feat(db): version PostgreSQL boot migrations and extract read cursor repository - #2684
TASK-504, wave 0 (Tier 2): boot-time versioned SQL migrations plus one behavior-preserving repository extraction for the following migration lanes.
Run ordered config/migrations/NNNN_name.sql files under a session advisory lock on one checked-out PG connection. schema_migrations tracks filename, SHA-256 and time. SQL plus its completion row commit together; errors roll back, release the connection, and keep PG boot unready through the existing retry loop.
Freeze the existing boot SQL as 0001_legacy_schema.sql. Its executable SQL matches main. config/schema.sql becomes a compatibility symlink for older fixtures/scripts. Existing backfill migration_records and their details retain their separate meaning.
Package SQL assets in npm run build and use that build in the backend image. Tier-1 shared fixtures now run migrations.
Extract agentPodReadCursorRepository: plain watermark reads and monotonic advances. Keep Mongo schema, deterministic key, independent timestamp/revision maxima, optional revision and duplicate-upsert loser retry. Production freshness callers now use the repository. ESLint restricts direct access to this extracted model.
Document the runner, test commands, upgrade rules, rollback limits and the repository template in docs/database/schema-migrations.md.
Verification
Node 22, private backend npm ci --ignore-scripts --include=dev.
31/31 focused migration, initialization, Mongo repository, freshness unit and delayed-Mongo-commit tests.
17/17 repository-boundary and existing PG boot/readiness tests.
39/39 existing threading/schema/backfill compatibility checks, plus 4/4 real cross-store freshness and decision-delivery checks (the latter exercises the shared fixture runner). A local MongoMemoryServer startup hook exceeded 30 seconds once under load; its unchanged 5-case freshness suite passed alone. An initial cross-store replay used the shared Mongo without credentials and was refused; the isolated Mongo/PG replay passes 9/9.
5/5 real PostgreSQL 17.10 migration cases: fresh schema/repeat boot; populated pre-runner schema preserving messages/backfill details; failed SQL rollback/retry; failed completion-row rollback/retry; two boots observed waiting in pg_locks, then one execution.
Backend TypeScript build and changed production-file ESLint pass (one existing max-length warning in freshness).
Compiled runner boots a fresh isolated schema using the copied SQL assets.
Three mutations caught by the committed tests: remove advisory-lock acquisition; remove BEGIN; replace cursor $max with $set. The failed-ledger case matters: a single multi-statement PG query can roll back on its own, so failing only inside that query would not prove atomicity with a separate ledger INSERT.
The pg-mem tests stub advisory functions and do not claim to prove locks or rollback. Those assertions run against real PG, with an isolated schema per case.
Rollout and review boundaries
Mongo remains the sole authoritative cursor store in this extraction. This does not introduce a PG cursor table, dual writes or model cutover.
Applied SQL is immutable. History mismatch, removed files or files inserted before applied history refuse boot. There is no automatic down migration. An image rollback after a new migration requires a compatible image carrying that history; never delete ledger rows to bypass the check. New migrations must remain compatible with rolling old/new application versions.
SQL files run inside transactions; online concurrent index builds and long data backfills remain separate operator jobs. Other models' existing self-bootstrap DDL is unchanged.
CodeRabbit follow-up
91c0c54 changes two test files only. Both first-pass observations are addressed: the real lock test scopes its count to the two boot PIDs while a third unrelated session waits, and history tests cover a removed applied file with identical SQL under a later filename plus an older image. These 15 cases pass; replaying the global-count check and removing each history guard goes red. Production code is identical to the fully green-CI head f71ac76. CI passed at 91c0c54. The second follow-up, 58dd417, pins the outsider PID too and awaits its result after cleanup. Both negative probes fail (outsider never waits; outsider errors after unlocking). The corrected lock case passes locally; in the wider five-case replay two unchanged cases hit connection-setup timeouts under host load, while both rollback cases passed. ESLint: zero errors. Exact-head CI is now fully green, including Test & Coverage, all three backend shards, both real-database service shards, E2E, kind smoke, frontend, CLI/MCP, typecheck/lint and CodeQL. CodeRabbit and coderabbit-threads are green. Non-author gates are still bound to 91c0c54 and have been asked to restamp 58dd417.

Walkthrough
The backend now applies PostgreSQL schema changes through ordered migrations at boot and packages migration assets in its build output. Agent-send freshness code accesses MongoDB read cursors through repository functions. Tests, lint rules, and database documentation cover both changes.

Summary
Baseline and migration runner backend/config/migrations/0001_legacy_schema.sql, backend/config/run-pg-migrations.ts, backend/config/schema.sql
The legacy schema is now a versioned migration. The runner validates migration files and history, applies pending migrations in transactions, and records checksums. schema.sql links to the baseline migration.
Database initialization now runs migrations and reports the applied count. The build copies migration assets to its output. Initialization tests cover success, failure, and retry.

The exact waiter count makes this test fail when Tier-1 suites run in parallel.
The advisory lock (1129270605, 504) applies to the whole database. Schema isolation does not limit it. testUtils.setupPgDb() now calls runPgMigrations(pgPool) against the same database, with the same lock key.
The trigger is ./dev.sh test:integration (INTEGRATION_TEST=true npm --prefix backend test). That command runs Jest without --runInBand, so several workers run at once:
This test holds the lock.
Another worker's setupPgDb() also waits for the lock.
pg_locks then shows 3 waiters, not 2.
Those waiters stay blocked until this test unlocks, so the count never drops back to 2.
waitFor times out after 5 s and the test fails.
CI does not hit this because it uses --runInBand. A normal local Tier-1 run can.
Change the check to wait for at least two waiters. The serialization proof still holds. Without the lock, the second boot would fail on the duplicate CREATE TABLE once_only or the duplicate schema_migrations key. With the lock, it returns [].
🐛 Proposed fix
- return result.rows[0].n === 2;+ // The lock is database-wide; other Tier-1 workers may also wait on it.+ return result.rows[0].n >= 2;

Update the waiter-count condition in the `waitFor` callback to accept at least
two advisory-lock waiters instead of requiring exactly two, so parallel Tier-1
workers do not cause the test to time out.
Add tests for a removed applied file and for history from a newer image.
This test name says it covers missing files. It does not test a removed applied file while other files remain.
The second assertion deletes every file.
The runner then throws PostgreSQL migrations are missing at Line 15, before it connects.
That check never reaches the history comparison.
Two documented history checks have no test:
Applied file removed, later file present. History holds 0001_initial.sql. The directory holds only 0002_*.sql. The runner should refuse because the names differ.
Image rollback. History holds more rows than the image has files. For example, history has 0001 and 0002, and the directory has only 0001. Then migrations[index] is undefined, and migration?.name !== row.version is the only guard that stops boot.
docs/database/schema-migrations.md documents both refusals, at Lines 16-17 and 41-43. If someone removed either guard, the current test would still pass. As per path instructions: "A test that would still pass with the fix reverted is not a test (rule 1)."

The lock test now counts the two boot connections by their real PostgreSQL backend PIDs and deliberately queues a third unrelated waiter. This avoids shared-database counts and preserves an exact check on the subject sessions. Replaying the old global-count predicate fails; the PID-scoped control passes.
Added the removed-applied-file and older-image cases. The removed-file fixture keeps identical SQL under a later filename, so a checksum-only guard cannot hide the missing name check. Removing the name check or skipping missing history entries makes the corresponding new test red.
Control: 10/10 pg-mem cases and 5/5 real PostgreSQL 17.10 migration cases pass. The correction changes only these two test files; production code is unchanged from f71ac76, whose CI passed completely.

Verify the outsider waiter by PID.
If the outsider query fails while another test worker waits on the same lock, total >= 3 can still pass. Promise.allSettled then hides the outsider failure. Record outsider.processID and require that PID to have an ungranted advisory lock. The test will then fail if its own outsider never waits. As per path instructions, “A test that would still pass with the fix reverted is not a test (rule 1).”

Ran failure injection against a throwaway postgres:16 with the PR's runner (run-pg-migrations.ts at head) and its shipped 0001_legacy_schema.sql:
Completion-ledger write refused: the ledger INSERT is made to fail on the baseline (CHECK constraint on schema_migrations). Result: refused, and the baseline's DDL is fully rolled back (no pods/users/messages created), zero ledger rows. The DDL and the ledger row share one transaction.
Older image (history has 0002, the image ships only 0001): refused with "migration history mismatch: 0002_b.sql". Nothing applied after that point.
Missing history on a populated legacy DB (tables and rows exist, no schema_migrations): the baseline adopts it, returns ['0001_legacy_schema.sql'], and the existing rows survive.
Applied file edited after the fact (same name, different SQL), with a new 0002 pending: refused with "migration history mismatch: 0001_a.sql", and 0002 did not run.
Static checks:
Legacy objects: every CREATE TABLE / CREATE INDEX / ADD COLUMN name in origin/main's schema.sql is present in 0001. The only apparent gap is a comment word, not an object.
Packaging: Dockerfile now runs npm run build, which copies migrations and schema.sql into dist/config via scripts/copy-pg-assets.js. The runner's default directory is __dirname/migrations, which matches dist/config/migrations.
Unit and real-PG tests in the PR cover the same cases (history mismatch, older image, ledger rollback, competing boots).
Non-blocking notes:
Ambiguous COMMIT (connection drops after the server commits): the runner treats it as failure, so the next boot re-runs that migration. The baseline is idempotent, so this is harmless today. Future migrations that are not idempotent need a rule about this case.
Deploy risk for SRE: 0001 creates idx_messages_pod_freshness_revision as a plain CREATE INDEX on messages. On a large populated messages table that is a write-blocking build during the first boot on each instance. Worth a CONCURRENTLY-built index outside the baseline, or a note in the rollout.
I did not run the backend Jest suites. The 15/15 check count is Tern's.

Tier 2 code gate for #2684 (TASK-504) at 91c0c54: competing boots with an unrelated waiter, and cursor monotonicity.
Probed against real Postgres 18.6 with the head's run-pg-migrations.ts and the shipped 0001_legacy_schema.sql, in a node:22 container:
Three concurrent boots on an empty database: one applied 0001_legacy_schema.sql and the other two applied nothing. The baseline ran once.
An unrelated session queued on a separate advisory lock was not held up by the migration boots.
An edited applied checksum refuses the boot with "migration history mismatch".
A history row with no matching file in this image refuses the boot.
An empty migrations directory refuses with "migrations are missing".
No migration advisory lock was left behind after the boots.
Cursor monotonicity, not run live: advanceReadWatermark uses $max on readAt and readRevision, so a late, older write cannot roll the watermark back. The duplicate-key retry on the racing first upsert also updates with $max and no upsert. The unit suite covers both cases with a mocked model. I did not run it against a live Mongo here.
Residuals, not blocking:
The baseline's CREATE TRIGGER messages_freshness_revision runs after DROP TRIGGER IF EXISTS in one transaction. On a production first boot after the runner ships, that takes a short lock on messages. SRE should expect it in the boot window. Every other object in the baseline is an IF NOT EXISTS no-op there, because main's schema.sql already creates them.
The probe did not cover a history that is shorter than the image's file list. The unit test refuses an older image whose file list is shorter than applied history covers that path, and Rhea's gate covers the ledger-write failure.
CodeRabbit review 5642438999 (pgMigrations.test.js): valid. Fixed at 58dd417. The outsider now has its own recorded processID and must appear as an ungranted advisory-lock waiter; another worker cannot satisfy that assertion. After allSettled cleanup, awaiting the outsider propagates its query error.
Two negative probes fail: replacing the outsider lock with a plain SELECT times out the waiter assertion; throwing after its unlock fails with the injected outsider error. The restored lock case passes. In the full local five-case replay, both rollback cases also passed; two unchanged cases failed during CREATE SCHEMA connection setup under heavy host load. Changed-test ESLint has zero errors. Production code remains identical to f71ac76; CI passed completely at both previous published heads. New-head CI must confirm the full suite.
One correction to the nonblocking ambiguous-COMMIT note in review 5642634202: if PostgreSQL commits before the connection drops, it commits the SQL and completion row together. The next boot reads that row and skips the migration. If the transaction did not commit, neither persists and the next boot retries it. An uncertain client response alone does not cause committed SQL to run again.
Exact-head CI at 58dd417 is now fully green, including both real-database service shards, Test & Coverage, CodeRabbit and coderabbit-threads. The last commit only strengthens the waiter test; production remains identical to f71ac76. Current-head independent restamps remain requested.

Tier 2 code gate for #2684 (TASK-504) at 58dd417. This renews my 91c0c54 PASS. The delta since then is backend/__tests__/service/pgMigrations.test.js only (+8/-4), so the runner is unchanged.
Falsified on real Postgres 18.6, using the runner at this head and a probe that mirrors the new test:
An outsider queued on the same migration lock key, observed by its PID. Both boots wait (own=2) and the outsider waits (outsider=1). Once released, the outsider takes and releases the lock first, then one boot applies 0001 and the other applies nothing. The table has one row and the history has one entry.
Falsifier A: no outsider queued, and the test's predicate still requires outsider=1. It never holds within 4s, so the assertion fails rather than passing vacuously.
Falsifier B: a wrong PID for the outsider. The predicate never holds, so the PID observation is what gates the test.
The new await unrelated after cleanup makes a failed outsider fail the test instead of being swallowed by allSettled. Correct.
Residuals, not blocking:
Cursor monotonicity is still not run against a live Mongo. It is checked by code and the unit suite, as at 91c0c54.
The rollback and history falsifications are Rhea's gate.
The freshness-trigger lock on a first production boot stays with SRE, as before.
Delta since the 91c0c54 PASS (review 5642634202): one file, backend/tests/service/pgMigrations.test.js. The change pins the outsider lock waiter's PID and asserts that it is waiting, and it stops swallowing the outsider's own failure with allSettled. No production file changed, so the runner, the baseline and the migration history checks carry over unchanged.
I did not re-run the Jest service suite at this head; the earlier failure-injection run against throwaway postgres:16 used the production runner and is unaffected by a test-only edit.
