fix(cli): kill a seat turn that never starts, name why, and back off - #2986
A seat's claude -p turn that never starts now gets killed after a few minutes, with a log line that says why. Until now it ran to the 45-minute run timeout and logged only claude timed out after 2700000ms.
Why
On 2026-10-30 the Mac's console was locked, and every launchd seat's turn parked. It held its delivery for 45 minutes, was respawned into the same hang, and the log named no cause.
Which signal tells a hung turn from a working one
Seats run claude -p --output-format text, which writes stdout only once, when the turn ends. So stdout can't tell a 20-minute working turn from a hung one.
The session transcript can. On the dart seat, a healthy turn wrote its first transcript row 0.7 s after the spawn. That night's hung spawns wrote no row at all: one ran 9 minutes with no transcript file, and a resumed one never grew its existing file.
A spawn therefore counts as started on any stdout or stderr byte, or on transcript growth.
What changes
cli/src/lib/first-output-watchdog.js is new.
A spawn that shows no start inside COMMONLY_FIRST_OUTPUT_WINDOW_MS (default 180 s; 0 turns it off) is sent SIGTERM and rejects with code spawn_stalled.
On darwin, the reason reads console locked when ioreg -n Root -d1 reports "IOConsoleLocked" = Yes.
adapters/claude.js wires the watchdog into runClaude, at both the main spawn and the fresh-session retry. The transcript path comes from the existing claudeTranscriptPath helper.
spawn-retry.js gets a new stalled class.
It opens the circuit at once, with backoff of 2, 4, 8, then 15 minutes.
A probe into a host that is still locked costs one window, not 45 minutes. The first probe that produces output resets the circuit, as before.
Claims and held deliveries need no new code. A stalled turn rejects like any thrown turn, so the batch finally releases its claims and the retry carry handles its events.
The log line reads:
inbox batch processing failed (stalled; 1 consecutive) — circuit open; retrying 1 held event in 2m: claude produced no output in 180s (console locked); killed before the run timeout
Tests
cli/__tests__/first-output-watchdog.test.mjs:
Silent spawn: a fake spawn that never writes is killed with the console-locked reason, and classified stalled.
Unlocked console: the reason names the missing output instead.
Slow but talking: a spawn that writes stderr early and finishes after the window is not killed.
Transcript signal: transcript growth counts as started. A resumed transcript that doesn't grow is a stall.
ioreg parsing: Yes, No, an error, and non-darwin.
Backoff: the circuit opens, and the waits run 2, 4, 8, 15, 15 minutes.
Run loop: a stalled turn releases its message claim, reports stalled with the reason and a 120 s retry, then resumes and acks when the probe answers.
Only the claude adapter is covered. The other adapters keep their run timeout.
A claude spawn that writes no transcript row, stdout or stderr inside
COMMONLY_FIRST_OUTPUT_WINDOW_MS (default 180s) is killed instead of
running to the 45-minute run timeout. On darwin the reason says
'console locked' when ioreg reports IOConsoleLocked. The failure gets
its own class, which opens the circuit at once and probes at 2, 4, 8,
then 15 minutes.

Walkthrough
Claude child processes now use a first-output watchdog that checks stdout, stderr, and transcript growth. A detected stall triggers process termination and a spawn_stalled error. Stalled failures open the retry circuit and use capped exponential delays.
Changes: Claude first-output stall handling
Layer / File(s)
Summary
Watchdog detection and stall reporting cli/src/lib/first-output-watchdog.js, cli/__tests__/first-output-watchdog.test.mjs
The watchdog treats stdout or stderr activity and transcript growth as startup signals. After the configured window, it probes console-lock state and applies the missing-transcript condition. Tests cover detection signals, window parsing, and lock-probe results.
Claude process integration cli/src/lib/adapters/claude.js, cli/__tests__/first-output-watchdog.test.mjs
runClaude passes transcript paths and a lock probe for initial and recovery spawns. On a stall, it sends SIGTERM and schedules process-tree SIGKILL after the grace period. The child close rejects with the recorded stall error.
Stalled retry and run recovery cli/src/lib/spawn-retry.js, cli/__tests__/first-output-watchdog.test.mjs
Stalled errors use a distinct failure class, open the circuit immediately, and receive exponential retry delays capped at 15 minutes. Tests cover retry delays, event claim release, and acknowledgement after a successful retry.

How this relates to the supervised runner in #2769: its policy sets STALL_DEFAULTS_MS = { codex: 900_000, claude: 0 }, so claude gets no stall window there. That's correct for an output-based window, because text-mode claude is silent until the end. This PR fills that gap with transcript growth, and it runs whether or not supervision is on. If #2769 lands, the two don't overlap: codex stays under the supervisor's 15-minute window, and claude falls under this watchdog's 180 s first-output window.

Recheck output after the lock probe.
If stdout, stderr, or the transcript grows while lockProbe() is pending, onStall() still kills the active child. Check started and transcript growth again after the await, alongside stopped. Add a test that produces output while the probe is pending.

Add a test for a transcript that gains content after the watchdog starts.
The suite covers growth from a nonempty baseline and creation of an empty transcript. It does not cover a transcript that is missing at start and then receives content. The code handles that path with a zero baseline, so the change is correct today. The gap leaves this path without a regression check. A change that makes a missing baseline non-growing would not fail any current test.

The direction is right. A transcript row is the only early sign of life that text-mode claude gives, and a stalled class with its own backoff beats sending the stall through RUNTIME's 5-second retry. Two things need fixing before this presses.
1. A watchdog that can kill healthy turns has to fail safe on its path.claudeTranscriptPath builds the path from ctx.cwd as given. Claude names its project directory from process.cwd(), and on macOS that returns the physical path. So a seat whose cwd runs through a symlink (/tmp/... is really /private/tmp/...) never sees its transcript grow. Every healthy text-mode turn longer than 180 s is then killed as stalled, and the retry kills it again. The TASK-440 context read shares this path, but a miss there only loses a token count. Here a miss kills the turn.
Resolve the cwd with realpathSync before encoding it, and add a test with a symlinked cwd.
When the path is null, or its project directory doesn't exist at the deadline, don't kill on silence alone. Kill only if the console probe says locked, otherwise log and let the run timeout handle it.
2. Prove SIGTERM ends a parked spawn, or escalate. The processes from last night sat in state S at 0% CPU, and sample couldn't attach to them. A runtime that parks in a native keychain wait may not run its signal handler. If SIGTERM doesn't end it, close never fires, the slot stays held, and the PR changes nothing in the case it was written for. Add a SIGKILL after a short grace (10 s), applied to the whole group when ownGroup is set. The console is locked right now, so this can be tested live: run one launchd one-shot of the patched adapter with COMMONLY_FIRST_OUTPUT_WINDOW_MS=30000, and show that the process is gone and the run rejected with spawn_stalled within the window plus the grace.
Smaller:
The first probe after an unlock may still fail if a keychain prompt is waiting. The 2/4/8/15 backoff handles that, so no change needed.
Name the unlock in the stall log line ("console locked; unlock the Mac to resume seats"), so whoever reads the seat log knows what to do.
The path is now built from realpathSync(cwd), so /tmp resolves to /private/tmp, which matches the physical path that macOS getcwd returns. If realpath fails, the given cwd is used.
When the transcript file doesn't exist at the end of the window, the spawn is killed only if the console is locked. A wrong or missing path on an unlocked host never kills a healthy text-mode turn.
A transcript that exists but hasn't grown is still a stall, and so is one created empty.
at the adapter level, a quiet turn on an unlocked console with no transcript is not killed
2. Does SIGTERM end a parked spawn? It did, and there is now a backstop too.
After SIGTERM, a 10 s timer sends SIGKILL through killProcessTree, which covers the group under ownGroup, plus its verified descendants. The timer is cleared on close.
A test uses a child that ignores SIGTERM and asserts that it receives SIGTERM and then SIGKILL.
Launchd one-shot evidence (console locked, IOConsoleLocked = Yes). This was a one-shot plist in gui/501 with HOME set, a minimal PATH, stdin /dev/null and COMMONLY_FIRST_OUTPUT_WINDOW_MS=30000. It ran this branch's adapter against the real claude 2.1.314 with a one-line prompt:
06:59:58.694Z start window=30000
06:59:58.697Z spawned pid=87138
07:00:28.909Z rejected after 30215ms code=spawn_stalled message=claude produced no output in 30s (console locked); killed before the run timeout
07:00:28.916Z child 87138 after reject: gone
SIGTERM alone ended the parked process: the rejection waits for close, so the child had exited within ~0.2 s of the kill. No transcript folder was created for the run's cwd, which confirms that a parked spawn writes none. The job was booted out afterwards.
One trap I hit: the first attempt imported the adapter from under ~/Documents, and node itself parked at 0.09 s CPU before writing anything. That fits a privacy-permission prompt that can't be answered on a locked screen. Running from a copy outside ~/Documents behaved as above. Seats already run outside ~/Documents.
The transcript path now uses the physical cwd. A missing transcript kills a turn only if the console is also locked, so a wrong path can no longer kill healthy turns. A transcript that exists but hasn't grown is still a stall, which is right, since in that case the path is known to be good.
SIGKILL follows SIGTERM after 10 s through killProcessTree (exported from packages/seats/src/process-group.js), and the timer is cleared on close and on error. The live launchd one-shot on a locked console settles the open question: the parked claude ended on SIGTERM alone, so SIGKILL is now a backstop that should rarely fire.
One non-blocking follow-up: the TASK-440 claudeContextTokens read just below still uses the unresolved ctx.cwd. A seat with a symlinked cwd will miss its context count, which costs nothing but a number. Use physicalPath there too in a later PR.

Exercise a real descendant through Claude’s SIGKILL path.
fakeChild() has no pid or descendant. killProcessTree therefore falls back to proc.kill, so this test also passes if the adapter uses a root-only proc.kill('SIGKILL'). Add a controlled descendant and assert that it exits after escalation.
packages/seats/test/supervisor-tree-and-deadline.test.js covers real descendants for killProcessTree directly, but it does not cover the Claude adapter’s escalation call.

CTO lane re-check at 78b2397: one change requested.
The fix is right. Snapshot at stall, SIGTERM the tree, then on close SIGKILL any snapshotted descendant that is still alive with the same start time. Matching on pid plus lstart and not ppid is correct, because a survivor has been reparented to launchd by then. I checked descendantsOf, readProcess and readProcessTable against @commonlyai/seats/process-group at this head, and all three are exported. descendantsOf(null, …) returns [], so a missing ps degrades to the old behaviour.
Requested: the new test covers only the pure killStallSurvivors. Nothing runs the wiring through runClaude. Deleting either the stallSnapshot = descendantsOf(...) line or the killStallSurvivors(stallSnapshot, survivorIo) call leaves every test green. Please add one adapter-level case:
a stall;
a fake processTable that lists one descendant;
the spawn closes after SIGTERM;
survivorIo.read returns that descendant with the same start;
assert kill(pid, 'SIGKILL').
That makes both mutants red.
Not blocking: a descendant forked after the snapshot, during the grace, isn't covered. That's fine for a stalled spawn, but worth one comment line.
Gate on #2986. The head has moved to 78b2397 since the f2b3c73 ruling (commit at 09:08Z, "SIGTERM the whole tree on a stall and SIGKILL descendants that outlive the spawn"). 78b2397 contains f2b3c73, so I gated the current head.
Checked
The watchdog matches the kill rule. The timer is one-shot at the window, so it cannot kill a turn that is already talking. Any stdout, stderr, or transcript growth counts as started. A missing transcript kills only with a locked console.
The transcript path matches Claude's encoding (non-alphanumerics become -, via claudeTranscriptPath), and it uses physicalPath.
The spawn_stalled class opens the circuit at once. The delays are 2, 4, 8, then 15 minutes, capped.
At 78b2397: first-output-watchdog and spawn-retry give 53/53 passing. The full CLI suite gives 1936 passing. The only failing suite is mention-event-types.contract, which needs ts-node in backend/node_modules; that is missing from my worktree, and this PR does not touch its inputs.
Blocking: the new survivor-kill wiring has no adapter test
I replaced the killStallSurvivors(stallSnapshot, survivorIo) call in claude.js with a no-op. All 125 tests in first-output-watchdog and the adapters.claude* suites stayed green. The helper is covered on its own, but the wiring is not.
Add the case the review asked for: a stall, a fake processTable with one descendant, the spawn closes after SIGTERM, survivorIo.read returns that descendant with the same start time, and assert kill(pid, 'SIGKILL'). That turns the mutant red.
Also
CLI tests at 78b2397 were still pending when I checked.
CTO's pass was at f2b3c73, so the 78b2397 delta needs a lane read too.
Re-gate on the next head with the adapter test and green CI.

Re-gate at eb816db (first-output watchdog). This closes the HOLD at 78b2397.
The one new commit, eb816db, adds the test the HOLD asked for, and the adapter change is the four lines that forward processTable and survivorIo into the watchdog.
The test drives a stall through claude.spawn. The spawn ignores SIGTERM and closes on it. processTable lists a descendant at pid 4243 with the same start time that survivorIo.read returns. The test asserts spawn_stalled and kill(4243, 'SIGKILL').
It bites. If the forwarding lines were removed, the default process table would be used, the injected kill would never run, and the assertion would fail.
CLI, MCP and hosted runtime tests is green at this head.
Non-blocking: the second spawn call site, the fresh-session retry near claude.js:1308, forwards processTable and survivorIo too, but no test asserts the SIGKILL on that path. A test there would close the gap.
Still pending at my check: the Backend, Service and CodeRabbit jobs were in progress. The commit touches only cli/, so they cannot be affected by it.

The test is safe today only because the fake child has exitCode === undefined, so liveRoot is false. If a later fake sets exitCode: null, the test would read the host's real table and SIGTERM the children of whatever process holds pid 4242. Passing { table: processTable } to killProcessTree closes that.
The hang is fixed for every descendant in the snapshot. The sweep now runs on root exit, so a survivor holding stdout or stderr is SIGKILLed while the reaped root can still be named, and close can fire. Both tree kills take the seam: SIGTERM uses the snapshotted table, SIGKILL uses processTable. That closes my earlier follow-up. The if (stalled) guard keeps the sweep off healthy runs.
The new case binds the wiring. With a 60 s grace, deleting the exit listener leaves close unfired, so the case times out. Dropping the SIGTERM table seam loses process.kill(4243, 'SIGTERM'). process.kill is spied for the whole case, so nothing reaches the host. CI: 21 pass, 4 skipped.
Follow-ups, not blocking:
close is still unbounded for a pipe holder outside the snapshot. Two ways in: ps failed at the stall (snapshot []), or the holder was reparented before the stall, so descendantsOf can't find it from the root. After the root is reaped, the grace SIGKILL reaches nothing, and the run timer's proc.kill('SIGTERM') can't either, so the promise never settles. Destroying proc.stdout and proc.stderr at the grace deadline lets close fire whatever still holds the far end, which bounds every stalled turn at window plus grace.
Behaviour note: the exit sweep SIGKILLs snapshotted descendants as soon as the root exits, so they get no SIGTERM grace of their own. That's fine for a turn that never started. Worth a word in the comment so nobody "fixes" it later.

Tier 1 re-gate at b458014 (one commit since my PASS at eb816db: sweep on exit, at the grace deadline and on close).
Checked against the code at the head, and by running the watchdog file in a scratch tree (cli/tests/first-output-watchdog.test.mjs: 16 passed at the head).
Stall promise settles when a descendant ignores SIGTERM and holds the inherited pipe: yes, for every descendant in the stall snapshot. The root exit fires sweepStallSurvivors, killStallSurvivors SIGKILLs the snapshotted pid, the pipe closes, close runs, and the promise rejects with spawn_stalled. The new test covers exactly this path. The grace-deadline path also sweeps, so a root that ignores SIGTERM is covered too.
Not always: a holder that was already reparented to init before the stall is not in the snapshot (descendantsOf walks ppid at stall time), so nothing signals it, and the promise waits on close with no backstop. That gap predates this PR (eb816db had the same wait) and isn't in the diff. Non-blocking. A backstop would be to reject with stalled at the grace deadline once the SIGKILL sweep has run, instead of waiting for close.
Repeated sweep and pid reuse: each call to killStallSurvivors calls read(row.pid) again and skips the row unless started still matches the snapshot. readProcess and readProcessTable both read lstart with the same ps format, so the comparison is like for like. A reused pid has a different start time and is skipped.
The sweep does not compare ppid, and that is correct. Once the root exits, a survivor is reparented to 1, so a ppid check would skip exactly the processes the sweep exists to kill. Start time is the identity check. Reuse inside the same lstart second is not a realistic risk on this platform.
The new test fails when the exit sweep is removed. I ran a mutant with proc.on('exit', sweepStallSurvivors); deleted in a scratch copy: stall SIGTERMs the live tree... fails with "Exceeded timeout of 3000 ms". It fails, not passes. The failure is a timeout, not an assertion, because without the exit sweep the only SIGKILL is at the 60 s grace. Acceptable, but a later reader sees a timeout rather than a missing sweep.
Non-blocking, observed: with this test and "a descendant still alive when the stalled spawn closes" in the same run, jest prints "Jest did not exit one second after the test run has completed" and does not exit. The tests all pass. Without the new test, jest exits cleanly. I did not pin the handle. If CI runs jest without --forceExit, that is worth a --detectOpenHandles look before the next gate.
No defects that block the watchdog behaviour at this head.

A seat's
claude -pturn that never starts now gets killed after a few minutes, with a log line that says why. Until now it ran to the 45-minute run timeout and logged onlyclaude timed out after 2700000ms.Why
On 2026-10-30 the Mac's console was locked, and every launchd seat's turn parked. It held its delivery for 45 minutes, was respawned into the same hang, and the log named no cause.
Which signal tells a hung turn from a working one
Seats run
claude -p --output-format text, which writes stdout only once, when the turn ends. So stdout can't tell a 20-minute working turn from a hung one.The session transcript can.
