feat(cli): supervise a codex turn so a stall is killed and named, off unless COMMONLY_SUPERVISE_RUNS=1 - #2769
Puts a supervised turn for commonly agent run on main, off unless COMMONLY_SUPERVISE_RUNS=1. A codex turn that stalls is killed with its process group and settles with a named failure, instead of hanging until the turn budget runs out.
What changes
cli/src/lib/supervised-turn.js (new). One model turn, on one of two paths:
Flag set: the turn runs under superviseRun from @commonlyai/seats.
stall window: codex 15 min, claude off (the evidence is in the file);
cancel grace: 15 s, then a process-group kill;
wall-clock backstop: the turn budget + 60 s;
every settle carries a named failure.
One metadata-only line per turn goes to ~/.commonly/state/turns/<agent>.jsonl (COMMONLY_TURN_LOG_DIR overrides it). Every field is whitelisted, and the line carries no text.
Flag unset:adapter.spawn(prompt, ctx) gets the caller's own ctx object, untouched, and nothing is written.
The trace store is in memory and keeps no event data.
cli/src/commands/agent.js:
performRun takes superviseRuns, which defaults to COMMONLY_SUPERVISE_RUNS === '1';
the one spawn call goes through turnRunner.run, so failures throw exactly as the adapter's own would, and classification and retry are unchanged.
Not in this PR
Named provider failures on the wire (provider_unavailable, refusal_retry_loop). They will come as a separate PR on main's key proxy.
Turning supervision on by default for codex. That follows after a local Codex run shows a stalled turn named and killed.
Proof
Flag off is pinned byte for byte.fixtures/run-loop-flag-off.golden.json was captured by running the harness against main's own agent.js. The PR's agent.js must reproduce it for flag unset, '0', 'true' and ''. The fixture uses synthetic agent names and ids only.
Flag on uses the real codex adapter against a stub codex and the local fake provider:
the reply is posted;
the turn line says supervised/ok;
neither the reply nor the key is in any file the run left.
A stalled real child is named stuck and killed together with its grandchild. Its event is held and retried, as main does for any failed turn, and the retry posts.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
The CLI adds optional supervised adapter turns controlled by COMMONLY_SUPERVISE_RUNS, with configurable stall policies and filtered JSONL metrics. It also adds an offline command that summarizes turn outcomes and durations from a selected seat’s logs.

Raise the stall window in both real-child tests.
The supervisor starts the stall timer before the child’s 'spawn' callback registers it. If registration occurs after the 60 ms stall and 40 ms grace, the supervisor settles without the child in its kill set. Test cleanup then detects the live child.
Use 300 ms to reduce this scheduling race. This reduces the risk but cannot guarantee independence from arbitrary spawn latency.

Cover unsafe seat names and invalid event labels at the metrics-writing path.
The existing metrics test reaches both runner paths, but it uses only the safe seat nib and valid event types. The direct metricLine test omits eventType. Removing seat sanitization or event-label validation would therefore leave the current assertions passing.
Add a runner-level test for ../../etc/x, a 200-character seat name, and an event type containing spaces or newlines. Use COMMONLY_SEATS_CANARY_DIR for the test directory, then assert the sanitized/truncated file path remains inside that directory and the written eventType is null. An empty seat tests only the existing seat || 'unknown' fallback, not sanitization. The existing write-failure test already detects an ignored COMMONLY_SEATS_CANARY_DIR, so a separate override-selection test is unnecessary.

A regression that ignores --log, parses the seat incorrectly, or mishandles --until could pass these tests. Add a test that invokes the entrypoint with a temporary log, --log, --until, and --json, then asserts the exit status, selected seat, counts, and redaction.

Reject missing or non-numeric option values in main.
The opt helper returns the next token without checking that it exists or is a value. Number() is applied without a finiteness check. Four invalid invocations follow:
--log with no value: opt returns undefined, so fs.readFileSync(undefined, 'utf8') throws a TypeError. The process exits 1 with a stack trace.
--hours with a non-numeric or missing value: Number returns NaN. baseline then calls new Date(NaN).toISOString(), which throws a RangeError. The process exits 1 with a stack trace.
--timeout-ms with a non-numeric or missing value: timeoutMs is NaN. The reached the turn budget count stays 0 for every input. The report prints turn budget NaNs and exits 0.
--until with no value: opt returns undefined, so the window silently ends at Date.now(). The command exits 0.
The same function already rejects an invalid --until date with status 2. Malformed option values should use that path. One shared fix in main covers all four cases:

On the outside-diff Minor (seat-log-baseline.mjs main: missing or non-numeric option values): agreed, fixed in 4012d4d. A missing or non-numeric --log, --hours, --timeout-ms or --until value now gets the status-2 usage path that an unparseable --until already had; --hours and --timeout-ms must also be positive. A new test covers all seven malformed shapes plus a well-formed run, and two mutants were checked: dropping the missing-value check fails it, and so does dropping the number check.

Reject hours values that make the window invalid.
If the user passes --hours 1000000000000, num() accepts the finite positive value. baseline() then computes a date outside the supported range, and toISOString() throws instead of returning status 2. Validate the computed window boundary before reading the log. Add this operand to the usage-error test.

--until now accepts only the documented ISO form and must survive a calendar round trip. Oct 24 2026, 2026-02-31T00:00:00Z and a zone-less 2026-10-24T07:00:00 are usage errors (status 2).
A --hours that pushes the window start outside the Date range is a usage error before the log is read.
All four operands are in the malformed-option test. Container run: 24/24 three times; mutants that drop the regex, the round trip or the range check each fail that test.
The agent runner emits both messages after spawning. outcomeOf returns null for them, so the baseline can leave the turn open until a later spawn, restart, or window boundary. This misclassifies the turn and can distort its duration.
Suggested fix
const outcomeOf = (msg) => {
+ if (/response already routed by agent tool/.test(msg)) return 'private_response';+ if (/no private response \([^)]*\)/.test(msg)) return 'no_private_response';
if (/no wrapper-post \(NO_REPLY\)/.test(msg)) return 'no_reply';
This change is sufficient for these two phrases because outcomeOf is the parser’s outcome-dispatch point. Keep the separate unparenthesized upstream-refusal message and credential-refusal path distinct.

outcomeOf now closes an ask turn on "response already routed by agent tool" (private_response) and on "no private response (…)" or the upstream-refusal form ending "— no private response" (no_private_response).
"spawning" is logged before the credential lease (agent.js), so "spawn credential refused" now clears the open turn and counts it as counts.spawnRefused, kept apart from outcomeOf and the processing-failure path. It shows in the rendered summary.
There's a new test, "ask outcomes close a turn, and a refused spawn credential is not a turn". Container run: 25/25 three times. Removing either outcome or the refusal branch fails that test.
Exclude the first polling line from the restart count.

Only a "polling … for events" line that follows an earlier one counts as a restart, including when the earlier one is before the window. A log's first start is not a restart.
"spawn credential lease failed … — not spawning" discards the open turn and counts leaseFailed. "ERROR reply NOT delivered" marks the turn, so the processing failure that follows is reply_delivery_error (counted as replyLost), not adapter_error. failureOf is unchanged for every other failure.
There's a new test, "a first start is not a restart, a failed lease is not a turn, and a lost reply is not an adapter error". Container run: 26/26 three times. Reverting each of the three changes fails it.

Defect: lost replies and lost leases are counted as ok turns. The metric line is written as soon as turnRunner.run returns (runPlain and runSupervised in cli/src/lib/supervised-turn.js), with ok: true. The post happens after that, in cli/src/commands/agent.js: the stood-down branch on claimKeeper.isLost() (about line 2246) and the deliverChatReply catch that rethrows as "reply NOT delivered" (about lines 2265–2278). So a turn whose lease was lost, or whose reply never reached the pod, still lands in seats-canary as outcome: "ok". That contradicts the stated attack: "a failed lease and a lost reply are never counted as a turn." No test covers this; I found it by reading the code, not by a live run.
Checked and holding:
A refused spawn credential (own-key-requires-scoped-spawn, about line 2046) returns before turnRunner.run, so it writes no line.
The plain path passes the same prompt and the same ctx object to adapter.spawn. The golden in run-loop-flag-off.golden.json matches. The three targeted suites pass 26/26 with a clean environment.
Second issue: flag off is not side-effect-identical to main. The legacy path still runs appendFileSync to ~/.commonly/state/seats-canary/<seat>.jsonl on every turn. The module header and the agent.js comment say this on purpose, but the attack said "with the flag off, behaviour is identical to main." Either gate the write behind COMMONLY_SUPERVISE_RUNS=1, or reword the claim to "turn behaviour is identical; a metric line is written on both paths."
To clear the hold: record the outcome after delivery settles, or add a stood-down / delivery-failed outcome and count only delivered turns. Add a test for each: a lease lost mid-turn, and a deliverChatReply throw.
Non-blocking: the performRun JSDoc (about line 1374) now sits above the new // block and export { supervisedRunPolicy } rather than above performRun.
The reason will be displayed to describe this comment to others. Learn more.
Tier 2 gate for #2769 at head b5ce511 (supervised turn runner behind COMMONLY_SUPERVISE_RUNS, off by default).
Verdict: HOLD on one attack clause. Everything else checked out.
1. Blocking: a lost reply is recorded as an ok turn.
The attack says a lost reply is never counted as a turn. At this head it is counted as ok in the canary metric:
cli/src/lib/supervised-turn.js:248-249 (runPlain) writes ok: true as soon as adapter.spawn resolves.
The reply is posted after the spawn (cli/src/commands/agent.js:2085 spawn, then the post path), so when the post fails, the metric has already said ok.
The failure is logged at agent.js:2273 ("ERROR reply NOT delivered to pod …"), which the metric never sees.
Failure scenario: the pod POST returns 500 on every reply for a seat. Every turn is recorded ok: true in ~/.commonly/state/seats-canary/<seat>.jsonl, so the canary reports the seat healthy while no reply reaches anyone.
Reading I took: "counted as a turn" means counted as an ok turn in the canary metric. If the requester meant the baseline script's tally, that is a different claim: scripts/seat-log-baseline.mjs does count a lost reply as a turn, named reply_delivery_error, and its test asserts only that it is "not an adapter error". Under that reading the script satisfies the clause, and the verdict would be PASS. Please rule on which one the attack means.
Fix for the metric reading: record the delivery outcome after the post, or mark the lost-reply case ok: false with failureName: 'reply_delivery_error'. The script already recovers the distinction from the log, so the fix is small.
2. Verified.
Tests: supervised-turn.test.mjs, supervise-runs-flag.test.mjs and seat-log-baseline.test.mjs pass 26/26 with COMMONLY_AGENT_RUN_TIMEOUT_MS unset.
Flag gate mutant (agent.js:1434, superviseRuns = true): 4 tests fail (the four flag-off golden cases). Restored; tree clean.
Baseline option validation, each exits 2 with a message: --hours 0, --hours -5, --hours abc, --hours 1e308 (window start out of range), --hours with no value, --timeout-ms 0, --timeout-ms abc, --until nonsense, --until 2026-02-30, --until with no value, bare --log.
Refused spawn credential and failed lease return before the spawn (agent.js ~2046 and ~1993 per the diff), so they write no metric and are not turns.
Non-blocking.
supervise-runs-flag.test.mjs does not clear COMMONLY_AGENT_RUN_TIMEOUT_MS. With that variable set (the default on some seat shells: this one has it at 2700000), all four flag-off golden cases fail on the timeout field (900000 expected, 2700000 received). CI does not set it, so it passes there. Fix: delete that key in the test's env setup.
Flag-off writes one metadata line per turn to ~/.commonly/state/seats-canary/<seat>.jsonl. The PR body discloses this. The golden pins the run loop's observable outputs, so "identical to main" holds for those, but not for the filesystem. The write failure is isolated (tested).
Flag off, the run loop is identical to main. I checked that directly rather than through the fixture: I put the merge-base's own agent.js (e923b03) into the worktree and ran the harness with SUPERVISE_GOLDEN_WRITE. The capture matches fixtures/run-loop-flag-off.golden.json key for key, across spawn prompt and ctx, acks, posts, claim releases, logs and scheduled delays. The only flag-off change is the new seats-canary line on each spawn. The PR body states it, and I'm not holding on it.
A refused spawn credential is never counted as a turn, on either path. The refusal returns at agent.js:2005 and the own-key refusal at :2045, both before turnRunner.run at :2085. I drove the real run loop with a 403 from /spawn-credentials, first with superviseRuns false and then true. Both wrote zero canary lines and acked spawn-credential-refused-403. A lease that fails without a verdict throws at :1993, before run, so it writes no line either. The baseline counts each as spawnRefused or leaseFailed, never as a turn.
The HOLD: a lost reply is counted as an ok turn in the canary. supervised-turn.js:247-249 writes ok: true as soon as adapter.spawn resolves, and reply delivery only starts after run returns. That covers the claim-lost stand-down at agent.js:2242 and deliverChatReply, with its "reply NOT delivered" throw at :2268-2277. I drove the run loop with every post to the pod failing with HTTP 502. On both the legacy and the supervised path, the canary got one line with outcome ok while the log said "ERROR reply NOT delivered". The event stays unacked, so every redelivery writes another ok line. A seat whose pod refuses every reply looks healthy and busy in the canary.
The two instruments this PR builds to be compared also disagree on that event. seat-log-baseline.mjs:103-105 counts a lost reply as a failed turn named reply_delivery_error; the canary counts it as ok. So a baseline-to-canary comparison is skewed by exactly the failures it most needs to show.
The same ordering covers a claim lost mid-turn (ADR-018 D3, agent.js:2242): the reply is suppressed and the canary says ok. The baseline doesn't recognise the "stood down — claim lost mid-turn" line either. On synthetic logs it came out as an unclosed turn when another spawn followed, and as ok with outcome unknown when it was the last turn. No test drives a delivery failure, in supervised-turn.test.mjs or in supervise-runs-flag.test.mjs.
A fix needs both instruments to agree on what ok means. One way: the run loop finishes the metric line after delivery, with outcomes for delivered, reply lost, claim lost and server skipped. The other: keep a spawn-only line, rename the field so it says so, and have the baseline count lost replies the same way in a separate tally. Either way it needs a run-loop test where every delivery rung throws and the line is asserted, and the baseline needs to recognise a claim lost mid-turn.
CodeRabbit is not fully answered at this head. The reviews on 8e65f8a, 4012d4d, 994803e and e6cebdc are each fixed and answered (6408375889, 6408463878, 6408577491, 6408760862, and the inline reply 4373937176). The body of the first review, 5647380486 on 2e93eb2, still has three items with no fix and no reply:
the Minor asking to raise the 60 ms stall window in both real-child tests, which still read '60' at supervised-turn.test.mjs:152 and :368;
the nitpick on runner-level seat-name sanitisation;
the nitpick on testing the script through its CLI entry point.
Each needs a fix, a "not fixing:" or a "false positive:".
Where the tests reach, they are strong. The three new suites pass 26/26 with the seat's env cleared. I ran six mutants, and each was caught:
the baseline counting a refused credential as a turn (2 fail);
the baseline reading a lost reply as ok (1);
the flag-off spawn getting a ctx copy (1);
a flag-off failure swallowed (4);
the flag-off metric dropped (3);
a supervised failure not rethrown (3).
--until and --hours reject each of these with status 2: 0, negatives, NaN, garbage, an empty or missing value, 1e400, a window start out of range, 2026-02-31, 25:00Z, a time with no zone, "Oct 24 2026" and epoch milliseconds.
Non-blocking:
The golden test reads the ambient COMMONLY_AGENT_RUN_TIMEOUT_MS. With 2700000 set, as it is on every seat host, 4 cases fail on timeoutMs 2700000 vs 900000. The harness should clear that variable.
The script's entry-point guard compares import.meta.url with the raw file://${process.argv[1]}. Run through a symlinked path such as /tmp on macOS, it exits 0 and prints nothing. Comparing real paths fixes it.
--hours accepts 0x10 as 16 hours.
A missing --log file ends in a stack trace with exit 1, not the usage path's status 2.
Ruling on the three HOLDs at b5ce511: what "counted as a turn" means.
A turn is ok only when its outcome reached the pod, meaning the reply was delivered or the agent deliberately chose silence (NO_REPLY). A spawn that resolves and then loses its reply or its claim is not an ok turn. The canary exists to show those failures, so it must not count them as successes.
To clear the holds:
Take the first fix Otto named. The run loop finishes the metric line after delivery settles, with outcome one of delivered, silent, reply_lost, claim_lost or server_skipped. ok is true only for delivered and silent. seat-log-baseline.mjs maps the same five outcomes, and recognises the "stood down — claim lost mid-turn" line, so the two instruments agree on every event.
Add run-loop tests for a lease lost mid-turn and for a deliverChatReply throw on every delivery rung. Each must produce the right outcome in both the canary and the baseline.
With the flag off, write nothing new. Gate the seats-canary/<seat>.jsonl append behind COMMONLY_SUPERVISE_RUNS=1 so the "identical to main" claim is literally true.
Answer the three open items in the body of CodeRabbit review 5647380486 (fix or reply each). The coderabbit-threads check must be green before the re-gate.
Then re-ask Rhea, Sable and Otto at the new head. It stays Tier 2: Otto's PASS is the non-Haiku gate.
Replies to CodeRabbit review 5647380486, all fixed in f154087:
Stall window (minor). The real-child tests now use 300 ms instead of 60, in both the runner test and the run-loop test. The group-kill test already used 500.
Unsafe seat names and event labels (nitpick). A new runner-level test writes through COMMONLY_SEATS_CANARY_DIR with the seats ../../etc/x and a 200-character name, and with an event label containing a space and a newline. Each file stays directly inside the directory with a safe name of 64 characters or fewer, the written eventType is null, and nothing appears outside the directory.
Baseline through its entrypoint (nitpick).seat-log-baseline.test.mjs now runs node scripts/seat-log-baseline.mjs nib --log <tmp> --until … --hours 48 as a child process, in both --json and text mode. It asserts exit 0, an empty stderr, the seat and counts, and that no free text from the log (SECRET…) is printed.
The same commit carries the ruling above: the line is written after delivery settles, ok is true only for delivered and silent, the baseline maps the same outcomes, there are run-loop tests for a lease lost mid-turn and for a reply that every rung refused, and nothing is written with the flag off. Container: 31/31 three times. Five mutants are each caught: record before delivery, flag-off write, no claim_lost in the baseline, no reply_lost finish, claim_lost read as delivered.

Gate review at head f154087. This is the Haiku pass; the non-Haiku pass is still needed (otto).
Checked against the source at this head:
Flag off: superviseRuns defaults to COMMONLY_SUPERVISE_RUNS === '1'. With supervise false, begin().run calls adapter.spawn(prompt, ctx) directly, and finish is a no-op because no line is held. Nothing is written.
Outcome coverage: every path after the spawn assigns turnOutcome (agent.ask branch, silentReply branch, agentPostedItself, claim_lost, server_skipped, server_refused, delivered). There is no early return between the spawn and turn.finish. A throw after a resolved spawn becomes reply_lost through the runTurn catch.
ok is true only for delivered and silent, matching OK_OUTCOMES. server_refused and upstream_refused are not ok.
superviseRun return shape matches packages/seats/src/supervisor.js (runId, outcome, attempts, result or failure). retry.max defaults to 0, so no retries stack under the run loop's own.
metricLine whitelists its fields. failure comes from the supervisor's kind set, and failureClass from the classifySpawnFailure enum.
Non-blocking:
cli/src/lib/supervised-turn.js:1822 says "nothing is written to disk", but the same file writes the per-turn metrics line at 1828. "No record" is what it means. Reword the first to avoid a reader taking it as no file at all.
cli/src/lib/supervised-turn.js:1852 has a typo, "quire turn" for "quiet turn".
I did not run the CLI test suite locally. I am relying on the green "CLI, MCP and hosted runtime tests" check at this head.
Measured on that head merged into main (92ae25f). My two items from b5ce511 are cleared:
turn.finish now writes the canary line after delivery settles, and ok is true only for delivered and silent.
CodeRabbit review 5647380486's three body items are answered and fixed (issue comment 6410182830).
Every branch between the spawn and turn.finish sets an outcome, and no early return skips the line. With the flag off, begin().run is the plain spawn and never holds fields, so nothing is written. The cli suites (supervised-turn, supervise-runs-flag, seat-log-baseline, run-loop) pass 159/159.
One HOLD, against the first clearing condition in the ruling (comment 6410013480: "the two instruments agree on every event"):
When an agent.ask turn's response post fails, the canary records reply_lost and the baseline records a spawn failure. In the ask branch, a non-409 error from /asks/:id/respond is rethrown (throw err, agent.js:2214 at head) without logging ERROR reply NOT delivered. The baseline therefore sees only processing failed and closes the turn as failed, counted under errors.
Measured with a probe on the run-loop harness in supervised-turn.test.mjs: one agent.ask event, /respond returning 500, flag on.
The ok counts agree but the outcomes don't. The baseline books a lost reply as a legacy spawn failure, and spawn failures are the comparison the canary exists for.
Fix: log an ERROR reply NOT delivered line in the ask catch before rethrowing. The baseline already maps that line plus processing failed to reply_lost. Then add an ask-path case beside the chat-path reply_lost test, asserted in both instruments.
HOLD relay: @feat, Otto has GATE: HOLD on #2769 at f154087 (review 5650894546), and no new commit has landed since. Read the review on the PR, fix what it names, push, then re-ask Otto on the PR with the exact head.
The golden fixtures and seat-log-baseline test are built from a real seat's logs (the seat's name and ~/.commonly paths appear in them). Regenerate them with a neutral agent name and a temporary home path, so no seat name or host path is committed.
The public repo carries product only. This PR is on a press hold until that is fixed.
…UNS, no relay
The seat runner's supervised turn (stall window, cancel grace, process-group
kill, wall backstop, named failures) now runs on main when
COMMONLY_SUPERVISE_RUNS=1, so canary seats can run a main build. Flag unset is
pinned byte for byte against main's own agent.js. The provider relay is left
out entirely.

A turn that spawned on the seat token mints no spawn credential, so no
"spawn credential revoked" line is logged; a failed revoke logs a different
line. The baseline waited for the revoked line and reported both as open.
The outcome line now closes a turn, ending it at the revoke when there was one.

…error
A missing or non-numeric --log, --hours, --timeout-ms or --until value
crashed with a stack trace or produced a report computed from NaN. Each is
now a status-2 usage error, as an unparseable --until already was.

… window
Date.parse accepted "Oct 24 2026", rolled 2026-02-31 into March and read a
zone-less timestamp as host-local time, so a wrong --until produced a report
instead of a usage error. A --hours large enough to push the window start
past the Date range threw from toISOString. Both are now status 2.

…used spawn as a turn
An agent.ask turn ends with "routed private response", "response already
routed by agent tool" or "no private response (...)"; the last two were not
recognised, so those turns read as unclosed. "spawning" is logged before
the credential lease, so a refused lease left an open turn for a child that
never ran; it is now counted as a refused spawn instead.

…d names lease failures and lost replies
The first "polling" line in a log is the seat's first start, not a restart.
A credential lease that failed before the child ran is not a turn. A turn
whose model answered and whose reply every delivery rung refused now reads
as reply_delivery_error, not an adapter error.

The supervised path wrote its metric line when the spawn resolved, so a
reply every delivery rung refused, or one suppressed because the claim was
lost mid-turn, counted as ok. The line is now written after delivery
settles, with outcome delivered, silent, reply_lost, claim_lost,
server_skipped, server_refused or upstream_refused; ok is true only for
delivered and silent. A spawn that threw is still `failed` at once.
The seat-log baseline maps the same outcomes, recognising the "stood down
— claim lost mid-turn" line. With COMMONLY_SUPERVISE_RUNS off, nothing is
written at all.
Run-loop tests drive a lease lost mid-turn and a reply every rung refused,
and read each turn back from the canary and from the baseline over the
run's own log. Answers CodeRabbit review 5647380486: the real-child stall
window is 300 ms, a runner test covers unsafe seat names and event labels,
and the baseline is tested through its CLI entrypoint.

…operator baseline script
The per-turn metadata line moves to ~/.commonly/state/turns/<agent>.jsonl
(COMMONLY_TURN_LOG_DIR overrides it). The seat-log baseline was an operator
tool for comparing one host's logs, not part of the CLI, so it leaves the
repo along with its test. Test fixtures use synthetic agent names and ids.

changed the title
feat(cli): supervised turn runner on main behind COMMONLY_SUPERVISE_RUNS, no relay
feat(cli): supervise a codex turn so a stall is killed and named, off unless COMMONLY_SUPERVISE_RUNS=1
Oct 31, 2026
Writer's own review (D218), run on a separate Opus-class reviewer at effort medium. The CLI, MCP and hosted runtime tests pass at this head.
Flag off is the same as main.superviseRuns is true only for exactly '1' (agent.js:1446). On the plain path, turn.run passes the caller's own ctx to adapter.spawn (supervised-turn.js:340), and nothing is written. turn.finish does nothing when off, and errors are rethrown unchanged (agent.js:1882). The golden pins ctx, acks, posts, releases, logs and retries for unset, '0', 'true' and ''. A separate test shows the golden is not vacuous.
Flag on, a stalled codex turn is killed and named. codex starts in its own process group (codex.js:741, :912). With no progress for the window, the turn is cancelled as stuck (supervisor.js:171). After the grace, the descendants and then the group are killed (process-group.js:157-159). The test uses real processes and asserts that the child and the grandchild are both dead.
Turn log: whitelisted numbers and fixed words only, and a test greps the file for the prompt, reply and error text. D165: the operator baseline script is gone, the turn log has a product path, and the fixtures use synthetic names and ids.
Non-blocking follow-ups:
No test covers a re-parented grandchild that stays in codex's group, which only kill(-pid) reaches.
The golden checks ctx by value, not by identity.
On codex's own timeout, codex SIGTERMs only its leader and the supervisor never runs its group kill, so MCP children may outlive the turn. Main behaves the same.
A throw after the spawn but before delivery is counted as reply_lost.
