Summary

Closes #7090.

When all 128 incoming Responses WebSocket slots are occupied, a new upgrade requests closure of the least recently used eligible idle socket, then immediately attempts admission once. Only the ordinary close callback releases the old slot. If all sockets are protected or closure has not released admission yet, the existing 503 / Retry-After response remains; a later upgrade can acquire the freed slot.

  • Protect active and superseded turns, pending client tool/approval results, native steering/injection owners, and buffered output. Keep activity metadata bounded and content-free, and preserve the selected response's provider-executed tool ownership through wrappers and combo fallback.
  • Preserve the parent WebSocket turn when a child relay completes. Unregistering a controller detaches only that controller and its abort listener; it does not release a lease still owned by the parent or cancel the next combo attempt. Cancellation and final-owner cleanup still release admission.
  • Protect client function/custom calls as soon as their arguments/input become final, using a bounded item-to-call identity map. EOF before output_item.done/terminal no longer makes these connections reclaimable. Matching accepted results release protection; incomplete arguments alone do not pin a socket. Duplicate, reused, malformed and conflicting identities have regression coverage.
  • Reconnection, handshake failure and expired previous_response_id tests use the existing continuation mechanism. No idle timer or setting is added; Live/Realtime and upstream socket reuse keep their existing lifecycle.

Verification

Maintainer integration follow-up

Current candidate: 579aa0252a58e7218ead09713c02db3196887d14, with dev at 2a3fd00985c4f825af9ac41547ee7ac42fce4570 merged via a merge commit; contributor commits remain intact. At final verification the head was five commits behind current dev (within the readiness policy).

  • Separate admission-release waiting from the client close event in the disconnect regression. Each wait has its own 10-second bound; independent upstream cancellation assertions remain unchanged.
  • Upgrade retries inspect the actual refused handshake response (503 and Retry-After: 1) and wait for admission release. The intended LRU victim close/code assertions remain separate. A real-network row delays the fixture server close callback by 20ms. Disabling retry handling fails that row (1 pass / 1 fail); restored code passes both rows.
  • Bun 1.4.3 focused regression and repository guards: 183 passed / 0 failed, 4,539 assertions across 9 files. Command: OCX_TEST_RUNNER_BUN=<Bun-1.4.3-binary> <Bun-1.4.3-binary> test --isolate tests/responses/ws-capacity-reclamation.test.ts tests/responses/ws-capacity-tool-ownership.test.ts tests/codex-integration/websocket-capacity-activity.test.ts tests/codex-integration/active-registry-admission.test.ts tests/codex-integration/active-turn-disconnect.test.ts tests/ci-workflows/file-size-ratchet.test.ts tests/test-layout.test.ts tests/test-layout-tooling.test.ts tests/ci-workflows/structure-ssot.test.ts.
  • bun run typecheck, bun run structure:check, bun run privacy:scan, and git diff --check: passed. An initial guard invocation with host Bun 1.4.0 could not resolve the pinned runner; the explicit Bun 1.4.3 command above passed without source changes.
  • Final-head bun run test:changed --parallel=2: 32,957 passed / 42 skipped / 1 failed, 456,551 assertions, 1,479 files, 521.66 seconds. This local broad run is not green. The sole failure was tests/cli/cli-headless-parity.test.ts, web --model surfaces the server's backend/model pair rejection (expected exit 1, received 0).
  • Comparison at base 2a3fd00985c4f825af9ac41547ee7ac42fce4570 and final head: OCX_TEST_NO_QUEUE=1 bun scripts/test.ts --parallel=1 tests/cli/cli-headless-parity.test.ts passed 106 / 0 on each side, 431 assertions each. These small comparisons intentionally overlapped with separate sandbox homes and one worker each; broad runs retained the shared test lock. The broad failure did not reproduce in isolation, so no baseline-failure attribution is claimed. Neither this CLI file nor CLI production code is changed by the PR.
  • The preceding head's changed run had 32,954 passed / 42 skipped / 3 failed / 1 error, beginning with a 5-second timeout in tests/responses/responses-retained-reasoning-stream-recovery.test.ts. bun scripts/test.ts --parallel=2 tests/responses/responses-retained-reasoning-stream-recovery.test.ts passed 4 / 0 on both base and head (24 assertions each); the final broad run also passed those tests.
  • Exact-final-head upstream checks succeeded: Ubuntu PR CI, attempt 2, including all four test shards and ci; catalog contracts, attempt 2; React Doctor, attempt 2. All policy checks also passed. Approving fork workflows created attempt 2; approval itself was not treated as a pass.
  • Full local suite: not run under the shared-host scope exception. Two import-connected runs each took about nine minutes plus queue time; focused regressions, negative control, typecheck and the current-head Ubuntu CI provide this change's validation. The isolated CLI pass does not certify the broad local run. Current-head macOS/Windows CI and a live Codex desktop end-to-end session remain outside this PR workflow's scope.

The following contributor validation describes the previous candidate and is retained as historical evidence.

Validated candidate: 4df1ab0d72151c57111ecea75a22b7c3c0645fd6, with dev at 947b9b6ab6f259eae060c576dbc368a58d54a487 merged without rewriting existing commits. Bun test runner: 1.4.3.

Windows failure reproduced, diagnosed and fixed

Before-fix Windows/Linux run checked out the original PR head 40733efcb62ace260b3721993b024568acf00259:

  • Windows: tool ownership 7 passed / 2 failed (both combo directions reported Upstream stream ended before response terminal event); reclamation 8 passed / 1 failed / 1 error (abrupt disconnect waited for upstream cancellation).
  • Linux: both original files passed, 18 tests total. The same original files also passed on macOS.

The Windows eager relay exposed a shared-lease lifetime bug: the first combo attempt completed and released its parent's admission, so registering the next attempt immediately aborted it as an already-settled turn. Controller-scoped unregister fixes this WebSocket ownership case. Tests explicitly exercise both Darwin tee and Windows eager relay selection on every platform, plus parent/child completion, duplicate registration, late cleanup and parent cancellation.

The abrupt-disconnect test incorrectly required upstream cancellation within 5 seconds, while the existing eager relay permits a 15-second post-cancel drain to observe terminal outcome/usage. It now proves prompt socket-admission release and a successful replacement request first, then checks both the actual fetch AbortSignal and the upstream server's cancellation callback within the bounded drain budget. The fixture disables its own HTTP idle timeout, so a server timeout cannot impersonate proxy cancellation. The production drain policy is unchanged.

After-fix Windows/Linux run: all 10 jobs passed, checking out the exact candidate above. Each OS passed 95 tests / 0 failures across:

bun test --isolate tests/responses/ws-capacity-tool-ownership.test.ts
bun test --isolate tests/responses/ws-capacity-reclamation.test.ts
bun test --isolate tests/codex-integration/websocket-capacity-activity.test.ts
bun test --isolate tests/codex-integration/active-registry-admission.test.ts
bun test --isolate tests/codex-integration/active-turn-disconnect.test.ts

These are manual, read-only-token jobs on disposable GitHub-hosted Windows 2025 and Ubuntu 24.04 machines in the contributor fork. The temporary validation workflow is not part of this PR. The workflow ref differs from the tested commit; each job logs the checked-out candidate SHA and Bun revision.

Local and adversarial validation

  • macOS focused integration and source-contract checks: 837 passed / 0 failed, 19,606 assertions across the 29 files below.
  • bun run typecheck, bun run privacy:scan, bun run structure:check, and git diff --check: passed.
  • Negative controls: the new shared-owner tests and Windows eager combo tests fail with the previous lifecycle implementation; the new early-argument EOF tests fail with the previous activity tracker. Temporary probes were not committed or pushed.

These are baseline-reproduced failures under the same local conditions, not a claim that they fail on every machine. They concern management validation, config-lock classification and Claude picker state. No production workaround or unrelated test change was made for them.

Full-suite scope exception: the import-connected run already took about ten minutes on this shared local host. An additional complete local suite is disproportionate after that run, the baseline comparison, 837 focused integration/source-contract tests, and the Windows/Linux regression matrix. Validation readiness here covers the documented focused scope; it does not certify the broad run as passing. Remaining full-suite and cross-platform coverage belongs to upstream CI, and required checks must pass before merge.

At this head, upstream Cross-platform CI, Catalog async contracts, and React Doctor are awaiting maintainer approval (action_required). They have not passed; the contributor-fork matrix above is separate evidence.

Scope limits: this is WebSocket parent/child ownership coverage, not a redesign of ordinary HTTP combo admission. No live Codex desktop end-to-end session or full cross-platform suite is claimed. Existing admission counters remain; additional open/idle/close-reason telemetry is outside this fix.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults. The PR adds no workflow or credential changes; activity correlation retains no request/tool-result bodies.

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

  • Required local validation passed; commands, results, and any full-suite exception are documented.

  • I pushed my PR to a recent dev commit (at most 10 behind; a maintainer may still ask for the exact tip before merge).

  • I resolved all correct Codex and CodeRabbit findings.

  • My PR is ready for review.

Summary by CodeRabbit

  • New Features
    • Responses WebSocket connections support up to 128 client connections. When capacity is reached, the proxy can reclaim the least recently used idle connection to admit a new one.
    • Connections with active responses, pending client tool or approval results, native steering or injection sessions, or buffered output are protected. Reclaimed clients can reconnect using existing continuation handling.
  • Behavior
    • If no eligible connection can be reclaimed, new connections receive the existing 503 capacity response. Connection capacity remains occupied until a reclaimed connection closes. This does not affect voice connections or upstream connection reuse.

📝 Walkthrough

The change adds activity and tool-ownership tracking for client-facing Responses WebSockets. At the 128-connection limit, the registry attempts to reclaim the least-recently-used eligible idle socket. Tests and documentation cover protected connections, capacity rejection, reservation cleanup, and continuation handling.

At capacity, the registry attempts to close an eligible idle socket and retries admission after its close callback releases the lease. Turn completion detaches the completing controller and keeps the lease active while other controllers remain. Tests cover capacity, cleanup, and continuation handling. Documentation describes the limit and its scope.

Explanation

The production changes remain connected to issue #7090. Activity tracking, LRU reclamation, admission protection, continuation handling, response tool-ownership propagation, and parent/child lease cleanup support the required Responses WebSocket lifecycle. The tests and transport documentation validate that behavior. No unrelated production behavior, workflow change, or cleanup is demonstrated.

Full details: Linked Issues check

Explanation

Issue #7090 requires bounded, content-free metrics for Responses WebSocket open count, idle count, and close reasons. The current implementation exposes only generic admission metrics in src/lib/admission.ts (AdmissionMetrics: active, peak, admitted, rejected, and releaseMisses). src/codex/websocket-registry.ts tracks admitted sockets for reclamation, but it does not expose open/idle counts or close-reason counts. tests/responses/ws-capacity-reclamation.test.ts verifies admission recovery and reservation release, but it does not verify those required metrics. The other issue objectives have supporting evidence: pressure reclamation and protection are implemented in src/codex/websocket-registry.ts and src/codex/websocket-activity.ts; continuation and cleanup coverage is present in the focused tests.

Resolution

Add bounded, content-free Responses WebSocket metrics for open count, idle count, and close reasons. Update the metrics for admission, normal close, abrupt disconnect, failed upgrade, and pressure reclamation. Add focused assertions that distinguish retained sockets from unreleased reservations.

Resolution

Write docstrings for the functions missing them to satisfy the coverage threshold.

🩺 Stability & Availability | 🟡 Minor | ⚡ Quick win

Track client calls when their arguments become final.

CodexWebSocketActivity.observePayload() records calls only from response.output_item.done or terminal output. The Responses streaming contract makes arguments final at response.function_call_arguments.done; a client can dispatch the function without waiting for response.output_item.done. If upstream EOF follows that event, sendResponseToWebSocket() forwards it, reports an incomplete stream, and sends a protocol error without a terminal output payload. The handler then releases finishActivity(), leaving no pending result. At capacity, reclamation can mark the socket closing while the client function is still running, and the handler ignores its later function_call_output. Correlate item_id with the call_id from response.output_item.added, then retain activity until the matching result is accepted.

Remote validation while carrying this into #7218: on Windows (Bun 1.4.3) the PR's own tests fail deterministically, both in a full-suite union run and when run alone at the carried head: tests/responses/ws-capacity-tool-ownership.test.ts (combo fallback case gets an error frame 'Upstream stream ended before response terminal event') and tests/responses/ws-capacity-reclamation.test.ts ('disconnect releases admission and allows a fresh request; abrupt=...' times out at ~5.6 s). Linux passes. #7095 is held out of #7218 and stays open; a Windows-safe fix would let it land in the next batch.

…continuation tests and MiMo envelope repair (#7122, #7152, #7068, #7214) (#7218)

Carried from #7122, #7152, #7068, #7214 (one commit per source PR, original authors preserved). #7095 held out.

Remote exact-head validation (hosted GitHub Actions stalled; local tests not run by maintainer rule): union candidate codex/prr-integ-6 (368006f, tree 27fdc37) on mini (Windows) and lidge (Linux), typecheck + full suite, compared with base-dev 4ae4936 on each host.

release from waiting for the client close event: update the `until` calls to
wait for `codexWebSocketAdmissionMetrics().active` to return to baseline first,
then wait for `client.closed`, allowing up to 10 seconds for each event.

test to handle a valid 503 when the reclaimed socket’s close callback has not
yet released its slot: assert Retry-After, wait for slot release, then retry the
upgrade. Keep a separate assertion verifying that the intended idle socket
closes.

Maintainer integration into dev (MAINTAINERS.md maintainer-integration exception) for the 2.88.0 train; fixes #7090.

Exact head 579aa0252a58e7218ead09713c02db3196887d14: PR CI completed successfully (gates, test 1/4-4/4, aggregate ci); all three CodeRabbit threads are resolved, including the maintainer follow-ups that separate admission and close waits and retry the replacement upgrade after the close callback. Reviewed the design: only a socket that is idle (no active turn, no pending client tool result, not closing, no native control, empty send buffer) is reclaimed, least recently used first, and admission is released only by the real close callback.
