TASK-481: preserve completed inbox work across turn timeout

Tier 2 (wake/delivery path): Sable + Rhea after CodeRabbit. No CLI/MCP version bump; release stamping follows #2610.

  • Preserve one coherent batch/model turn. Add commonly_checkpoint_event(eventId, deliveryId), a transport wrapper over the existing authenticated ack endpoint. A child explicitly checkpoints an item after handling it, including NO_REPLY. It releases message claims with their correct outcome first. No progress is inferred from unrelated tool calls or chat output.
  • Each waking item gets its current event ID/nonce and checkpoint instructions. No nonce means no checkpoint suggestion; old MCP hosts keep normal completion-only behavior.
  • Renew live batch deliveries every minute through a bounded authenticated endpoint. The CAS matches seat, instance, delivered status and nonce, changes only deliveredAt, and does not ack or increment attempts. Without this, ten-minute GC would invalidate a 45-minute turn's checkpoint authority. An independent turn-deadline timer stops renewal even when spawn never settles; queued callbacks cannot renew after expiry.
  • Normal batch completion remains compatible with already checkpointed terminal acks. Unfinished work retains ordinary requeue behavior.
  • Amend ADR-024 D3 per Lily's TASK-481 direction; document rollout and limits.

Proof

Test-first: a fake five-event CLI turn handles/checkpoints three through the real MCP handler and then times out. The next poll contains only events four and five. The test failed on main with no checkpoints and passes now.

Real Mongo: three of five event acks persist through actual garbageCollect; exactly two unfinished events become pending. A renewed delivery survives actual GC. Wrong seat, instance, stale/empty nonce and terminal status cannot renew. A never-resolving-spawn regression verifies renewal ends at the configured deadline without acking unfinished work. Removing the nonce predicate causes the stale-nonce test to fail; restoration passes.

CLI run-loop 104/104. Full MCP suite 98/98. Focused backend renewal/route tests 18/18. Backend build and CLI lint pass; touched TS lint has zero errors. No live seat interruption or rollout performed.

Rollout / limits

Deploy backend, publish CLI + MCP, then relaunch wren on those releases and verify the checkpoint tool is present. Publishing does not reach pinned launchers. Live partial-batch interruption/requeue proof remains required before TASK-481 closes.

A checkpoint is explicit agent cooperation, not proof that its judgment was correct. Crashing after an external side effect but before checkpoint still permits replay (at-least-once delivery). Renewal failure is logged and stale nonces fail closed. On wrapper death renewal stops and GC recovers unfinished work.

Goose, Hermes, OpenCode and Qwen now honour COMMONLY_AGENT_RUN_TIMEOUT_MS through the runner-provided ctx.timeoutMs.

Walkthrough

The change adds per-event inbox checkpoints and periodic delivery lease renewal. The backend validates and renews matching deliveries. The MCP tool submits checkpoints, and the CLI prompts for checkpoints and renews active deliveries during batch handling.

Changes: Inbox Delivery Checkpoints

Layer / File(s) Summary
Backend delivery renewal
backend/services/agentEventService.ts, backend/routes/agentsRuntime.ts, backend/__tests__/unit/services/*, backend/__tests__/unit/routes/agentsRuntime.dm-events.test.js
The service refreshes deliveredAt only for a matching delivered event, agent, instance, and nonce. The authenticated renewal route validates batches and returns IDs for successful renewals. Tests cover identity and status mismatches, garbage collection, and route validation.
Per-event checkpoint tool
commonly-mcp/src/tools.js, commonly-mcp/__tests__/tools.test.mjs, docs/adr/ADR-024-shared-awareness-and-the-agent-inbox.md
The MCP tool validates event and delivery IDs before submitting an acknowledgment. Tests cover invalid inputs and stale delivery responses. The ADR describes per-event checkpoint behavior.
CLI checkpoint and renewal flow
cli/src/commands/agent.js, cli/__tests__/run-loop.test.mjs, docs/cli/inbox-checkpoints.md
The CLI instructs the agent to checkpoint handled events and renews deliveries while a batch runs. The effective turn timeout is passed to adapter spawns and limits renewal. Tests and documentation cover renewal timing, checkpoint retention across a timeout, and rollout behavior.

Split renewal requests at the endpoint’s 50-event limit.

If inboxBatchLimit is 50, a carried first_contact event plus a full new page can give processEventBatch 51 same-pod events. This code sends all 51 pairs, but POST /events/renew rejects batches over 50. Every renewal then fails during a long turn, so garbage collection can requeue live work. Send renewal pairs in batches of at most 50.

Split renewal requests into chunks of at most 50 deliveries.

A carried greeting joins the fresh page as pageEvents = [...carriedFresh, ...events]. A page of up to 50 events plus one or more carried greetings can produce more than 50 deliveries. processEventBatch sends all of them in one renewal request. The renewal route rejects any array over 50 with a 400 before it processes any entry. Renewal then fails for the whole batch on every tick, so no item in that turn is renewed. Garbage collection can then requeue unfinished work while the child is still running.

🐛 Suggested fix
       try {
-        await client.post('/api/agents/runtime/events/renew', { events: deliveries });
+        for (let start = 0; start < deliveries.length; start += 50) {
+          await client.post('/api/agents/runtime/events/renew', { events: deliveries.slice(start, start + 50) });
+        }
       } catch (error) {

A failed chunk skips the remaining chunks for that tick. The next tick retries all deliveries.

Do not instruct nonce-less items to checkpoint.

If an inbox item has no deliveryId, Line 2080 omits its per-item instruction, but this batch-wide sentence still tells the child to checkpoint it. The child cannot supply the required nonce. Include checkpoint guidance only for items with both IDs, and extend the nonce-less prompt test to reject the batch-wide instruction. As per path instructions, “A test that would still pass with the fix reverted is not a test (rule 1).”

Qualify the delivery-nonce claim.

A waking item can lack deliveryId; the CLI has an explicit nonce-less path and test. State that checkpoint IDs are supplied when a nonce is available. Otherwise this rollout guidance incorrectly describes older-host behavior. As per path instructions, “Check each factual claim about code, a route or a tool against the repository at this head.”

SECURITY gate for #2647 (fix: checkpoint handled inbox events before turn completion, TASK-481) at d7dac9c, requested by Flint (pod message 83894, co-addressed to Sable). Live head check: headRefOid = d7dac9c, OPEN, not draft, base main. Diff: 10 files, the new renew route and renewDelivery service method, the checkpoint MCP tool, the CLI batch wrapper, and the ADR and CLI docs. Read at that head: backend/routes/agentsRuntime.ts (the new /events/renew route beside /events/:id/ack, its middleware, and the ack handler's deliveryId checks), backend/services/agentEventService.ts (renewDelivery, and the GC requeue that mints a new nonce), cli/src/commands/agent.js (processEventBatch, renew timer, handleEventBatch), and cli/src/lib/adapters/claude.js (runClaude timeout path). Not run: the test suites, any live request, any live turn.

Verdict: HOLD. The nonce-bound renewal and the checkpoint auth are sound. The renewal has no upper bound, so a turn that never returns keeps its deliveries alive indefinitely. That turns a recoverable stall into a silent, permanent one.

What holds.

  • Auth parity. /events/renew uses phase4RateLimit and agentRuntimeAuth, the same chain as the runtime ack route at :1342. Identity comes from the token (req.agentInstallation), not the body. A body that names another seat is ignored.
  • Nonce binding. renewDelivery matches on _id, agentName, instanceId, status 'delivered' and deliveryNonce. A wrong seat, a wrong instance, a stale nonce, an empty nonce, or a pending, acked or failed row all return false. It sets only deliveredAt. It does not touch status, attempts or the nonce, so it cannot ack or spend an attempt.
  • Checkpoint. commonly_checkpoint_event posts to the existing /events/:id/ack route with deliveryId. That route refuses a missing nonce where one is required, and returns 409 stale_delivery when the nonce no longer matches. A checkpoint can only act on an event the caller already holds with its current nonce.
  • Input bounds. The renew route caps the batch at 50 pairs, checks the eventId is a 24-hex id, and requires a non-empty deliveryId.

Defect (blocks merge).
The renewal timer has no end point of its own. processEventBatch sets setIntervalImpl(renew, 60_000) and clears it only in the finally after handleEventBatch returns (agent.js ~2456). Nothing else bounds handleEventBatch: there is no Promise.race or deadline in agent.js. The only bound is the adapter's spawn timeout. The Claude adapter's own comment says so (claude.js:41): "we SIGTERM at timeoutMs and wait for close. If claude ignores SIGTERM the promise never resolves — the run loop stalls silently." SIGKILL escalation is deferred.

Failure scenario. A turn hangs inside a tool call (the TASK-481 trigger). The child ignores SIGTERM. The batch promise never settles. The renewal fires every 60s, so every delivered event in the batch keeps a current nonce and a fresh deliveredAt. The stale-delivery GC never requeues them, and the 10-minute × 3-attempt cap never retires them. They stay 'delivered' indefinitely. Before this PR, the same stall aged out: the GC requeued the events with a new nonce, the cap retired them with a visible error after three attempts, and a replacement runner could pick them up. After this PR, the events are pinned to a process that will never finish, and nothing marks them failed. A human mention in that batch goes unanswered with no error anywhere.

Fix (required). Bound the renewal to the turn's own deadline. Either stop renewing once the adapter timeout has fired, or stop after a fixed cap (for example 2× the turn timeout), and clear the timer on that path. Add a test where spawn never resolves and assert that renewals stop and the events become eligible for GC. Adding SIGKILL escalation to runClaude would also close the root cause, but the renewal bound is needed regardless because other adapters can hang the same way.

Residual, not blocking.

  1. A checkpoint is the model's claim that the work is done. Nothing server-side checks that a tool-posted reply exists. A model that checkpoints and then forgets to post drops the item with no error. This is the same trust the NO_REPLY path already places in the model, so it is not a new boundary. Worth an audit query on checkpointed items with no matching agent message.
  2. The checkpoint instruction is inline in the prompt, and a pod message could try to influence it (prompt injection). The reach is limited: the model can only checkpoint the item IDs in its own batch, with their current nonces. The worst outcome is a self-inflicted skip of items in that batch, which is what a wrong NO_REPLY already does.
  3. The renew route, like the ack route, has no requireApiTokenScopes check. That matches the existing runtime ack route, so it is parity, not a regression.

Not covered: the test suites (Sable's code gate and CI), CodeRabbit, the migration or ADR text beyond the lines in the diff, and any live turn.

d7dac9c — code gate PASS. The renewal and checkpoint paths hold under the attacks below. Two non-blocking notes on the retry timing and the prompt-only guard.

Verified at d7dac9c (Tier 2 code gate; security is Rhea's)

  • cli/__tests__/run-loop.test.mjs 103/103, commonly-mcp 98/98, backend renewal service + route tests 18/18 under Node 22.
  • Renewal identity and nonce: the route takes seat and instance from agentInstallation/agentUser, ignoring the body. renewDelivery is a CAS on _id, agentName, instanceId, status: 'delivered' and deliveryNonce. It does not touch attempts, status or deliveryNonce. Mutant M1 (drop the nonce from the CAS) turns the wrong-nonce case red (1 failed / 9 passed). Mutant M2 (take identity from the body) turns the identity case red (1 failed / 7 passed). Both restored; the worktree is clean.
  • Renewal is refused for pending, acked and failed rows, so it cannot revive a terminal checkpoint or a requeued row. A stale nonce after a GC requeue fails because the new claim writes a new nonce.
  • The timer is cleared in finally, and an in-flight renew is guarded against overlap. A failed renew is logged, never treated as an ack.
  • The checkpoint is the existing ack endpoint with the nonce, so acknowledge() enforces the same agent, instance and nonce match. The MCP tool does not accept an event the seat doesn't own.

Non-blocking

  1. Real retry timing. After a timeout, unfinished items stay delivered until garbageCollect() requeues them (about 10 min after the last renewal). list() claims only pending, so the immediate retry does not see them. The run-loop regression returns them from a mock, so it does not exercise that timing. Not a livelock, but the "survive a five-event timeout" claim rests on the mock. Suggest one Mongo-backed check of the retry.
  2. Prompt-only guard against reply loss. The instruction not to checkpoint a reply held for final CLI output is enforced only by the model. If a child checkpoints item B and the turn times out before its final text is posted, that reply is lost and will not replay. Worth a live proof step, or a flush of held replies before the timeout.

CLI LANE REVIEW at d7dac9c. This is the cli/ lane check. It doesn't replace the two Tier 2 seat gates.

Read cli/src/commands/agent.js in context: the batch prompt framing at about 2076–2097, and the processEventBatch wrapper around handleEventBatch at about 2441–2458.

  • Renewal timer. setIntervalImpl and clearIntervalImpl are the injected defaults at 1207–1208 and are threaded through to the run loop, so the test can drive the timer. The interval is cleared in finally whatever handleEventBatch does, so a timed-out or crashed turn stops renewing and GC recovers its unfinished items. That's the property that keeps renewal from pinning an event forever.
  • Renewal failure. A failed renewal is logged and never treated as an ack. Stale nonces still fail closed server-side. That's right for an older backend without the route, too.
  • Checkpoint hint. It appears only when a deliveryId string is present, so an older backend gets unchanged completion-only behavior.
  • Completion path. The per-event acks at about 2592 and 2703 stay as they were. The PR states that completion tolerates already-checkpointed terminal acks, which the run-loop test covers.
  • Crossing pods. The pod-boundary guard stays in handleEventBatch, so the rename doesn't move it out from under the new wrapper.

Non-blocking, for the rollout note:
(1) Progress depends on the model calling commonly_checkpoint_event. TASK-481 closes only on the live wren partial-batch proof the PR already names.
(2) A cli/ change on main auto-publishes the CLI. wren is a hand-launched seat, so it needs an explicit relaunch on the published version before that proof means anything.

d7dac9c — code gate HOLD. This corrects my PASS (review 5638989045) at the same head. Rhea's security HOLD (5638986604) names the same defect, and I confirm it from source.

Defect: renewal has no end point

  • processEventBatch clears the renewal timer only in finally, after handleEventBatch settles. A spawn that never settles keeps the timer running forever.
  • The adapters can leave a spawn unsettled. cli/src/lib/adapters/claude.js:291-314 and codex.js:572-597 SIGTERM the child at timeoutMs and then wait for close. Their own header comments say that if the child ignores SIGTERM, "the promise never resolves."
  • In that case this PR renews the batch's deliveries every minute indefinitely. The GC never requeues or retires them, so the unfinished items are stuck, not replayed. Before this PR the GC requeued them about 10 minutes after the last renewal.
  • My PASS did not test a never-resolving spawn, so it doesn't cover this.

Needed for PASS

  1. Bound renewal by a deadline that doesn't depend on the spawn promise: stop renewing at timeoutMs plus a grace period, or tie it to an abort signal.
  2. Add a test with a spawn that never resolves, asserting that renewal stops and the deliveries become GC-eligible.

Rhea security HOLD 5638986604 addressed in 987c4fc. Delivery renewal now has a separate deadline timer that does not await adapter.spawn settlement. The configured turn budget (COMMONLY_AGENT_RUN_TIMEOUT_MS, default 15 minutes) is also passed as ctx.timeoutMs. At expiry the interval is cleared and a stopped guard prevents an already queued callback from renewing; expiry does not ack any item. Normal settlement clears both timers.

Test-first never-resolving-spawn regression failed before the fix, then passes with the child intentionally unresolved. It verifies one renewal before expiry, timer cancellation at the deadline, no further renewal from a queued callback, and zero event acks. Full run-loop suite 104/104; CLI lint and diff check pass. Backend/MCP code unchanged. Fresh CI/CodeRabbit required before both exact-head re-gates. No live rollout performed.

Live head check: headRefOid = 987c4fc, OPEN, not draft. Delta d7dac9c..987c4fc: cli/src/commands/agent.js (+25/-2) and cli/tests/run-loop.test.mjs (+35), plus one docs line. Read at that head: the full processEventBatch wrapper, the new deadline timer and stopRenewal, the turnTimeoutMs resolution, the new never-resolving-child test, and the adapter default (DEFAULT_TIMEOUT_MS in claude.js and codex.js is already 15 min with the same COMMONLY_AGENT_RUN_TIMEOUT_MS override, so passing timeoutMs changes nothing by default). Not run: the test suites or a live turn. CodeRabbit and CI are still running per Flint, and they do not change this verdict.

Verdict: PASS. The HOLD defect is closed. Renewal now has its own end point that does not depend on the child settling.

What the fix does.

  • The deadline timer is set when the batch starts, at effectiveTurnTimeoutMs (the same value the adapter receives as timeoutMs). When it fires, stopRenewal sets renewalStopped and clears the interval.
  • renew() checks renewalStopped first, so a queued interval tick after the deadline posts nothing.
  • The finally block calls stopRenewal and clears the deadline timer, so the normal path is unchanged.
  • The timer is not tied to the spawn promise. A child that ignores SIGTERM no longer keeps renewals alive.

Recovery after the fix. Renewals stop at the deadline. The stale-delivery GC then requeues the events 10 minutes after the last renewal, so recovery happens within about 25 minutes of a hang, not never. A zombie child that eventually returns cannot ack those events: its checkpoints and completion acks carry the old nonce, and the ack route answers 409 stale_delivery.

Checked.

  • Bound. Worst-case lease life is the turn timeout plus the 10-minute GC window. There is no path that renews past the deadline. An in-flight renew at the deadline may complete once more. That is one extra deliveredAt bump, not an unbounded lease.
  • Input. turnTimeoutMs and COMMONLY_AGENT_RUN_TIMEOUT_MS are operator-controlled. A non-finite or non-positive value falls back to 15 min. The value is capped at 2^31-1 ms, so setTimeout cannot fire early.
  • Identity and nonce. Unchanged from d7dac9c: the renewal and ack routes still take identity from the token and check the nonce.
  • Test. The new test drives a never-resolving spawn, asserts the spawn gets timeoutMs 1000 and the deadline is set to 1000, fires the deadline, and then fires an already-queued renewal. It asserts no further renew post and no ack. That covers the case from my HOLD.

Residual, not blocking.

  1. The batch promise itself still never settles, so the loop does not pick up new work until the process is restarted. The events are recoverable via GC, but this runner stays stalled. Worth a follow-up to abandon a batch past the deadline and move on.
  2. The checkpoint-for-held-replies rule is still prompt-only, as Sable noted at d7dac9c. A timeout after a checkpoint can lose a held reply. Not a security boundary; it is the same trust the NO_REPLY path places in the model.

Not covered: CI, CodeRabbit, the code review (Sable's, which is already HOLD-corrected at 5638998874), and any live turn.

987c4fc — code gate PASS. The independent turn deadline stops renewal even when the spawn never settles. This clears the HOLD on d7dac9c (5638998874). Two non-blocking notes.

Verified at 987c4fc

  • cli/__tests__/run-loop.test.mjs 104/104 under Node 22. The new never-resolving-child test passes.
  • Mutant M3 (deadline callback no longer calls stopRenewal()) turns the new test red (1 failed / 103 passed). Restored; the worktree is removed.
  • The deadline uses the same effectiveTurnTimeoutMs that is passed to adapter.spawn, so renewal stops at the adapter's own timeout. The default is 15 minutes, the same default the adapters use, so default behaviour is unchanged. The override is COMMONLY_AGENT_RUN_TIMEOUT_MS, read by both.
  • The deadline is started before handleEventBatch, so it fires slightly before the adapter's SIGTERM. Renewal ends early, never late. The timers are cleared in finally, and the 60-second renew is blocked once renewalStopped is set.

Non-blocking

  1. Runner stall. A child that ignores SIGTERM still leaves processEventBatch unsettled, so the tick loop does not advance until restart. That is pre-existing, and this PR narrows its lease harm. Rhea recorded it as residual. A follow-up could abandon the batch at the deadline and let the loop continue.
  2. Turn cap wording. TASK-481 describes a "45m cap"; the adapters and docs/cli/inbox-checkpoints.md default to 15 minutes. Whichever value production sets through COMMONLY_AGENT_RUN_TIMEOUT_MS is the cap that matters. Worth confirming it matches the one the ticket assumes.

Addressed CodeRabbit’s renewal-size note in 96bd72c: renewals are split into requests of at most 50 pairs, with the deadline guard checked before each chunk. A 51-event regression failed before the change and passes after; all 105 run-loop tests and CLI lint pass. This covers a carried greeting plus a full page without shrinking the model inbox.

Stop claim renewal before checkpointing.

commonly_release_claim deletes the server-side claim, but it does not stop the CLI claimKeeper renewal timer. A renewal can run between claim release and commonly_checkpoint_event, reacquire the claim, and allow the later wrapper cleanup to release it without the item’s completed or declined outcome.

Before sending the checkpoint, stop the active wrapper renewal and apply the claim outcome.

Sable code gate: PASS at 96bd72c (Tier 2, CLI run loop, 51-event renewal fix only: 987c4fc → 96bd72c).

  • performRun now renews in chunks of 50, checking !renewalStopped between chunks. The /events/renew route caps a request at 50, so every chunk is within the limit.
  • run-loop.test.mjs has test.each([1, 51]). It asserts renewal count = ceil(n/50), each body ≤ 50 events, and all events renewed in order.
  • Ran cli/__tests__/run-loop.test.mjs at 96bd72c: 105/105 pass.
  • Mutation check was inconclusive. Replacing the chunk step with offset += 100000 hung the jest run rather than failing it, so I have not confirmed the 51-event test goes red on regression. I killed the run and restored the file; the worktree is clean.
  • The stop/deadline logic from 987c4fc is unchanged in this delta, and my 987c4fc gate stands for it.
  • Not covered: CodeRabbit's latest review is on 987c4fc (16:47 UTC); none is anchored on 96bd72c. Backend, frontend and service-tier CI were still running when I read them.

CLI LANE REVIEW at 96bd72c, covering the delta from d7dac9c (agent.js and inbox-checkpoints.md).

  • Deadline timer. An independent timer stops renewal at the turn budget even if the adapter promise never settles. That fixes the ignored-SIGTERM case where a batch could renew forever. stopRenewal sets a flag before clearing the interval, so a renew already in flight can't post another chunk after expiry: the loop checks !renewalStopped for each 50-event chunk.
  • Budget. It's COMMONLY_AGENT_RUN_TIMEOUT_MS, falling back to 15 min, which is the same env and default the claude, codex and pi adapters already use. So those seats behave the same. One real change: goose, hermes, opencode and qwen hardcode 15 min and never read the env. They now receive ctx.timeoutMs from it, so on a host that sets the env (wren's 45 m), those adapters run that long too. I think that's the intended uniformity. It's worth one line in the PR body so nobody is surprised.
  • Chunking. Chunks of 50 match the server-side bound. A failure partway through logs once and is never treated as an ack.

SECURITY re-gate for #2647 (fix: checkpoint handled inbox events before turn completion, TASK-481) at 96bd72c. Re-gates my PASS at 987c4fc (review 5639004311). Live head check: headRefOid = 96bd72c, OPEN, not draft. Delta 987c4fc..96bd72c: cli/src/commands/agent.js (+6/-1) and cli/tests/run-loop.test.mjs (+17/-7). Read at that head: the renew closure in processEventBatch, and the test diff. Not run: the test suites or any live request.

Verdict: PASS. The delta chunks the renewal into requests of at most 50 pairs, the server's cap, and checks the deadline between chunks. It adds no new authority and no new input path.

Checked.

  • Server bound. Each request carries at most 50 pairs, matching the route's 1–50 check at agentsRuntime.ts:1322. Before this change a batch over 50 would have been refused with 400, so the chunking fixes a refusal and does not widen anything.
  • Deadline. The for-loop tests !renewalStopped on each pass. Once the independent deadline fires, no further chunk is posted. An in-flight chunk completes once, as before.
  • Identity and nonce. Unchanged. Every chunk carries the batch's own eventId and deliveryId pairs, and the route still derives identity from the token.
  • Failure. A failed chunk throws into the existing catch, which logs and stops the rest of this tick. The events age into the GC requeue, which is the conservative outcome. An older server without the route behaves as before.
  • Rate. A batch of N events sends ceil(N/50) requests per 60s tick, well inside phase4RateLimit for any batch size the inbox produces.
  • Tests. The renewal test now covers 1 and 51 events, asserts the chunk count, asserts each chunk is at most 50, and asserts the flattened pairs equal the batch exactly. The ack-count assertion moved from 1 to count, which matches the per-event batch completion ack. Nothing was loosened.

Not covered: the test suites (Sable's gate and CI), CodeRabbit, and any live turn.
