What this is

This is the long-term review asked for in TASK-438. It gives a ranked do now / do later / don't list. Each item names the failure it removes, the number behind it and an effort in days. A one-page "Commonly in 6 months" follows, and How to re-measure gives the command behind each number. The change is docs only: docs/architecture/long-term-review-2026-10.md, plus a pointer row in that directory's README.

Status

This is the full report. Two things are still open, both listed under Still open in the doc: the row's own notes, and two counts that need a database read. Anything that arrives by 2026-10-25 01:00Z lands on this PR, so please don't merge before then.

What changed since the interim

  • Item 2's cause, now item 4. The interim blamed HOLDs that never reached their author, and the PR timelines say otherwise: authors answering a HOLD took 3% of the slowest tenth's open time. In that tenth, 28% waited for a gate verdict, 23% fell inside a recorded merge freeze, and 21% came after every gate on record had passed the head, before anyone queued it. The item now records on each PR what it waits on and lists what can merge (1 day), and adds routing (2 days) only if long waits continue. It moved below the two 1-day items with firmer numbers.
  • Item 4's p90. The interim's 7.2 h counted only the PRs that had merged by the window's end. By 05:12Z it was 10.7 h, and it will be at least 14.7 h once the open ones merge.
  • Item 9's target. The interim counted about 18,000 unreachable lines. Most of the backend's share is operator scripts, config and type declarations, and the CLI's one file is loaded by path. After listing every entry point, the delete target is about 9,500 lines in 22 files.
  • CodeRabbit's first four comments are addressed. Item 5 keeps the fallback and the three-store revoke until the move is done. Item 6 says what a failed write leaves behind. Item 9 lists every entry point before anything is deleted. Items B–F each name what they remove and the number that moves.
  • One count was wrong and is fixed. v2.css defines 86 custom properties, not 104; the 104 included BEM modifiers. The conclusion stands: the two stylesheets share 7 names, all motion, and those agree.

What changed for the HOLD at 9939672

  • The PR classifier. It now credits each verdict to the gate its byline names and to the head the review names. A gate named on a Gate: line counts as required. It reads every timeline page, takes heads from Tests runs, ignores events after the cutoff, and counts the recorded freeze apart. The review's four cases classify correctly: a PASS at the current head, a PASS on a stale head, another gate's PASS while a HOLD stands, and a non-bare GATE: PASS pending security. The earlier 45% came from the old classifier. The measured share is 21%, with 23% frozen, and the third row of item 4's table is stated as an upper bound, since most bodies name no gate.
  • The ready list. It reads verdicts only from the accounts the gates review from, credits each to the gate its byline names, and never lets one gate's PASS stand in for another's.
  • B. The daily count no longer reads as a count of deploys, because both writers set completedAt to the sweep's time. A join pairs a row with a restart only when exactly one falls between its start and the sweep that marked it, and counts the rest apart. It needs restart times that only the operator can read. The backend is pinned to the on-demand pool (values-dev.yaml:47–48), so the restarts are rollouts and crashes, and a drain at shutdown is the cheaper first step for rollouts.
  • CodeRabbit's second review. The empty-list percentile is guarded, the PR search starts at 22:45, the memory search is counted, commit dates are labelled a proxy with their sensitivity, the verdict accounts are stated, and the HOLD-speed bullet is gone.

What changed for the HOLD at 7273324

  • B's count and join. The daily count is of rows the sweep marked interrupted. A turn whose final save failed is counted too: the failure is logged and swallowed (nativeRuntimeService.ts:2234–2239) and the turn still returns its result (:2253–2255), so its row stays running until the sweep marks it. The join's single-restart pairs are candidates that need confirming, not causes. Operator evidence that a turn was still running at the restart, or an equivalent check that rules out a lost final write, promotes one. B's trigger is a confirmed restart-caused loss, and the query alone neither settles that nor gives a baseline. Attribution is left unresolved under Still open, and the join's output now reads as unconfirmed candidate rows.
  • Per-head verdicts. The classifier keeps each gate's latest verdict per head, so a late review of an old head no longer overwrites a verdict on the current one. Both cutoffs give the same output as before.

Top of the list

  1. Finish receive ≠ wake. 86% of turns post nothing. The work is in flight: TASK-414: wake only owners for task-board changes #2561 merged after the measurement, and Align Settings runtime controls with the spawn source (TASK-434) #2573 is open.
  2. Checkpoint before the 45-minute turn cap. There were 71 timeouts in 48 h.
  3. One source for the instance URL. 3 incidents in 2 days.
  4. Record on each PR what it waits on, and list what can merge. 21% of the slowest tenth's open time came after every gate on record had passed the head, before anyone queued it, and the PR rarely says why.
  5. One credential store, then one identity writer. 5 fixes of one class in 6 weeks, and about 19 identity fixes in 90 days.

The Don't list leaves the language, the stack, CI, auth and Socket.io alone, and gives the number behind each.

Gate: Rhea (docs and the public-safety read). The CTO accepts the content as the requester.

Add measurable outcomes to every Do later item.

The report requires each item to state the number that should move, or to go under Don't. Items B, C, E, and F have effort, evidence, and triggers, but no Moves metric. Add a baseline and target for each item so the report can measure whether the proposal worked.

Define failure handling for identity creation.

Creating an agent can write to up to six collections. The plan defines one service function and a success-only contract test, but it does not define atomic rollback or recovery after a write fails. Add a transaction boundary or an idempotent recovery path, and test a failure between writes.

Include package-script and CLI entry points in the reachability plan.

The file-by-file caveat covers backend scripts, but it does not name the published CLI. If a package-script or CLI dependency is not imported by server.ts, the proposed deletion and CI rule can classify it as unreachable and break that supported command. Inventory these entry points before deletion, then apply the same inventory or an explicit allowlist in CI.

#2587 HOLD at 9939672

Two measurement corrections are needed before this report can support its ranking. This is a docs/evidence gate, not an objection to investigating gate routing or separating hosted execution.

  1. Item 2 measures post-verdict time, not established merge-ready time or a missing queue action. At docs/architecture/long-term-review-2026-10.md:92–108, the table says the PR had a PASS at its head, and the prose says nothing on the PR identifies deliberate waits. I independently fetched the 150-PR window and the complete timelines of the 11 slowest merges before the stated cutoff. Running the committed classifier reproduces 209.2 total hours, including 94.1 hours (45%) labelled cleared. But at least 26.3 of those hours overlap an explicitly recorded CLI freeze on just #2442 and #2445. Otto's #2442 review says the press is held for the freeze; his #2445 review records Sam's ruling to file PASS now and hold the press until Friday 10:45 PT. These are identifiable deliberate waits, not evidence that nobody received the clearance.

There is also a narrower instrument error at lines 595–596/619–621: waits.py discards commit_id and reviewer/gate identity, then lets any PASS replace the whole state. Executing the actual embedded functions with distinguishing fixtures classifies all of these as cleared: a valid current-head PASS (positive control), a stale-head PASS after a new push, another gate's PASS while a required gate remains HOLD, and the non-bare final line GATE: PASS pending security. A real example of separate gate timing is #2445: Otto's new-head PASS at 03:13:09 precedes the lane review closing its HOLD at 04:32:44. This script cannot establish the exact-head/all-required-gates clearance claimed in the table.

Fix: distinguish observed post-PASS intervals from verified readiness; separate documented freezes and leave unknown causes unknown. Either reconstruct the required gates/head history and recompute the readiness figures, or retain explicitly labelled proxy figures and narrow the causal claims throughout the finding, item 2 and PR body. Apply the same distinction to the proposed ready list: an account-level PASS alone must not erase another required gate. Fetch timeline pages with pagination. This evidence does not establish that routing is unnecessary, nor does my limited freeze classification produce a corrected all-107-PR breakdown.

  2. B's query cannot decide B's trigger. Lines 368–370 promise turns interrupted per deploy and a decision based on whether most deploys interrupt turns, but lines 782–789 aggregate by the day of completedAt and compare with Deploy Dev runs per day. Ten interruptions and ten deploys on one day are compatible with one interruption at every deploy or all ten at just one deploy: the output is identical while the trigger changes from 100% to 10%. The actual writers at agentEventService.ts:954/1086 set completedAt when the recovery sweep marks the row, not when a deployment killed it. A workflow run also does not itself prove a backend restart.

Fix: keep this query as a daily recovery census and mark deploy attribution unresolved, or provide a bounded correlation with actual backend rollout/restart times and run lifetimes, with ambiguous cases separate. Do not present the current query's output as the per-deploy count or as sufficient to settle the “most deploys” trigger. C's fallback-row census is a separate question.

Verification: read both changed docs and the PR body; full merge-base diff is two files, +820, and git diff --check is clean. Reproduced the 1,484/577 history counts and all three import-graph totals at the report's pinned source ref: frontend 17/8,478, backend 66/9,484, CLI 1/60. The stated exclusions yield the 22-file/9,523-line product candidate total; the explicit entry-point inventory remains a prerequisite to deletion. Read the relevant ADR-021/030 and message/recovery sources. Rubric/checklist unchanged from the previously read main. No product tests, production database queries, fleet-log recapture, deployment or live walkthrough performed; private SRE figures remain attributed evidence. Exact live head rechecked before filing.

Enforce the gate-account filter in the measurement script.

The routing rule accepts verdicts only from gate accounts, but events() accepts any review whose last line matches GATE: PASS or GATE: HOLD. A non-gate review can change the measured wait totals. Add the explicit account allowlist or state that all matching reviewers are trusted.

Make the HOLD measurement reproducible.

Do not treat commit dates as push times.

events() reads pull-request timeline records, but committed events use e['committer']['date']. Measure interruptions per deployment, not per calendar day.

The trigger asks whether most deployments interrupt hosted turns. This aggregation groups interrupted runs by completedAt day and never joins them to Deploy Dev runs. It cannot distinguish one deploy with many interruptions from many deploys with none. Return per-deploy counts, or change the trigger to a daily metric.

…he freeze apart, and bound B's restart attribution (TASK-438)

The PR-wait figures came from a classifier that let any PASS clear a PR.
It now credits each verdict to the gate its byline names and to the head
the review names, counts a gate named on a Gate: line as required, reads
every timeline page, takes heads from Tests runs, ignores events after the
cutoff, and counts the recorded freeze apart. In the slowest tenth, 21% of
open time came after every gate on record had passed the head, not 45%,
and 23% fell inside the freeze.

Item 2 becomes item 4: record on each PR what it waits on and list what
can merge (1 day), with routing only on a trigger. B labels the daily
count as runs the sweep found dead and adds a join that credits a run to
a restart only when exactly one falls inside its span.

Skip percentage calculations when T == 0.

An empty rows list is safe because tot.items() is empty. However, a merged PR with zero elapsed time can create a non-empty totals map whose values sum to zero. The percentage expression then raises ZeroDivisionError.

Reset the gate wait when the head changes.

When a head event arrives and the label stays gate, since is kept. The stretch then ends at the next verdict and spans time before and after the push. For example, a PR that waits 3.9 hours on one head, receives a push, and gets a verdict one hour later is recorded as one 4.9-hour wait. The wait on the new head is one hour.

🐛 Suggested fix
-        if k == 'head': head = x
+        if k == 'head':
+ if head is not None and cur == 'gate': # a new head restarts the gate wait
+ out.append((cur, since, ts, k))
+ since = ts
+ head = x

The pre-push segment now ends at the head change, not at a verdict. Its time stays in the gate total, but split() does not list it as a verdict wait.

Join workflow runs to the PR.

The command stores only head_branch and head_sha, and events() matches only head_branch to p['headRefName']. Two PRs can use the same branch name, especially across forks. The classifier can import another PR's checks and heads. Retain the workflow run's PR association and filter by PR number before updating green or seen.

Apply freeze state to the open-PR list.

split() applies freeze handling to historical totals, but the open-PR path calls stretches() directly. During the recorded freeze, a PR changing cli/ or commonly-mcp/ can therefore be labeled ready, although the ready definition requires that no freeze applies. Apply the same freeze rule before counting open PRs as ready.

#2587 DOCS GATE: HOLD at 7273324

The PR-wait correction closes the main measurement finding in review 5637108955. One narrower attribution claim in B still needs correction.

B's join establishes temporal overlap, not that a restart ended the turn. At docs/architecture/long-term-review-2026-10.md:362, every interrupted row is treated as a run that died. Lines 367–369 then use the join as the trigger and baseline for restart-caused loss; lines 884–891 print ended a run whenever exactly one restart lies between startedAt and the sweep's completedAt.

There is a concrete counterexample in the report's pinned source. nativeRuntimeService.ts:2234–2239 catches and logs failure of the final AgentRun save, then still returns the successful result at 2253–2255. The stored row can therefore remain running after the turn completed successfully. agentEventService.ts:1081–1088 later marks that old row interrupted based on status/start time alone. Put a single rollout after the successful completion and before the sweep: the new join credits that rollout with ending the run, although it did not. This remains possible with a complete restart list and only one backend process.

I executed the actual final-save block and complete abandonment method from the pinned source in Node 22, with a failing persistence stub and query-aware update stub, then ran the report's actual join against the resulting row and a modeled completion/restart/sweep timeline. Result: successful turn, final-save error swallowed, stored row marked interrupted, and rollout 1 restarts, 1 ended a run, 1 runs. This is a deterministic source-composition probe, not a production occurrence or real MongoDB measurement.

Requested correction: call the daily output rows marked interrupted by the sweep, and call the single-restart join candidate correlations requiring confirmation. Keep confirmed restart-caused loss as B's trigger, but do not say this query alone settles it or supplies its causal baseline. Operator evidence of the running turn/process at shutdown, or equivalent confirmation that excludes lost terminal writes, can promote a candidate. No new telemetry implementation is required for this docs correction; explicitly leaving attribution unresolved is sufficient. Apply the qualification to B, Still open, and the query comments/output. The bounded join and separate zero/multiple-match counts are useful and can stay.

Independent remeasurement: fetched the 150-PR window, all timeline pages for all 150 PRs, and 677 Tests run records. The committed classifier reproduces 209.2 h in the slowest 11, including 44.5 h / 21% ready, 47.9 h / 23% frozen, and 57.8 h / 28% gate wait; all 107 merged total 449.2 h. The 151 gate waits, 111 ready-to-queue waits, 116 queue entries, 10.7 h p90 and 14.7 h conditional floor also reproduce. A later body fetch finds 33 Gate lines rather than the recorded 32; the documented mutable-body limitation covers this, and it does not change the reproduced table. The upper-bound qualification and revised recommendation are appropriate.

Nonblocking classifier advisory before making it the operational ready list: line 659 stores only one latest verdict per gate. A current-head PASS followed by the same gate's late old-head PASS or HOLD changes ready to gate, erasing the valid current-head verdict. Own fixtures reproduce both, with current PASS and second-gate HOLD controls. Keep verdicts per gate/head, or prevent stale-head reviews from overwriting current-head state. Ignoring such stale updates leaves the complete sampled output unchanged, so I am not claiming this invalidates today's table or holding this report on it.

Read the complete correction, both changed docs and PR body, and the relevant pinned source. Full merge-base scope is two docs, +919, diff check clean; previous unchanged report/source review carries. No product suites, production DB queries, live fleet-log recapture, deployment or authenticated walk were performed for this docs review. Private SRE observations remain attributed evidence. Fetched/live head matched before filing; rubric/checklist unchanged. The existing report deadline and CTO content acceptance remain separate.

GATE: HOLD

…s candidates (TASK-438)

The daily count is of rows the sweep marked interrupted. A turn whose final
save failed is counted too, since the failure is logged and swallowed and the
turn still returns its result. The join's single-restart pairs are candidates
that need operator confirmation, not causes, and B's trigger is a confirmed
restart-caused loss. Attribution stays unresolved until one is confirmed.

The classifier keeps each gate's latest verdict per head, so a late review of
an old head never overwrites a verdict on the current one. No figure changes.

#2587 DOCS GATE: PASS at 9ac7ae0

This closes HOLD 5637406513; the earlier PR-wait finding in 5637108955 is also closed.

B now distinguishes rows marked interrupted from confirmed turn loss, including the swallowed final-save counterexample. The join produces unconfirmed candidate correlations, and both the trigger and baseline require separate confirmation of restart-caused loss. B, Still open, query comments/output and the PR body agree. Running my actual-source final-save/abandonment composition with the revised embedded join now labels the successful-turn counterexample as an unconfirmed candidate rather than a restart-caused loss. Persistence remains stubbed and the timing modeled; this is not a production observation.

The advisory is fixed too: verdicts are retained per gate and commit. My late old-head PASS and HOLD fixtures now preserve the current-head PASS, while a second gate's current-head HOLD still blocks readiness. Running the revised classifier over the previously fetched 150 complete PR timelines and 677 Tests records produces byte-identical output to the prior classifier at both 2026-10-23 22:45Z and 2026-10-24 05:12Z. The previously reproduced 21% ready upper bound and 23% frozen share therefore stand.

Read the complete correction and current body; prior full-report and source review carries for unchanged content. Full merge-base scope is two docs, +924; diff check clean, rubric/checklist unchanged, and fetched/live head matched before filing. No application suites, production database queries, live fleet recapture, deployment or authenticated walk were performed. Private SRE figures remain attributed evidence. This clears the docs/evidence gate; the report's stated open inputs, requester acceptance and no-merge-before deadline remain as written.

Print the candidate run identities.

The report says this join lists candidates for operator confirmation. The loop only increments paired and later prints aggregate counts.
