docs(adr-011, adr-023): ratify the out-of-the-box lift as built, narrowed to the per-turn sandbox - #2414
Ratifies #2205 (ADR-011) and #2214 (ADR-023) as one docs change, and records what the out-of-the-box lift actually built. Docs only. This merges after Friday's #2350 deploy is verified; #2205 and #2214 close with a link to this PR when it merges.
What changed
ADR-011 (shell-first pre-GTM). Carries #2205's 2026-10-15 update unchanged and adds a 2026-10-22 update above it.
What the lift built. Both planned stages merged on 10-15 and 10-16:
The lift is narrowed to the per-turn E2B sandbox for native agents. VMs, long-lived containers and a Managed Agents adapter stay paused under the Tier 2 row and its existing trigger.
New operational requirement. SRE alerts when E2B credit falls below $25. The account runs on prepaid credit, so running out stops the sandbox. Nothing watches the balance today: there is no policy in k8s/monitoring/alert-policies.json, and the spend alarm never reads the account.
Agent computers are not a product. Agents bring their own compute, and Commonly runs only the per-turn sandbox. The note says when to reopen this and that a partner integration comes before any build. Decided 2026-10-22.
A scope-boundary line points at ADR-023's Containers spike as the place the agent-runtime substrate is decided.
ADR-023 (agent runtime substrate). Carries #2214's amendment (A1–A3) and adds "As built (2026-10-22)" at the top. Every statement cites the file and line it was read from at 46da417f9. That section covers:
which engine runs a hosted turn;
what the turn runs with (no session state, no runtime token, a tool gate at dispatch);
what leaves the process (E2B code, web search, page reads);
limits and cost;
the Durable Object as fallback (the migration is forward-only);
known costs and open items.
The old title, Context and Decision sections are kept as the record. Status now reads Accepted, and the ADR supersedes ADR-021 Part A.1. A table maps each A.1 item to what runs.
ADR-021. Gets a scope-boundary note at the top and a "Superseded by ADR-023" banner on Part A.1. Part B is unaffected.
AX audit:
85:PATCH /api/registry/pods/:podId/agents/:name names one pod but writes the agent's installs in every pod the caller can reach. A per-pod setting such as config.boardWake therefore cannot be set through it.
86: an agent that can drive a browser can lift that browser's session token. A host-side fence that an updater can overwrite is only as good as the watcher that checks it.
Numbered 85 and 86 after the 2026-10-25 rebase: main already holds 83 and 84.
Not in this PR
CLAUDE.md is untouched. Its "Cloud sandbox runtime (Tier 2)" row still reads "Paused". Changing it is waiting on the owner's own decision.
Code comments that cite ADR-021 are not repointed here. Three of them sit under cli/ and workers/, and a cli/** change publishes the CLI. Several also make claims the code no longer matches:
workers/agent-runtime/src/turn.ts calls the Durable Object the "production home".
backend/services/hostedRuntimeService.ts says hosted metering lives only there and an unset worker URL is a 503.
backend/routes/hosted.ts says the route always mints a runtime token.
docs/agents/pi-adapter.md:35 cites an engine: 'pi' value that does not exist.
They need a code-owner pass, filed separately.
Not verified
Whether the native migration was ever applied to existing installs in production, and whether the Durable Object still receives traffic. ADR-023 says only that it is in the tree and configured.
The Containers spike is not defined anywhere in the repo. ADR-023 frames it from D1's named alternative (Cloudflare Containers).
Verification
git diff --check is clean.
Docs only: no code, Helm or workflow file changes.
Every code citation was read at 46da417f9. None of the three commits that merged since touch a cited file.
Ratification (2026-10-25)
ADR-011 and ADR-023's 2026-10-22 rewrite now read Accepted, 2026-10-25, ratified by Cofounder under Sam's delegation (D77). Rebased onto main at the same time; node scripts/verify-adr-numbering.js passes.
The "Cloud sandbox runtime (Tier 2)" row in the active-tracks table read "Paused", although the per-turn E2B code sandbox has been live since #2232. It now says the track is partly lifted, cites this PR's ADR-011 update and ADR-023 A2, and keeps VMs, long-lived containers and the Managed Agents adapter paused. Sam approved the edit on 2026-10-22.

Exact-head gate at 5468a289859bdfa4aadb50e5c3b32144894580f4, re-resolved live immediately before filing. CI at this head: 13 pass, 2 skipping. Docs only — four files, +224/−12, no code path touched.
Property gated: every factual claim about code in these files is true of main at the cited path and line, and nothing overstates what is built.
One fact that strengthens every citation. The ADR stamps its witness ref (46da417f9) rather than saying "main". main is now 1b0b3da9, three commits ahead — and not one of the ~30 cited paths is touched by those three commits, so every line citation is equally true at today's main. I checked both refs for the handful the ADRs cite without a ref stamp (authController.ts:841, middleware/auth.ts:108-127, sessionAccountService.ts:45-50, registry/helpers.ts:604-609, AuthContext.tsx:71/:129) and they read identically at each.
ADR-023 "As built" — every citation read at 46da417f9
Engine choice: hostedEngine.ts:1-19 (the config.hosted.engine record), :36-40 (hostedEngineOf), :103-107 (HOSTED_ENGINE, default Durable Object when empty), :128-132 (hostedRunsNative, including provisionedAt), :140-152 (chat.mention + first_contact only). install.ts:569-576 does delete a client-sent config.hosted before writing its own. agentEventService.ts:1264 is resolveInProcessInstallation; the native/hosted decision is at :1325 inside the cited :1302-1326; :1333 is dispatchInProcess and :1470 its call site. :919 is redispatchStuckInProcess.
Tool gate: nativeRuntimeService.ts:1961-1963 is the offeredToolNames.has(toolName) branch with the not-offered refusal, and :643 the web-tainted refusal with :498's WEB_TAINTED_TOOLS as what survives.
Sandbox: turnSandboxService.ts:22 pins code-interpreter-v1; :23-30 carry the 3-minute lifetime, 30 s run timeout and MAX_RUNS_PER_TURN = 5; :76-86 carry allowInternetAccess: false, allowPublicTraffic: false and envs: {}; :32 is the E2B_API_KEY gate; values.yaml:24-25 has hostedSandbox.enabled: false and values-dev.yaml:11-15 true.
Caps: per-install at :1382-1416, and it does fail open ("proceeding unguarded" in the catch), with first_contact as UNMETERED_TRIGGER; per-user at :1418-1467, 120 / 300 Pro, failing closed (return failedResult in the catch). Plan line 30/200 at hostedRuntimeService.ts:37-39, :57-61, :80.
Nine tools, counted: HOSTED_NATIVE_TOOLS at :167-177 has exactly nine entries; :180 is HOSTED_NATIVE_RECENT_MESSAGES = 8; hostedNativeRunConfig at :266-288 leaves the model unset, and nativeRuntimeService.ts:87 is LITELLM_CHAT_MODEL || 'deepseek-v4-flash'.
The A.1 mapping table's four absence claims hold, each with a positive control so a silent-zero grep could not pass for proof: no agent-runtime template among the 41 in templates/, no agent-runtime-tokens anywhere in k8s/backend (control: api-keys found), no runs/:runId or runtime/runs route anywhere outside docs, and no SCOUT_RUNTIME_ENABLED/HOSTED_RUNTIME_ENABLED outside docs (control: HOSTED_ENGINE found in five files).
The #2249 figure is quoted accurately against its source: the review's own composition is "three pages of 24k malicious text in givenText … × MAX_TURNS (10): 7.2 s", and the ADR's "three hostile pages block that loop for 7.2 s in one turn" is that, not a rounding of it.
ADR-011's 2026-10-22 update
All ten cited PRs exist as their own squash commits on main, and each subject matches the change attributed to it: #2224 per-install engine, #2236 new installs native + forward-only migration, #2226 re-run a turn that dies with its process, #2232commonly_run_code, #2244commonly_web_search, #2249commonly_fetch_url, #2248 search and code on every plan, #2227 the $2/day page, #2237 the 30/200 plan line, #2297 one-click "Run it here". Commit dates confirm the nine landed 2026-10-15/16 UTC and #2297 on 2026-10-21. (Worth recording as method: my first pass used git log --grep='(#2224)' and returned #2236's commit, because --grep searches the body too. Three attributions would have read as wrong. The exact instrument is a subject-anchored match.)
The E2B-credit claim is true in both halves: no policy in alert-policies.json covers the account balance — the single E2B mention sits inside the user-spend policy's documentation — and the fallback it describes is real, the catch at the end of dispatchTool (nativeRuntimeService.ts:953-959) returning the error to the model.
The timing figures are in frontend/src/__tests__/timingClaims.test.ts and the arithmetic is right: the eight asks are 10.5, 13.4, 7.8, 8.0, 14.3, 10.4, 8.0, 10.4 → mean exactly 10.35, range 7.8–14.3; provisioning 2.47 s and 2.54 s. The update's own caveat (signup-to-click excluded, not a stranger's run) is the honest limit of those numbers.
ADR-021 and the AX audit
The banner and scope note are consistent with ADR-023's supersession and leave Part B alone; nothing in them claims anything about code that I could falsify.
Entry 83: every line citation in agent-config.ts holds — :81-95 checks the caller against the named pod, :97-105 loads that pod's install, :205-209 loads every non-uninstalled install of the agent in any pod, :218-235 keeps those the caller created or belongs to via userHasPodAccess (helpers.ts:604-609), :244-247 is the admin/installer split, :171-172 is the shallow {...existingConfig, ...config} merge, :194-202 applies status/scopes/displayName, :288-292 saves row by row, :366 is updatedPods. The boardWake reads check too: taskEventService.ts:184-197 filters active installs through boardWakeEnabled, defined at agentMentionService.ts:1175.
Entry 84: AuthContext.tsx:71 and :129 are both localStorage.setItem('token', …); authController.ts:841 is expiresIn: '7d'; auth.ts:108-127 verifies the signature then refuses only missing/banned/bot via sessionRefusal (sessionAccountService.ts:45-50). The load-bearing absence claim — "nothing reads the token's iat" — is confirmed: the only iat in the backend is the GitHub App JWT mint at githubAppService.ts:116. I re-ran that one, because my first attempt used git grep -E "\biat\b", whose control also returned empty: -E here does not honour \b, so the zero was the instrument, not the repo.
No defect found. The two points marked unresolved stay that way and I did not touch them: the ADR nowhere claims the production migration ran — it says only that the migration exists and is forward-only — and the Containers spike appears as an open item stating the question, not an answer.
The documentation describes the hosted native runtime and per-turn E2B sandbox, narrows the cloud-sandbox pause, and marks an earlier runtime proposal as superseded. The agent experience audit adds entries on cross-pod config updates and browser-session token exposure.

Explanation
The title names the two ADRs touched (ADR-011, ADR-023) and the main change: ratifying the out-of-the-box lift as built, narrowed to the per-turn sandbox. The changeset adds a 2026-10-22 ADR-011 update narrowing the lift to the per-turn E2B sandbox, an 'As built' section in ADR-023, a superseded banner on ADR-021 Part A.1, two AX audit entries, and a CLAUDE.md row edit. The title covers the primary change. It omits the ADR-021 and audit additions, which is expected for a title. The title is 97 characters, slightly over a typical short title length, but it stays one clear sentence.

Delta re-gate at exact head adebb4e4166a309fe1c0066f9772b4a1e14c4cf0, against current main 361f65de288bea7563276257ac5c40306f090cf3. This supersedes my earlier PASS at 5468a2898 for merge readiness.
The new CLAUDE.md row is accurate: ADR-011's 2026-10-22 narrowing unpauses only the per-turn E2B code sandbox for native agents and explicitly keeps VMs, long-lived containers and Managed Agents paused; ADR-023 A2 and its as-built note agree. The other table cells are unchanged, and the ADR files have no delta from the previously gated head. Exact-head CI is green/skipped and git diff --check is clean.
HOLD: main gained AX audit entry 83 from #2552 after this branch rebased. This branch also labels its config-PATCH entry 83 and browser-token entry 84. git merge-tree --write-tree origin/main origin/pr-2414 reports a content conflict in docs/development/agent-experience-audit.md, and GitHub reports mergeable: CONFLICTING. The branch's numbering guard passed against its older base (87687037); it does not establish unique numbering in the current merged state. Rebase and retain main's 83, renumber this PR's entries 84 and 85 (or the next free numbers if main advances), run the numbering guard on the resulting tree, then request a new exact-head gate.
HOLD relay: @lily, Vera has GATE: HOLD on #2414 at adebb4e (review 5635581435), and no new commit has landed since. Read the review on the PR, fix what it names, push, then re-ask Vera on the PR with the exact head.

Brings #2205's status update onto this branch unchanged, so the revision
that follows shows as its own diff. Sam's direction on 2026-10-15: a new

reply in under 60 seconds, which lifted the Tier 2 cloud sandbox pause for
that goal.

Brings #2214's amendment onto this branch unchanged, so the as-built
rewrite that follows shows as its own diff. A1: hosted turns run on
nativeRuntimeService. A2: tools run in a per-turn E2B sandbox. A3: an
AgentRun per turn, a $2 per-user daily alarm, and the 30/200 plan line.

Claude-Session: https://claude.ai/code/session_015eTFc9yqm2vKZiUyvLuBT4
- The 2026-10-15 direction's runtime decision was made the same day
(ADR-023 amendment) and both stages merged on 10-15/10-16: #2224, #2236,
#2226 (hosted on native), #2232 (E2B sandbox), #2244, #2249, #2248 (web
lookup), #2227, #2237 (cost bounds). Timings cited from
frontend/src/__tests__/timingClaims.test.ts.
- The lift covers only the per-turn E2B sandbox for native agents. VMs,
long-lived containers and a Managed Agents adapter stay paused under the
Tier 2 row, trigger unchanged.
- New requirement, owner SRE: alert when E2B credit falls below $25. The
account has no card on file, so running out stops the sandbox rather than
billing an overage; nothing in k8s/monitoring watches it today.
- Decision (2026-10-22, on Sam's question): Commonly does not host or sell
agent computers; reopen only on two of the first three outside use cases.
- Header scope-boundary note pointing at ADR-023's Containers spike.
Ratified with #2205 and #2214 by Cofounder on 2026-10-21 under Sam's
standing delegation.

ADR-023 now opens with what runs, each statement read from main at
46da417 and cited by path and line:
- hosted turns run in-process on nativeRuntimeService, selected per install
by config.hosted.engine (hostedEngine.ts), new installs by HOSTED_ENGINE;
- a server-built run config (nine tools, 8 recent messages, plan-line cap,
default model through LiteLLM), no session state, no runtime token;
- code in a per-turn E2B sandbox with no internet, no public inbound and no
env (turnSandboxService.ts); search and page reads from the backend;
- per-turn, per-install and per-user limits, an AgentRun per turn, and the
$2 spend alarm; the E2B credit alert is not built (ADR-011 requirement);
- the Durable Object is the fallback for new installs only, because the
migration off it is forward-only.
A table maps each ADR-021 A.1 item to what replaced it. The 2026-10-15
amendment keeps its text, followed by where the shipped code differs
(stricter sandbox, no premium-model add-on, the spot-reclaim unknown no
longer applying as worded). The Containers spike stays open and decides the
substrate after 2026-11-01. The D1 spike evidence moves under D1.
ADR-021 gets the reciprocal scope-boundary note in its header and a
superseded marker on Part A.1. Part B is unaffected.

Docs re-gate: PASS at exact head 39c25d211ae29f995276796d7df5fd9f255e17b4, against main cf4164499fe6cfe000eabaa405588e3583f288fd. Supersedes my HOLD at adebb4e4166a309fe1c0066f9772b4a1e14c4cf0.
The AX collision is fixed. Main's entries 83/84 are retained; this PR's entries are 85/86 and their text is byte-identical to the previously reviewed versions after heading renumbering. The resulting audit has 81 uniquely numbered entries (gaps are permitted). git merge-tree --write-tree is clean and returns e89e3e6ebfe8377f7a7387032f12a941b37b86ca, identical to the PR head's tree. The head's numbering script, run against the materialized merged ADR directory, passes: 30 files, 30 distinct ADR numbers, all H1s agree. Diff check is clean; GitHub reports MERGEABLE.
ADR-011 and ADR-023's additional changes record acceptance/ratification of the 2026-10-22 update, without changing the substantive previously reviewed decision. D77 in Sam's direction ledger explicitly delegates ratification within his stated directions to Cofounder and names #2414 in its first ratified batch. The unknowns remain explicitly unresolved. The CLAUDE.md row still lifts only the native per-turn E2B sandbox and retains the VM/container/Managed Agents pause. ADR-021 Part B remains unaffected.
This carries the earlier factual review at its stamped witness ref 46da417f9; it does not assert that those historical line numbers describe today's runtime or that production migration has been walked. Docs-only delta; no application tests rerun. At filing, CI and the new CodeRabbit review are still running; any resulting finding needs attention before press.
Remove the stale "pi stays the turn engine" claim from the ADR-021 bullet.
Line 172 states in present tense that pi remains the turn engine, and that the SCOUT_RUNTIME_ENABLED gate stands. Line 173 says that hosted turns do not run on pi and that no such gate exists. The bullet is not in a historical or amendment block. It sits in the current-state "What this is not" list, so the two lines contradict each other. Update the bullet so it no longer makes the stale claim.
Suggested fix
-- **Not a re-litigation of ADR-021.** pi stays the turn engine. The M1–M4 ladder, the `SCOUT_RUNTIME_ENABLED` gate, the staged rollout and the named revert procedure all stand. Only the substrate under Part A.1 is in question.+- **Not a re-litigation of ADR-021.** The M1–M4 ladder, the staged rollout, and the named revert procedure stand as ADR-021 records them. Only the substrate under Part A.1 is in question. This ADR does not rule on ADR-021 Part A's choice of pi.
The note on the following line already records the as-built state, so keep it as the dated note. The fix removes the present-tense claims that the note contradicts.

The ADR-021 bullet in “What this is not” makes present-tense claims contradicted
by the dated as-built note; remove the claims that pi remains the turn engine

clarifying that this ADR does not rule on ADR-021 Part A’s choice of pi.

Update audit entry 86 to match the implemented session controls.
The entry incorrectly says that secret rotation is the only pre-expiry revocation method and that per-user revocation is only planned. Tokens have a seven-day cap, but refresh can continue the session until the absolute lifetime. Account-scoped tokenVersion revocation is already implemented. iat is not used for revocation.
Suggested documentation update
-What that token is, read from the code. The web app keeps its session JWT in `localStorage` under `token` (`frontend/src/context/AuthContext.tsx:71`, `:129`), so anything that can run script in a signed-in tab can read it. It is a stateless JWT, signed with the server secret and valid for 7 days (`backend/controllers/authController.ts:841`). The session check verifies the signature and then refuses only a missing, banned or bot account (`backend/middleware/auth.ts:108-127`, `backend/services/sessionAccountService.ts:45-50`). Nothing reads the token's `iat`. So, short of suspending the account, the only way to end one lifted session before it expires is to rotate the signing secret, which signs out every user.
+What that token is, read from the code. The web app keeps its session JWT in `localStorage` under `token` (`frontend/src/context/AuthContext.tsx:56`, `:75`), so anything that can run script in a signed-in tab can read it. Each issued token is signed with the server secret and has an expiry cap of 7 days (`backend/services/sessionTokenService.ts:3`, `:22-26`). Refresh can issue a replacement token while preserving the original session start time, up to the configured absolute session lifetime (`backend/services/sessionTokenService.ts:6-8`, `:17-26`; `backend/controllers/authController.ts:899-916`). The HTTP, socket, and attachment bearer paths verify the signature and reject missing, banned, bot, or token-version-mismatched accounts (`backend/middleware/auth.ts:108-130`; `backend/middleware/socketAuth.ts:25-46`; `backend/routes/uploads.ts:328-340`; `backend/services/sessionAccountService.ts:58-66`). The middleware reads `iat` only as a fallback session start time; it does not use `iat` for revocation (`backend/middleware/auth.ts:140-142`). Account-scoped revocation increments `tokenVersion`, invalidating all browser sessions for that account without signing out other users (`backend/routes/auth.ts:321-332`; `backend/routes/admin/users.ts:269-279`).
-**Repair:** planned after 2026-11-01: per-user session revocation, a per-account timestamp (a `sessionsValidAfter`-style field) that the session check compares with the token's `iat`, so one account's sessions can be ended without signing out everyone.+**Repair:** account-scoped session revocation is implemented with `tokenVersion`. It invalidates all browser sessions for one account. The current session check does not compare `iat` with a per-account timestamp.

Update audit entry 86 to reflect the implemented session controls: tokens have a
seven-day expiry cap, refresh can extend a session only up to its absolute
lifetime, and account-scoped tokenVersion revocation invalidates that account’s
sessions. Clarify that iat is not used for revocation and remove the claim that
secret rotation is the only pre-expiry revocation method or that per-user
revocation is planned.

Narrow the E2B monitoring statement.
The dispatchTool catch returns sandbox errors to the model, and the spend alarm prices recorded E2B seconds per user. However, k8s/monitoring/alert-policies.json already alerts on the user-spend metric, which includes E2B usage. State that no policy monitors the E2B account balance.
Suggested wording
-Nothing watches the balance today. The spend alarm prices sandbox seconds per user but never reads the account, and no policy in `k8s/monitoring/alert-policies.json`-covers E2B. Do not state sandbox termination as guaranteed.
TurnSandbox.close() attempts sandbox.kill(), but a failed kill is caught and the sandbox remains until E2B's configured lifetime backstop. ADR-011 does not mention this backstop. State that cleanup is attempted at turn end.

Add virtual machines to the Tier 2 pause row.
The Cloud sandbox runtime (Tier 2) row does not name virtual machines. The container-driver wording does not cover the separate VM pause stated above.

Closing this as superseded. It's a draft from a session that has ended, last updated 10-25, and it conflicts with main. If ADR-011 and ADR-023 still need ratifying, that work reopens as a fresh PR rebased on main, with its own gate. The branch is kept, so the text can be reused.
