fix(connectors): a drifted hosted-MCP pin reaches its operator - #3286
On 2026-11-01 Sentry respelled seven of its search reads. Each one was refused as tool_drift, which is correct: a pin whose schema moved is never offered. But the only trace was a console.warn line that nobody reads. The connector sat with 7 of 11 tools dark until a template capture tripped over it, and #3283 re-pinned them. This PR gives drift a reader.
What changes
hostedMcpEntryService.ts. The drift cache takes an onAssessed(row, entry, tools) hook. It sees every fresh assessment, clean or not, and never a cached one or an outage. It runs detached: nothing on the call path awaits it, and what it throws is logged (hosted_mcp_assessment_hook_failed) and can't change the outcome.
New hostedMcpDriftAlertService.ts. On tool_drift only, it files one Needs-you card per drifted pin set, for the operator named in the SystemSetting hostedMcp.driftAlert ({ recipientUserId, podId }).
The source id is drift:<entryId>:<16 hex of the alert pod, the pin digests and the sorted drifted tool names>. One drift is one card, and a second row seeing the same drift adds nothing. Another tool drifting under the same pins, or one only another row's consent lists, files its own card. A re-pin is a new set, so its next drift files afresh.
The config is read on every fresh drifted assessment, before any memo. A per-process memo of filed cards, keyed by that source id and the recipient, skips repeat writes, so a newly set or changed operator gets their own card on the next assessment. The card's unique index holds across replicas, and losing the insert race to another replica counts as filed.
Nothing undelivered is memoized. An unconfigured or undeliverable drift tries again on the next fresh assessment, and only its error line is throttled, to once an hour per drift and reason. A malformed id counts as unconfigured.
The recipient lives in the database, set per instance, so no account or pod id is committed.
It refuses to be silently inert, like the onboarding alert. Drift it can't deliver is a console.error naming why: unconfigured, recipient_not_member or failed. A failed write is retried on the next fresh assessment.
attentionItemService.recordHostedMcpDrift. The card has kind blocked and source type hosted_mcp (new on AttentionItem). It goes only to a recipient listed as a human member of the pod, because the queue hides any row whose recipient can't open its pod. No email: the queue is the delivery. It stays open until the operator marks it handled, since another row may still see the drift.
hostedMcpToolDefinitions.ts. The runtime cache passes the alert as its hook, loaded on first use: the alert reads SystemSetting, and the frontend glyph-catalogue test imports this module without mongoose.
tool_unavailable detail. It now says this connection's tools/list does not include X. A row's list depends on what its owner consented to: Sentry offers update_issue only with its Triage skill, and the Linear smoke row lists 26 pins as absent because its scope is read-only. "Upstream no longer lists X" read as a vendor removal on 11-01 and led to a wrong claim in fix(connectors): re-pin Sentry to its 11-01 tools/list #3283's first draft. For the same reason, absence files no card.
Names in the card and the logs come from the pins, and ids come from our rows. Nothing comes from the vendor's descriptions or schemas.
After deploy
One write via ops-runner sets hostedMcp.driftAlert to the connector operator and its pod. Until then a drift logs delivery: unconfigured, which is no quieter than today.
Tests
hostedMcpDriftAlertService.test.js (new, 19 arms): a moved alert pod gets its own card for the same operator. drift files a card naming the entry and each drifted tool, in the singular for one tool. The same drift files once across rows. A changed operator gets their own card, and the first isn't filed twice. Another tool drifting under the same pins files its own card, and the key ignores order. A re-pinned entry files again. Absent and unchanged tools file nothing and read no config. Unconfigured, half-configured, a malformed id, not a member, a failed write and a failed config read each log. A config set after an unconfigured drift, or an operator who joins the pod after the first error, gets the card on the next assessment. The error line repeats hourly, not on every assessment. Failures retry, and losing an insert race counts as filed.
hostedMcpEntry.test.js: the reworded detail. The hook sees fresh assessments, clean ones too, and never cached ones or outages. A hook that throws or rejects can't change the outcome.
attentionItemService.test.js: one blocked hosted_mcp card for a human member, with no email, and nothing for a recipient the pod doesn't list.
hostedMcpToolDefinitions.test.js: the runtime cache hands a fresh assessment to the alert.
frontend/src/v2/__tests__/platformGlyphCatalogue.test.tsx imports hostedMcpToolDefinitions for its pure projection. It failed CI at c7d2fa7 with TextDecoder is not defined, because the alert's eager import pulled in mongoose. It passes, 7 of 7, at c7c4add.
The four backend suites: 216 of 216 pass at 7f47bfffa. Every suite that loads the hosted-MCP, broker, attention or activity modules: 1,505 pass. One rate-limit suite failed once under host load and passes 3/3 alone, on this branch and on main.
tsc --noEmit is clean. eslint has no errors, and the changed files carry the same warning count as main.
Reverse checks. Each mutation was applied, the four suites run, and the file restored.

Review round
A fresh Opus-class subagent reviewed c7d2fa7 and passed it (GATE: PASS, filed as a review on this PR), with no must-fix. It checked:
that nothing on the call path awaits the hook;
that no vendor text reaches the card or the logs;
that the new hosted_mcp source type breaks no reader (the queue, acknowledge, V2ActivityPage);
that the old wording appears nowhere.
It ran two reverse checks itself. All four of its should-fix items are in e1918d5, each with an arm that went red without it (R12–R16): the dedupe key, the recheck, malformed ids and the insert race. It also flagged that "First seen on" can be wrong after a restart, so the detail now says "Seen on".
CodeRabbit
Its two Major threads are both taken in c7c4add and answered in-thread:
don't memoize an undelivered alert (on c7d2fa7, where e1918d5's hourly recheck only shortened the delay);
read the config before the memo, and key the memo by recipient and pod (on e1918d5).
Its review of c7c4add raised one Minor outside the diff, in its body only: the card is unique per recipient and source id, and the recorder only inserts, so a moved alert pod matched the old card and the new pod got none. Taken in 7f47bfffa: the pod is part of the source id (R21).
Its review of 7f47bfffa raised one more Minor in its body: lowercase the pod id before hashing, so a case-only rewrite of the setting can't open a second card in the same pod. Not taken. The setting has one writer, the after-deploy ops-runner step, which writes both ids from an ObjectId's toString(), and that is always lowercase. A case-only rewrite isn't a path we have. If a second writer appears, normalize in configFrom, which already validates both ids.
On 11-01 Sentry respelled seven search reads. Each was refused as
tool_drift, and the only trace was a console.warn, so the connector sat
7 of 11 tools dark until a template capture tripped over it.
A fresh drift assessment now goes to an onAssessed hook. For tool_drift
it files one Needs-you card per drifted pin set, for the operator named
in SystemSetting hostedMcp.driftAlert, in a pod that operator is listed
in. No email. Unconfigured or undeliverable drift is a console.error.
tool_unavailable files nothing: a row's list depends on its owner's
consent, and its detail now says so instead of "upstream no longer
lists", which read as a vendor removal.

Fresh hosted-MCP assessments now trigger alerts for tool_drift. The alert service reads recipient configuration and records a blocked attention item for a current human pod member. Unavailable tools do not trigger alerts.
The attention item schema accepts hosted_mcp sources. recordHostedMcpDrift checks pod membership and records a blocked item without email for eligible recipients.
The drift cache calls onAssessed for fresh assessments, including clean assessments. Hook errors are logged without changing the result. Unavailable-tool details now refer to the connection’s tools/list.

The one review round (D218): a fresh Opus-class subagent reviewed this head. It found no must-fix.
Checked:
Call path. The hook is detached after entries.set. Nothing on the call path awaits it, a synchronous throw lands in its .catch, and outages, cached hits and the connection-changed race all return before it.
No vendor text. The card and the logs carry entry.title, the drifted pin names and row.connectionId, and nothing from the vendor's descriptions or schemas.
The new hosted_mcp source type breaks no reader.getOpenQueue works on kind, acknowledgeAttention accepts blocked, and V2ActivityPage branches only on approval_action. Nothing else switches on source.type.
The old wording is gone.git grep "upstream no longer lists" finds nothing.
Tests. 207/207 pass. It spot-checked two reverse checks, the !== 'ok' filter and marking a failure handled, and both went red.
Should-fix, all taken in the next push:
The dedupe key ignored which tools drifted. A later drift of another tool under the same pins, or a tool only another row's consent lists, was never reported.
An unconfigured or recipient_not_member result was memoized until restart, so setting the config later delivered nothing.
A malformed id threw a CastError and logged failed on every assessment.
A replica race on the upsert (E11000) logged failed although the card exists.
Nits: AttentionSourceType lacks approval_action, which predates this PR, so it is left. "First seen on" can be wrong after a restart, so it is reworded.
…iverable drift
Review on c7d2fa7. The card's source id now hashes the drifted tool
names with the pin set, so another tool drifting under the same pins,
or one only another row's consent lists, files its own card. An
unconfigured or undeliverable drift is looked at again after an hour,
so a config set later delivers without a restart. A malformed id counts
as unconfigured, and losing the insert race to another replica counts
as filed.

Keep hostedMcpToolDefinitions.ts import-light.
The static import loads hostedMcpDriftAlertService during module evaluation. That service loads SystemSetting and attentionItemService, which loads multiple Mongoose-backed models.
The glyph-catalogue test reaches this module through toolInstallables.ts, so a catalogue-only test loads the alert dependency chain. The grants route already imports Mongoose models, so the original grants-route impact is overstated.
Load the alert service inside onAssessed. This preserves the import-time contract and does not change alert delivery.

…eep mongoose out of the catalogue import
- Read the drift-alert config on every fresh drifted assessment, before the
memo, and key filed cards by drift, recipient and pod. A newly set or changed
operator now gets their card on the next assessment.
- Never suppress an undelivered drift. Only its error line is throttled, to
once an hour while it lasts.
- Load the alert service lazily from hostedMcpToolDefinitions. The eager import
pulled SystemSetting (mongoose) into the frontend glyph-catalogue test, which
failed with "TextDecoder is not defined".

Scope hosted-MCP drift identity by pod.
When hostedMcp.driftAlert.podId changes, driftAlertSourceId remains unchanged because it contains no pod identity. The recorder then matches the old row by recipient and source. $setOnInsert preserves the old podId, so the new pod has no card in its scoped queue. Include the configured pod in the hosted-MCP source identity after reading the configuration.

The card is unique per recipient and source id and is only ever inserted, so
a source id without the pod matched the card in the old pod and left the new
pod empty. The pod is now part of the drift's source id. The error-line
throttle keys on the drift alone, as before.

Normalize the pod ID before hashing it.
If the configured podId changes only from lowercase to uppercase hex, this line generates a new sourceId for the same pod and drift. The attention-item upsert then treats it as a new card. Lowercase the pod ID before hashing, and test equivalent-case IDs. The configuration parser accepts either case, while the card’s unique key uses sourceId. (raw.githubusercontent.com)

Normalize podId to lowercase when constructing the key in the hosted MCP drift

the existing pins and drift components. Add coverage verifying that lowercase

toString(), and that is always lowercase. A case-only rewrite isn't a path we have. If a second writer appears, normalize inconfigFrom, which already validates both ids.🤖 Generated with Claude Code
