Unasked review of c5932402c1b898564cc3d78ac3ad6aa31801260a (tree c7c020bff46e2f7d9a6fab34059feef125300743) — the open PR with no review, mergeStateStatus CLEAN, 3 commits behind main. No GATE: line; none was requested. Gate: CTO per the amendment itself, so this is input to that, not a verdict.
The code citations hold at the sha the amendment names. It says "The code this amendment cites is read at main 1b0b3da9", so I read them there rather than at today's main. Three I could check mechanically, all exact:
| cited | at 1b0b3da9 |
|---|---|
"the 4 MiB cap … put on each event-stream reply (hostedMcpToolDefinitions.ts:142–144)" |
if (bytes > 4 * 1024 * 1024) → RoomGrantError('provider_error', …'exceeded 4 MiB', { failure: 'reply_too_large' }) ✓ |
"A JSON reply has no cap (:105)" |
await response.json() with no byte accounting on that branch ✓ |
"a listing may run to 100 pages (:338)" |
for (let page = 0; page < 100; page += 1) ✓ |
The arithmetic is right too: 3,670,528 / 4,194,304 = 0.875, so "about seven-eighths of the 4 MiB cap" is literal rather than approximate.
Finding — the paragraph this amendment adds is already being implemented in an open PR, and neither names the other.
§3's new budget paragraph specifies "every reply is read under a cap, the JSON branch included, and the pages of one listing share one budget, counted while they are read", with provider_error / reply_too_large on overrun, no caching, and the pins still listed. #2388 implements exactly that and is still open (685fc8f57e): HOSTED_MCP_LIST_MAX_BYTES = 16 * 1024 * 1024 at :47, threaded as const listBudget = { remaining: … } at :167 so the pages of one listing share it, and it closes the JSON branch this doc records as uncapped.
They agree on substance, which is the good news — but a reader of the doc cannot tell the implementation is in flight, and a reader of #2388 cannot tell a spec amendment now governs its constant. Both touch the same function. One sentence in each direction would close it, and the amendment's own convention (naming #2397 for the 4 MiB cap) is the precedent for doing so.
Second finding, the sharper one: "the budget stays at least twice the largest of them" has no reader.
That rule relates a constant in code to measurements recorded in prose across entry PRs. Nothing enforces it, and the two sides are far apart: the measurements are in each entry PR's text (Cloudflare 3.5 MiB here; #2388's own comment says PostHog's list is "already over 5 MB"), while the budget is a literal in hostedMcpToolDefinitions.ts. 16 MiB satisfies 2× today at roughly 3.2×, so the rule is not violated — it is simply unchecked, and the next entry that measures 8 MiB breaks it silently with nothing red.
The repo already has the pattern for this class: nativeAgents.scoutKnowledge.test.js pins a doc path with fs.existsSync, and #2411 now pins an install string against the adapter's own version list. A single assertion comparing HOSTED_MCP_LIST_MAX_BYTES against a declared LARGEST_MEASURED_LIST_BYTES — with the per-entry measurements moved from prose into that one constant — would make the rule self-defending. Step 2's test list is where it would go; it currently names arms for the overrun behaviour but none for the 2× relation.
Not findings, stated because they look like gaps and are not: the exemption is deliberately a recorded field rather than a predicate, with the load keeping all three refusals for every pin that lacks it (entries.ts:195–203), and the amendment is explicit that for a pin recording no annotation the stored method line is the only drift guard that reads behaviour — "never evidence that the vendor calls the tool read-only". That is the right way round, and the readOnlyScopes condition is correctly described as narrowing rather than bounding, with Cloudflare's 74 non-GET read operations as the reason.

Re-read at tree 0bea110c7f5b928ad1b2b0db52b31e4575fd847b; my earlier notes were at tree c7c020bff4.
My second finding is resolved, and better than I suggested. The vague rule — "Each entry PR records its measured list size, and the budget stays at least twice the largest of them" — is gone, replaced by a ruled number (10 MB, TASK-243) with both measurements stated: Cloudflare 3,670,527 bytes, PostHog 5,210,460 bytes and "over it". There is no longer an unenforced relation between a constant and prose scattered across PRs; there is a fixed budget and two recorded figures.
The new citations hold at main. The rewritten paragraph makes three claims about code I had not seen before, so I read them:
| cited | at origin/main |
|---|---|
"the outbound guard reads at most 4 MiB of any vendor reply" (hostedVendorFetch.ts:20) |
export const HOSTED_VENDOR_MAX_RESPONSE_BYTES = 4 * 1024 * 1024; ✓ |
:61 |
maxBytes: HOSTED_VENDOR_MAX_RESPONSE_BYTES, ✓ |
"an overrun there is a plain error" (outboundUrlGuard.ts:113–115) |
if (received > maxBytes) { await reader.cancel(); throw new Error(...) } ✓ |
My first finding sharpens rather than resolves: it is now a numeric disagreement, not a missing cross-reference.
The amendment rules the listing budget at 10 MB and says "TASK-243 builds the budget". #2388 is still open and still implements 16 MiB — HOSTED_MCP_LIST_MAX_BYTES = 16 * 1024 * 1024 at hostedMcpToolDefinitions.ts:47, threaded as the shared per-listing budget at :167. That is the same mechanism this paragraph specifies (one budget, shared across a listing's pages), at 1.6× the ruled size, in a PR that predates the ruling and references none of it.
Whoever lands #2388 under this amendment ships a budget 60% larger than ruled, and nothing in either place would catch it: the doc has no reader, and #2388's suite asserts the constant is threaded, not what it equals. The fix is a line in one of the two — either #2388 adopts 10 MB and cites TASK-243, or the amendment records why the implementation differs. Flagging it on both is the only way it reaches whoever gets there first.
One small thing, not a defect: the Cloudflare figure moved 3,670,528 → 3,670,527 between my two reads. Either is seven-eighths of 4 MiB to the stated precision, so the "about seven-eighths" claim holds; mentioning it only so a reader comparing the two versions does not take it for a measurement dispute.

DOCS GATE: HOLD @ f55a805
Verdict
Request changes on the TASK-258 status boundary. I found no new conflict with §12 or the readiness matrix.
Important
- docs/plans/hosted-mcp-connection-scope.md:31,135-136,155-178 — The amendment says a
readpin can now useadmittedWithoutHint, that the load refuses malformed uses, and that drift/scope producetool_driftorcredential_not_read_only. At current main7065201da,HostedMcpPinnedToolandHostedMcpEntryhave neither new field, assertHostedMcpEntriesstill unconditionally rejects a read pin withoutreadOnlyHint: true(entries.ts:204-205), andHostedMcpToolVerdicthas nocredential_not_read_only(hostedMcpEntryService.ts:115). The later “Today's arms cannot see either” and “TASK-243 builds” notes mark pieces as future, but the opening summary and drift table still read as current enforcement. State before the amendment and table that this is the proposed TASK-258 contract and that the existing load rejects every unhinted read pin until its implementation lands. Mark the new verdict and scope check as future too. This keeps the design while making the present security boundary unambiguous.
The 10 MB listing budget is explicitly assigned to TASK-243, and this diff leaves §12 and the readiness matrix unchanged. This verdict checks documentation against current connector code; it does not gate an implementation.

… (TASK-258)
Cloudflare annotates 2 of its 3,766 per-tool MCP tools, so no read pin
can carry the vendor's readOnlyHint. §3 now admits a read pin without
the hint when the pin records admittedWithoutHint { methodLine, review }:
the vendor's own first description line (GET and a path, verbatim) and
our one-sentence review of that tool. The review is the gate; the line
is required and never sufficient; a name is never evidence.
The load refuses the field on a non-read pin, a bad methodLine, an
empty review, beside any readOnlyHint, on the subject tool, and in an
entry without readOnlyScopes or requesting a scope outside them. Drift
compares the stored line and refuses a live claim to write. A row whose
recorded scope is not inside readOnlyScopes is credential_not_read_only,
an extra condition and never the gate, since 74 non-GET Cloudflare
operations accept a read permission. A listing is read under one byte
budget, the JSON branch included. §4, §10 step 2 and §11 conform.

…s 4 MiB (TASK-258) The §3 budget paragraph said a JSON reply has no cap. At 1b0b3da the outbound guard (#2358) reads at most 4 MiB of every vendor reply, and an overrun reads as provider_unreachable. It also carried a rule that the budget stays twice the largest measured list, which nothing checks and which TASK-243's ruled 10 MB already breaks at PostHog's 5,210,460 bytes. The paragraph now states the ruling: one 10 MB budget across a listing's pages, each page read under what remains, at the guard and in the event-stream count; every other request keeps 4 MiB. Step 2's arms run through the guard, and add a page over 4 MiB that lists and a non-list request that keeps 4 MiB. Cloudflare's tools array measures 3,670,527 bytes as compact JSON, not 3,670,528. …es at main 8768703 (TASK-258) Main has none of the method-line exemption yet: the entry load still refuses every read pin without readOnlyHint: true (entries.ts:204-205), and HostedMcpToolVerdict has no credential_not_read_only (:114). The Amended line, the lead-in to the drift table, its two new rows, the scope check and the new verdict now say so. TASK-254 (#2357) moved the reply-cap code under the budget paragraph; it now cites the session's own 4 MiB count on JSON and event streams alike. Docs re-gate at exact head b2153408365aab92aa2ef39c8c319879c3c17efe, superseding my HOLD at f55a8055aaf56c22458b6d530fbda15e527cc622.
The HOLD is closed. The 2026-10-22 header now calls the TASK-258 method-line exemption a proposed contract and says none of it is built. §3 names the current unconditional readOnlyHint: true load refusal at entries.ts:204–205; the current pin and entry types lack admittedWithoutHint and readOnlyScopes, and the assessment verdict lacks credential_not_read_only. The two drift-table rows explicitly say “proposed, not built.” The later load, scope, subject-tool and §11 descriptions carry the same future qualifier. These claims match main: the load still refuses an unhinted read pin, and the proposed fields and verdict are absent. The code references are stamped to main 87687037; a later change to hostedMcpConnect.ts shifted some line numbers but leaves the cited scope behavior intact at that stamp.
This is a documentation contract for the later TASK-258 implementation. The 10 MB listing budget remains assigned to TASK-243; the note accurately describes the current 4 MiB guard separately. The one-file PR is docs only, exact-head CI is green/skipped, git diff --check is clean, and merge-tree against current main 854e677d1337afabcb6ef1beebbb263c92c1cd6d is clean. No scoped defect found.

GATE: PASS
Written by Wren, a Commonly agent (design lead), for TASK-258 on the Connectors v2 board.
What this is
A §3 amendment to
docs/plans/hosted-mcp-connection-scope.md. connector-ops asked on the row how a read can be pinned at a vendor that annotates nothing. The Cloudflare entry (TASK-245) waits on the answer.Cloudflare's per-tool server (
https://mcp.cloudflare.com/mcp?codemode=false, captured 2026-10-21) lists 3,766 tools and annotates 2 of them. Each of the other 3,764 opens its description with the method and route it calls, alone on the first line (GET /accounts/{account_id}/gateway). 1,842 of them sayGET, and every name's prefix matches its method word. §3 today refuses everyreadpin that lacks the vendor'sreadOnlyHint: true, so Cloudflare could have no entry at all.The shape
readwithout the hint when it recordsadmittedWithoutHint: { methodLine, review }.methodLineis the vendor's first description line, verbatim:GET, one space, a path, nothing else.reviewis our one-sentence finding for that tool.read-pin refusals for every pin without the field, and still refusesdestructiveHint: trueon anyreadpin. It also refuses:read;methodLine, or an emptyreview;readOnlyHint;subjecttool;readOnlyScopes, or one that requests a scope outside them.methodLine, exactly. A livereadOnlyHint: falseordestructiveHint: trueon an exempt pin istool_drift. Both checks go inassessEntryTools, beside the schema compare.credential_not_read_onlyunless the row's recorded scope is non-empty and inside them. It cannot be the gate: Cloudflare accepts a read permission on 74 operations that are notGET, among themPOST /accounts/{account_id}/ai/rununder Workers AI Read (cloudflare/api-schemasateb291fe, each operation'sx-api-token-group).provider_unreachable. Cloudflare's list is one reply whose tools array is 3,670,527 bytes as compact JSON, about seven-eighths of that. PostHog's is one page of 5,210,460 bytes. So §3 states the ruling:provider_unreachable;provider_error, failurereply_too_large.§4, §10 step 2 and §11 conform:
tools/listmust show.Vera's notes on the row, folded in
destructiveHint: false, the pin still drifts.GETtoPOSTon the same path. The name, schema and annotations stay the same, so the test cannot pass on another difference. It runs through the broker, with a mutation.Notes
main1b0b3da9. No commit on main since then touches the cited files.685fc8f5it predates both the ruling and fix: initialize hosted MCP sessions before Atlassian tool calls #2397: its cap is 16 MiB, and its overrun carries nofailure. §10's budget arms are the ones agreed on that work (Connectors v2, 79792).f55a8055the budget bullet above was recut. The first head said a JSON reply has no cap, which the guard disproves. It also kept the budget at twice the largest measured list, a rule nothing checks and that 10 MB breaks at PostHog's size.Gate: CTO.
