Initiative v1 row 3 (TASK-024), per design #2788 §4. Builds on #2790 (TASK-023), merged 10-27. main is merged into this branch (merge commit 5cf8ede, since force-push is refused), so the net diff against main is the row-3 files only: AttentionItem.ts, attentionItemService.ts, initiativeNudge.ts, and initiativeStallNudge.test.js.

What

  • 'initiative' source type. Added to AttentionSourceType and to the schema's source.type enum. This is the single schema change §4 names.
  • attentionItemService.recordInitiativeStall. It sits beside the other attention writers and reuses recordForRecipients, so the existing $setOnInsert upsert and unique index {recipientUserId, source.type, source.id} dedupe it.
    • Kind is blocked.
    • The source id is initiative:<id>:stall:<episodeStart ISO>.
    • The row goes to one current human member of the pod: the owner if the owner is a person, otherwise the agent's installer, resolved via the existing installerForAgent.
    • No agent is woken, so ADR-018's wake policy is untouched.
  • initiativeNudge.nudgeStalledInitiative(initiative, derived, tasks).
    • Only the selected status stalled nudges. needs your check, blocked, waiting on you, and a stalled part inside a moving initiative never do.
    • The episode is derived.activity.since: the newest evidence when the stall began, or createdAt with no evidence.
    • It returns not-stalled | already-nudged | no-pod | no-human | failed | nudged, so the digest (Sharpen 565) can print "no human to nudge". On nudged it returns the {episodeStart, nudgedAt} entry for the model's nudges audit trail (Sharpen 564).

Two choices §4 leaves open (routing ruled by lily, pod 88976; CTO is adding it to §4)

  1. Which pod. AttentionItem requires podId, and an initiative spans pods. The nudge goes to the pod of the open linked Task with the lowest taskNum, which is §3's "Next" rule. Failing that it goes to the first pod link. With neither, nothing is written (no-pod). An owner who is not a human member of that pod also gets nothing (no-human).
  2. Closing a stall row. A new episode opens a new row and resolves the previous episode's row. An initiative that is no longer stalled resolves its open stall row. Without this, a card for an ended stall would stay in the needs-you queue.

Witness (§6 row 3)

__tests__/service/initiativeStallNudge.test.js runs on real Mongo, so the unique index is under test. It covers:

  • two runs in one episode give one row;
  • an agent owner with no installer gives no row;
  • needs your check gives no nudge;
  • the installer path;
  • an owner outside the pod;
  • the first pod link as a fallback;
  • no pod;
  • episode turnover and resolve-on-moving;
  • nudgePodId.

… (TASK-023)

Initiative v1 row 2 (docs/plans/initiative-primitive.md §2). A pure
deriveInitiativeStatus reads three dated records — the merge webhook's
task update, a ruled DecisionRequest, and a close written by the
direction-giver — and returns moving / stalled since / blocked on /
waiting on you / needs your check / closed / closed by owner.

Owner-written task updates, lease renewals and a self-reported done are
deliberately not inputs. The merge update text moves to a shared
MERGE_EVIDENCE_TEXT constant so the webhook writer and this reader cannot
drift; a test feeds the writer's real output to the reader.

…staller (TASK-024)

Initiative v1 row 3 (design #2788 §4). Adds 'initiative' to the
AttentionItem source type (union and schema enum) and a stall writer
beside the other attention writers. Only a `stalled` status nudges. The
episode is dated by the newest evidence when the stall began, so every
run inside one episode upserts onto one row through the existing unique
index; a new episode opens a new row and closes the old one, and an
initiative that moves again closes its open stall row.

The row goes to one current human member of the nudge pod: the owner if
a person, else the agent's installer via installerForAgent. No agent is
woken. The pod is the open linked task the digest names as Next (lowest
taskNum), else the first pod link; with neither, or no human, nothing is
written and the result says why. A failed write reports `failed`, not
"no human".

An initiative that stalls again within the same episode after being resolved can be reported as nudged while the owner sees nothing in their queue. The installer lookup error path only mislabels the failure and is retried on a later run. Fix the resumed-stall case before merging. No production caller exists yet, which limits current exposure.

Add a real-model round-trip test for merge evidence.

The current tests mock Task.findOneAndUpdate and pass the unpersisted $push.updates payload directly to mergeEvidenceDates. No test invokes completeTaskForMergedPull through the real Task model and parses the reloaded update. Add a MongoMemoryServer test that persists the update, reloads the Task, and asserts that mergeEvidenceDates returns one date.

Assert the email side effect of the nudge.

Both calls reach recordInitiativeStall; the second run is not short-circuited. The row-count assertion does not detect an email sent on every upsert. Import and clear sendNeedsYouEmail, then assert one call after both runs and verify the owner recipient.

Add a failed-materialization and retry test.

The suite does not exercise the failed branch. This fixture selects a human pod member, so the Pod.findById spy reaches recordInitiativeStall and forces its caught error before recordForRecipients. Restore the spy, then assert that the retry writes the row.

🎯 Functional Correctness | 🟠 Major | ⚡ Quick win

Wire nudgeStalledInitiative into production initiative processing.

nudgeStalledInitiative is referenced only by its implementation and tests. No production route, worker, or scheduler calls it. Therefore, a production initiative entering stalled never creates the stall attention item. Add one production integration that invokes the function and persists its returned nudge in initiative.nudges; otherwise repeated calls cannot reach the intended already-nudged path.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Match merged PR URLs case-insensitively when attaching merge evidence.

The merge update at githubGigService.ts:84 compares prUrl with exact string equality. The PATCH route stores prUrl without normalization and does not restrict it by status. A pending or claimed task can therefore hold a PR URL whose owner or repo casing differs from the webhook payload. In that case the merge update matches no task. The task stays pending or claimed, and the initiative does not reach needs-your-check.

Suggested fix

Opus gate (Tier 2, wake behaviour) at 5cf8ede.

What holds

  • Race and restart. Six concurrent runs in one episode wrote 1 row and sent 1 email (real Mongo, scratch probe). The dedupe is the unique index on recipientUserId + source.type + source.id, so it survives a restart. The upsert is $setOnInsert only, so a card the human resolved stays resolved on the next run in the same episode.
  • No wake. Recipients come from currentHumanMembers only. An agent owner routes to its installer, and an owner who isn't a human member of the nudge pod gets nothing. The agent-owner probe wrote 1 row to the installer and 0 AgentEvents.
  • No loop. initiativeStatus reads attention only for decision_request, so the blocked/initiative row can't feed back into the status.
  • Tests fail without the change. 6 of 7 one-line mutations go red, each on the test named for it: episode dropped from the source id (2 red), a nonce added to the source id (2), gate on activity.state (1), agent owner nudged directly (1), not-stalled leaves rows open (1), audit check removed (1). PR suite 11/11 at the head. CI 20 pass, 5 skipping, 0 fail. Merges clean onto main 035d466, and main hasn't touched these 4 files since the merge base.

Why HOLD

  1. Three CodeRabbit items on this PR's own files have no answer at the head. Each needs a fix or a not fixing: <reason> reply.
    • Major, review at 98456ef: no production caller. Still true at the head. nudgeStalledInitiative, recordInitiativeStall and resolveInitiativeStalls appear only in their own files and the test.
    • Review at 9866cca: the suite doesn't assert the email count. The behaviour holds in my probe (1 email for 6 runs), but no test pins it.
    • Review at 9866cca: no test for the failed branch and the retry after it.
  2. One episode can nudge twice. The row key includes the recipient, so when the owner changes mid-episode a second open card goes to the new owner and the first stays open. Probe: owner A then owner B in the same episode gave 2 open rows. resolveInitiativeStalls only closes other episodes ($ne: keepSourceId). The nudges audit check would stop it, but nothing persists nudges yet (see 1). Either skip the write when any row already exists for this episode's source id, or land the caller that persists nudges here, with a test for the owner-change case.

Non-blocking

  • The 'initiative' enum entry is never exercised. recordForRecipients writes with updateOne and no runValidators, and nothing calls save() or validate() on an AttentionItem, so removing the entry keeps 11/11 green. Fine as a declaration.
  • resolveInitiativeStalls runs an updateMany on source.type plus a source.id regex on every call. No AttentionItem index starts with source.*, so it scans the collection. Cheap today. Worth an index or a narrower filter before a scheduler calls it per initiative per tick.
  • activity.since moves if someone links evidence that is older than now but newer than the current last evidence. That starts a new episode, so one continuous stall can get a second nudge.
  • CodeRabbit's items on githubGigService.ts and initiativeStatus.test.js belong to #2790's files, now on main. They're outside this diff but still unanswered on this PR.

HOLD relay: @keel, Vera has GATE: HOLD on #2792 at 5cf8ede (review 5677171585), and no new commit has landed since. Read the review on the PR, fix what it names, push, then re-ask Vera on the PR with the exact head.

…in email and failed-retry (TASK-024)

Vera's HOLD at 5cf8ede: the unique index carries the recipient, so a new
owner mid-episode opened a second card. recordInitiativeStall now returns
the episode's existing row, to whoever holds it, before writing.

Tests: owner change in one episode -> 1 row, 1 email (red without the
guard); the two-run case asserts exactly one email to the owner; a failed
write returns `failed`, writes and mails nothing, and the next run nudges.

Replies to the CodeRabbit items and Vera's HOLD (review 5677171585), at head c87d73f.

Review 5648456604 (98456ef), Major: no production caller for nudgeStalledInitiative. Not fixing: this is row 3 of the design's §6 table, whose witness is the service test. The caller is row 4, the weekly digest generator (TASK-025), which iterates Initiative rows. Row 1's Initiative model (Sharpen TASK-564) is not on main yet (git ls-tree origin/main has no initiative model), so there is no collection to persist nudges into and no scheduler that could call this. The double-nudge case Vera found no longer depends on nudges being persisted (see below).

Review 5648353088 (9866cca): the suite doesn't assert the email count. Fixed. The two-run test now asserts exactly one sendNeedsYouEmail call, addressed to the owner.

Review 5648353088 (9866cca): no test for the failed branch and its retry. Fixed. The new test a failed write reports failed, and the next run retries it makes one updateOne reject. It gets failed, with 0 rows and 0 emails, and then the next run returns nudged, with 1 row and 1 email.

Vera item 2: owner change mid-episode nudged twice. Fixed in recordInitiativeStall. Before writing, it looks up any row for this episode's source id, whatever the recipient, and returns that row's recipient. Test an owner change inside one episode does not nudge the new owner too: 1 row, still open, to the first owner, and 1 email. It goes red when the guard is removed. Remaining race: two concurrent runs with different owners in the same instant can still both insert. The unique index can't cover that without dropping the recipient from the key, which §4 relies on.

Review 5648456604, Minor: githubGigService.ts:103 case-sensitive PR URL match. Review 5648353088, nitpick: initiativeStatus.test.js real-model round trip. Not fixing here: both files came in through #2790, are on main, and are outside this PR's diff. I've raised the case-insensitive match with lily as a separate row.

Non-blocking items from Vera, not changed: the enum entry stays as a declaration. The source.* index waits until a scheduler calls resolveInitiativeStalls per tick. The activity.since re-date belongs to the derivation (row 2), and I've noted it for that row.

Make episode deduplication atomic.

If two runs overlap across an owner change, both can find no row and insert for different recipients. The existing unique index includes recipientUserId, so it permits both rows and both emails. (raw.githubusercontent.com)

Enforce uniqueness on the initiative episode independently of recipient, handle the losing insert, and test concurrent runs with different owners.

Based on learnings, overlapping same-key calls need race-safe deduplication.

…ecipient is still in the pod (TASK-024)

CodeRabbit discussion_r4397463788: a departed owner's row is hidden by
the queue, so returning it reported a nudge nobody sees. The guard now
fails toward a second card. Test: owner leaves, new owner gets the
episode (red against c87d73f).

CodeRabbit reply at head 2312b8c (main merged in, no force-push; the net diff against main is still the 4 row-3 files).

#2792 (comment) (revalidate the existing recipient): fixed. The episode guard now counts an existing row only while its recipient is still a current human member of that row's pod. A row the queue would hide no longer blocks the new owner, so the guard fails toward a second card. New test: when the nudged owner has left the pod, the new owner gets the episode, with 2 rows and 2 emails. It is red against c87d73f. Suites: 14/14 for the stall nudge, 23/23 for status.

Use strict installer lookup for initiative stalls.

installerForAgent intentionally swallows errors for optional email-recipient lookups. In recordInitiativeStall, the installer is the required recipient. If AgentInstallation.findOne throws, the current code returns no-human instead of failed.

Use a strict lookup for this path while preserving the existing swallowing behavior for optional email lookups. Add a test that rejects AgentInstallation.findOne and expects failed, separately from the absent-installer no-human case.

The no-human result does not update initiative.nudges, so a later run retries the episode. This can miss the nudge for one run and misreport the cause, but it does not permanently suppress retries.

Reopen a resolved row before returning its recipient.

When all linked tasks are merged, deriveInitiativeStatus selects needs-your-check before activity.state. nudgeStalledInitiative then resolves the stall row. If a new unmerged task is added, it adds no evidence, so the initiative can later become stalled with the same activity.since.

When initiative.nudges does not already contain that episode, recordInitiativeStall returns the resolved row’s recipient without reopening or writing it. The caller reports nudged, but getOpenQueue excludes the resolved row.

Do not fix this by only adding status: 'open' to the lookup. The unique index still matches the resolved row, and recordForRecipients uses $setOnInsert, so it will not reopen that row. Reopen the resolved row explicitly, or use a distinct source identity for the resumed episode. Add a test for the stalled → not stalled → stalled sequence.

#2792 (comment) (reopen a resolved row for stalled → not stalled → stalled): not fixing in this PR.

  • In design §4, an episode is "the timestamp of the newest evidence record when the stall began", and §4 allows one nudge per episode. A stall that resumes with no new evidence is the same episode under that rule, so the second nudge is the case §4 rules out. A distinct source identity for a resumed episode would change the episode definition, which is the derivation's (row 2), not this row's.
  • Reopening would also break a property Vera verified at 5cf8ede: a card the human resolved stays resolved on later runs in the same episode. The row doesn't record whether a human or resolveInitiativeStalls resolved it, so an explicit reopen would bring back human-dismissed cards too.
  • This is not new with the guard. Before it, the $setOnInsert upsert matched the resolved row the same way and returned nudged.

I've raised the question with lily together with Vera's activity.since note: should a resumed stall count as a new episode? If it should, the change belongs in the episode identity, with the stalled → needs-your-check → stalled test this comment asks for.

…iled, not no-human (TASK-024)

installerForAgent swallows lookup errors because its other callers only use
it for an optional email owner. For an initiative stall the installer is the
required recipient, so recordInitiativeStall now uses a strict lookup and a
read error reaches the outer catch as `failed`. CodeRabbit review 5678666214.

CodeRabbit review 5678666214 (at 2312b8c), Minor, strict installer lookup for initiative stalls: fixed at fac0100.

  • The lookup body moved into lookupInstaller, which throws. installerForAgent wraps it and keeps swallowing errors for the optional email-owner callers, so their behaviour is unchanged.
  • recordInitiativeStall calls lookupInstaller directly, so a failed AgentInstallation.findOne reaches the outer catch and reports failed instead of no-human.
  • New test: a failed installer lookup reports failed, not no-human, kept separate from the absent-installer no-human case. It goes red when the call site is put back on installerForAgent.
  • Stall suite 15/15 and attentionItemService unit 61/61. eslint on the service file shows 0 errors before and after.

Vera, could you re-gate #2792 at fac0100? Your HOLD was review 5677171585, filed at 5cf8ede.

Fixes since 5cf8ede:

  • c87d73f. recordInitiativeStall returns the episode's existing row before it writes, whoever that row went to. An owner change mid-episode no longer sends a second nudge. Added tests for the email count, failed then retry, and owner change.
  • b0e46e7. An existing row only blocks a second nudge while its recipient is still a human member of the pod. Test: when the nudged owner has left the pod, the new owner gets the episode.
  • fac0100. If the installer lookup fails, the stall now reports failed instead of no-human (CodeRabbit review 5678666214).
  • 2312b8c. Merged main in, no force-push. Against main the net diff is still only the four row-3 files.

CodeRabbit:

  • Every item has an answer: issuecomment-6462030330, the two discussion replies, and issuecomment-6462733551.
  • coderabbit-threads is passing.
  • The resumed-stall question, your activity.since note and the githubGigService URL match all belong to other rows, and I've raised them with lily.

CI at fac0100 is green: 20 pass. Please file the verdict with gh pr review --comment and put GATE on the last line.

Opus gate (Tier 2, wake behaviour) re-stamp at fac0100. This replaces my HOLD 5677171585 at 5cf8ede.

What my HOLD named, at this head

  • Owner change mid-episode. Fixed. recordInitiativeStall returns the episode's existing row before writing. Removing the guard loop reddens only an owner change inside one episode does not nudge the new owner too.
  • The guard's membership check. Making it count any existing row reddens only when the nudged owner has left the pod, the new owner gets the episode.
  • The failed branch and the retry after it. Reporting failed as no-human reddens both failure tests.
  • Strict installer lookup (CodeRabbit, 2312b8c). Putting the call site back on installerForAgent reddens only a failed installer lookup reports failed, not no-human.
  • No production caller (CodeRabbit Major). Answered as not fixing: the caller is the digest row, and the Initiative model isn't on main yet. I accept that.
  • Email count. Now asserted (1 for two runs, 2 when the first owner has left).
  • Reopening a resolved row for a resumed stall. Answered as not fixing under design §4, which keeps a resolved card resolved within one episode. I agree.
  • CodeRabbit's quiet Minor at c87d73f (make the episode dedupe atomic). The PR comment at 06:07Z already names this race and why it stays: a key without the recipient would forbid the second row that the owner-left case needs. I'm reading that as the answer.

Measured

  • Real Mongo: six concurrent runs with one owner gave 1 row and 1 email. Two concurrent runs with different owners gave 2 rows and 2 emails. That's the race named above. It needs an owner change that lands inside two overlapping runs.
  • PR suite 15/15 and initiativeStatus 23/23 at this head. CI has no failing check. It merges clean onto main ada43fa, and main hasn't touched the four files since the merge base.

Non-blocking

  • After this head, the email dedupe for concurrent runs depends on recordForRecipients emailing only when upsertedId is set. The guard now short-circuits the second sequential run, so nothing pins that check: emailing on every upsert stays green here and in the attention, needs-you-email and teammate-approval suites. A Promise.all of several runs asserting one email would pin it.
  • The guard reads membership in the existing row's pod (row.podId). Reading the current nudge pod instead also stays green, because no test moves the next task to another pod mid-episode.
  • From before, still open: the 'initiative' enum entry is unexercised, and activity.since can shift. The new find on source.id also has no index starting with source.*, so it adds a second collection scan per call next to resolveInitiativeStalls. That's fine until a scheduler calls it for every initiative.

Second Tier 2 gate (Opus) on #2792 at fac0100: the TASK-024 stall nudge.

Read: AttentionItem.ts, attentionItemService.ts (lookupInstaller extraction, recordInitiativeStall, resolveInitiativeStalls), initiativeNudge.ts, and the service test.

Checked:

  • installerForAgent behaves the same as before. Its body moved into lookupInstaller unchanged, and the try/catch that logs and returns null stays in installerForAgent. Only the initiative path calls the throwing form, so a read error surfaces as failed.
  • recordForRecipients throws on a write error, so a failed upsert reaches the catch and returns failed: true. It is never reported as no-human.
  • derived.activity.since is typed Date, never null (initiativeStatus.ts:125, :214 falls back to createdAt), so toISOString()/getTime() are safe for in-process callers.
  • Consumers of the new 'initiative' source type: the queue renders kind blocked, and "Mark handled" goes through acknowledgeAttention, which accepts kind: 'blocked' regardless of source type. Nothing else switches on source.type for it. renderKind only remaps decision+task.
  • One nudge per episode still holds after a human handles the card. The existing-row lookup has no status filter, so a handled row still counts while its recipient remains a member.
  • Nothing calls nudgeStalledInitiative yet (the digest is a later row), so this adds no live behaviour.

Ran at head: initiativeStallNudge + attentionItemService + initiativeStatus give 99/99. Three mutants, each killed:

  • lookupInstaller → swallowing installerForAgent fails "a failed installer lookup reports failed, not no-human";
  • dropping the recipient-membership check on existing rows fails "when the nudged owner has left the pod, the new owner gets the episode";
  • a constant episode in the source id fails "two runs inside one episode…" and "a new episode opens a new row…".

CI: 21 pass, 5 skipped. coderabbit-threads: all answered.

Notes, not blockers:

  1. When an existing row satisfies the guard, recordInitiativeStall returns that recipient and nudgeStalledInitiative reports nudged with a fresh nudgedAt, though nothing new was written. That only happens when the caller failed to persist the earlier nudges entry, so the audit trail would date the nudge late. Returning already-nudged there would be more exact.
  2. I agree with Vera on the concurrent-run test (Promise.all of two runs, so one row and one email). The follow-up keel committed to is the right place for it.
