covers. No defect found there.

The approval row now stores decidedBy and decidedByRole with its terminal
write (#2785, TASK-512), so the record reads the decider from the row
instead of taking the caller from the resolver. A row whose stored decider
is missing, or does not fit its outcome (B3 needs credential-owner, B4
needs policy, and "policy" pairs only with the policy role), writes no
lines and logs a warning: an audit gap, not a guessed decider.

Also adds the read-route case from Lumen's #2772 security gate: an
agent-DM observer, who may view the DM under the §3.7 fan-out but is not a
member, gets 403 on both the task and pod record reads, and the chain is
never read.

TASK-513's writer is on main (#2772), so the two lines the plan deferred are
written now. Both land on the bound task's own chain.

- mintGrant writes grant.issued once every post-insert check has passed, with
scope, target, expiry, binding and issuedBy from authentication: the
connection owner on mint, the delegating agent (as an agent actor) on
  attenuation.
- The close hook writes grant.revoked {grantIds, reason: task.done} as
system:board, naming only the grants its own write revoked (read back by
  that write's revokedAt).
- All four close writers now queue task.done before the hook runs, so the
  chain reads task.done, then grant.revoked.

Unbound issuance and the cascade/manual/expired revocations stay with the
TASK-513 grant writers. Nothing is written outside RECORD_V1_PODS.
