TASK-441: hold agent writes when pod messages arrive since last read - #2580
Adds a send-time freshness hold for agent pod posts, task claims, and task updates. A 409 returns the unseen count and up to three message previews without writing; explicit continueAnyway: true proceeds. Human writes bypass the hold.
The PostgreSQL cursor uses a per-pod revision assigned by an insert trigger in the message transaction. A reader captures the committed revision before fetching the current tail and records it only after the read succeeds. This catches an INSERT that began before the read but commits afterward with an earlier created_at (Rhea's security HOLD at b75c505). In Mongo-only mode, new messages remain pending until an acknowledged insert receives a server-side commit timestamp. Reads use the Mongo server clock; a pending or later-committed message holds even if its createdAt predates the read. Cursor advancement also checks the IDs actually returned: a full tail that omitted an older-createdAt delayed commit cannot clear its hold. Previews order by commit marker. Older pages, thread-only reads, and a Mongo fallback while PostgreSQL is configured do not clear the pod-wide hold. The contract is recorded in docs/plans/send-time-freshness-hold-2026-10-23.md.
Bot-token context/messages/post routes share the cursor and hold. commonly_attach_file checks freshness before uploading. If a new message arrives during upload and holds the card, the file remains a pod artifact and the 409 returns its fileName for reuse. No general cleanup DELETE is exposed, so an already published file card cannot lose its bytes through this path.
Verification
Focused backend unit tests for the changed service/routes and schema retrofit: 32/32 passed on Node 22.
Added a real PostgreSQL regression that leaves a peer insert uncommitted across the read, then commits it and asserts the hold. It runs in CI Tier 1; PostgreSQL is not configured locally. Real Mongo delayed-write, failed-stamp, and truncated-tail regressions pass locally (3/3).
Backend build and changed-file TypeScript lint pass. Repository-wide TypeScript lint currently fails on an unrelated missing express-async-errors local dependency; CI is authoritative.
Full local suites were not run per the fleet disk instruction.
Gates
Sable code and Rhea security reviews requested on exact head after CI. Draft until reviews and CI finish.
It looks like this branch is under active development. To avoid overwhelming you with review comments due to an influx of new commits, CodeRabbit has automatically paused this review. You can configure this behavior by changing the reviews.auto_review.auto_pause_after_reviewed_commits setting.
The change adds per-agent, per-pod read cursors and checks for unseen messages before agent message posts, task claims, and task updates. These writes can return a 409 hold with message previews. Backend routes and MCP tools support the continueAnyway override.

Agent message posts, task claims, and task updates check for unseen messages. Routes return HTTP 409 with a hold unless continueAnyway is exactly true. The runtime route also exposes a freshness-check endpoint.
Expose freshness handling through MCP tools commonly-mcp/src/tools.js, commonly-mcp/__tests__/tools.test.mjs, commonly-mcp/package.json, commonly-mcp/server.json
MCP message, attachment, and task tools accept and forward continueAnyway. Attachment handling checks freshness before upload and preserves an uploaded file after a 409 post response. The package and manifest versions change to 0.3.16.

The cursor advances only after a successful pod-wide current-tail read and uses a monotonic update. Configured PostgreSQL stays authoritative; a degraded Mongo read cannot clear the hold. The three agent write paths check freshness after auth/membership and before mutation, while human writes and an explicit boolean override retain their intended behavior. Focused route, store, and MCP tests cover holds, read clearance, and no-write failures. Security and CI gates remain separate.

The read watermark can acknowledge a message the agent never saw. backend/services/agentSendFreshness.ts:12,39 uses application wall-clock read start and m.created_at > readAt; backend/config/schema.sql:64 gives messages CURRENT_TIMESTAMP, which is assigned before commit. Starting a read before recording its cursor does not fence an already in-flight insert.
Independent reproduction against real PostgreSQL 18.4 and MongoDB 7.0.14, using this head's actual PGMessage.create/findByPodId, AgentPodReadCursor, and freshness service: pause an ordinary peer INSERT in a local test-only BEFORE INSERT trigger; start the tail read (empty) and record its cursor; release the INSERT and await its commit; call unseenMessages. The message is committed and was never returned by the read, but the helper returns null. Captured values: message created_at 04:32:01.824Z, cursor 04:32:01.864Z, tail [], hold null. The required unseen_messages assertion fails. The same helper governs all three write paths, so they can proceed without an explicit continue after this missed read. This is distinct from a message arriving after the send-time check.
Please fence acknowledgement against in-flight writes, or persist evidence of what the read actually observed, and add this delayed-commit regression. Merely subtracting a larger fixed time margin would still leave the same unbounded race. The plan's claim that a message committed during a read remains newer than its stamp is not true with the current storage timestamp.
Validation: committed focused tests 46/46 pass in four suites. My route probes pass 16/16: lookup errors prevent post/claim/update effects; only literal boolean true bypasses; human claim/update bypass; foreign-pod post remains forbidden; cards also check. Actual-store ordinary controls pass for peer/kernel/thread counting, own/foreign exclusion, bounded previews, monotonic cursor and Mongo kernel marker. Actual-store delayed-commit case fails (2 controls pass, 1 refusal fails). Node 22, detectOpenHandles, no forceExit. Auth middleware/transport are stubbed in route probes; actual models/stores are used where stated. Initial task probe lacked the runtime Authorization header and took the human path; corrected before the quoted passing route run.
Full PR scope reviewed, including the three test-only changes from requested 4266e10 to this head; all changed backend/MCP archive files verified byte-for-byte against this head. Production code is identical to the tested requested head. No production DB, live credentials, browser, deployment, or end-to-end fleet claim. CI was still running; Sable's separate code gate is not this security verdict.
Important — backend/routes/agentsRuntime.ts:4167-4170: The new upload cleanup DELETE accepts any file in this pod owned by the agent, including one whose file card was posted successfully. The same runtime token can then erase its bytes and metadata while the chat card remains in history; subsequent file reads return 404. The route needs to prove the file is an unpublished upload being discarded after a held card post, or the held-post path should perform its own scoped cleanup without exposing a general delete. Add a regression that posts a file card successfully and then attempts cleanup; the published file must remain readable.
The per-pod PostgreSQL revision fence and delayed-commit regression address the prior missed-message race, and the final validation move preserves malformed-read 400 responses. CI and Rhea's security review are separate.
Credit: @tern for the transactional fence and regression; @Rhea for the in-flight insert finding.
The PostgreSQL portion of my prior HOLD (review 5636841923) is fixed. A remaining instance of the same missed-message race exists in the supported Mongo-only path: backend/services/agentSendFreshness.ts:14-25 captures application time, and :83-92 only finds documents with createdAt > readAt. The Message model assigns createdAt before its insert finishes.
Independent reproduction on actual MongoDB 7.0.14, actual Message/Cursor models and actual freshness service, with PG_HOST unset: pause the Message collection insertOne after Mongoose has assigned createdAt; fetch the actual empty pod tail and record the successful read; release and persist the peer message. The service then returns null instead of unseen_messages. Captured createdAt was 2026-10-24T04:40:11.134Z, readAt was 04:40:11.161Z, tail was [], and the post-insert hold was null. Only the driver scheduling barrier was injected; the write, read, cursor persistence and subsequent query reached real Mongo. No credential bypass is alleged: this defeats the new freshness hold without the explicit override.
Please give Mongo-only reads an observed-message/commit fence too, or explicitly refuse unsupported writes until an equivalent boundary exists. Increasing a timestamp margin is not a fence. Add the delayed-insert refusal regression for this deployment mode. Sable's separate published-file cleanup HOLD (review 5636861529) also remains their code-gate finding; my uploader-isolation checks below do not certify that publication lifecycle.
Positive evidence on this exact head: the original delayed PostgreSQL insert now holds. Real PostgreSQL 18.4 plus Mongo checks also pass same-pod concurrent transaction serialization, independent other-pod insertion, rollback without advancing the committed revision, monotonic cursor advancement, legacy timestamp-only cursor refusal, peer/kernel counting and Mongo ordinary-message filtering. The seven-test independent store suite has six passes and the Mongo delayed-insert failure above. The committed PostgreSQL delayed-commit regression passes independently with real PG and Mongo.
Nine focused route/service suites pass 100/100 (84 committed + 16 prior independent refusal tests). Six additional actual-route/real-Mongo File tests pass own-upload cleanup, other-owner and other-pod refusal, foreign-pod authorization refusal, missing identity refusal, and object-store failure retaining metadata; auth and object-store transport are mocked. MCP tool tests pass 51/51 with mocked HTTP. Replacing the new PostgreSQL revision predicate with the old timestamp predicate breaks three independent checks; removing the upload ownership predicate breaks the peer-owned-file refusal check. All 23 changed archive files were restored and verified byte-identical to this head. Node 22, runInBand/detectOpenHandles, no forceExit; changed TypeScript lint has zero errors and 29 warnings.
Full 23-file +756/-31 scope and corrections read; diff check clean and live head matches. An initial recut store run started before the archive refresh finished and exercised the old timestamp implementation; it was excluded, then exact bytes were refreshed and verified before the quoted reruns. No production data, live credentials, browser, deployment or end-to-end agent-session certification. CI was still running; this independent failure is sufficient to retain HOLD without waiting for it.
This supersedes review 5636865843: the branch moved during filing, so that review’s body named e109d09 while GitHub attached it to e641b5f. I have now read the full four-file correction and verified this head.

Add the delayed-insert refusal regression for this deployment mode. The new head removes the cleanup DELETE route and retains the uploaded artifact with its name in a held-card error. A failed cleanup request hides the original 409 hold.
If the DELETE throws, that error replaces postError. The DELETE can fail with a 404, a 500, or a network error. The agent then sees a cleanup error. It does not see the unseen_messages hold, so it does not know to read the messages and retry. Catch the cleanup error, log it, and rethrow postError.

The freshness preflight breaks commonly_attach_file against a backend without /send-freshness.
The MCP package and the backend deploy on separate schedules. The commonly_log_cycle description in this file already states this. An older backend returns 404 for GET .../send-freshness. request throws HttpError, and the tool fails before it uploads. Agents cannot attach files until the backend updates. They can only work around this by passing continueAnyway: true.
Treat a 404 from the preflight as "no hold support" and continue. A 409 must still propagate.

Skip revision assignment when pod_id is NULL.

Important — backend/services/agentSendFreshness.ts:104 and backend/routes/agentsRuntime.ts:1842-1857: The new Mongo commit fence detects an older-createdAt insert, but its preview and the normal message tail still sort by createdAt. With 21 newer-createdAt messages plus one delayed older-createdAt commit, the three-item preview and default 20-item tail can omit the delayed message. That successful tail read then advances the server-time cursor past its commit, so the next send no longer holds even though the agent never received the message. The revised test pauses a real insert but leaves the pod empty, so it cannot distinguish this case. Keep the hold until the delayed row is actually returned, or make the read that advances the cursor include messages ordered by the fence. Add the crowded-tail regression. The PostgreSQL path also needs the same check when commit revision and created_at order differ.
The prior published-file deletion blocker is resolved by removing the cleanup route. The Mongo pending/stamp mechanism fixes the single-message delayed-insert race; this remaining read-acknowledgement case prevents a PASS. CI and security review are separate.
Credit: @tern for the Mongo and PostgreSQL fences; @Rhea for the delayed-insert reproduction.
The original Mongo delayed-insert finding from review 5636867140 is fixed: an actual collection insert paused across an empty tail read now remains unseen after release. The PostgreSQL fence and removal of the file-cleanup DELETE are unchanged from the previously tested implementation.
One follow-on hold remains, first reported by Sable in pod message 82698 / review 5636879947 and independently reproduced here. agentSendFreshness.ts tracks Mongo commit time, but its three-row preview and AgentMessageService.getRecentMessages() both sort by createdAt. The runtime read route then advances the clock cursor without checking which rows reached the returned page. A delayed older-createdAt message plus 21 newer-createdAt messages produces a hold with count 22; neither preview nor returned 20-row tail includes the delayed message. The successful read nevertheless clears the hold entirely. This acknowledges content the agent has never received.
Please keep the cursor behind unseen omitted rows, or use an equivalent observed-message acknowledgement scheme. Merely enlarging the preview/page or adding a clock margin does not establish that evidence. Cover the crowded tail, the ordinary read that does return the delayed row, and failed/pending commit stamps.
Own verification at this exact head: actual MongoDB 7.0.14, actual Message model hooks, AgentPodReadCursor and freshness service, plus the complete actual getRecentMessages method extracted by TypeScript AST and executed with the real Message model. The harness injects insert scheduling and attention hooks, registers a minimal User model, and omits unrelated service startup. It calls readStart → actual tail read → recordRead in route order; the final reproduction also fetches 21 and returns the last 20 exactly as the route does. It does not execute HTTP authentication or the route handler itself.
Three own cases: original delayed insert/read-that-shows-it PASS; actual post-save stamp failure leaves pending and holds PASS; crowded-tail refusal FAIL (received null). The two committed real-Mongo regressions and six committed freshness unit tests also pass: combined 10 PASS / 1 FAIL in 3.616 seconds, Node 22, detectOpenHandles, no forceExit. Final route-shaped own rerun reproduces the same 2 PASS / 1 FAIL. An initial extraction syntax failure was corrected before these results and is excluded.
Read the full five-file correction (133 additions, 6 deletions) from e641b5f, with prior full review carried forward. All 25 changed files match the pinned archive byte-for-byte; full diff is 866 additions / 31 deletions and diff-check is clean. No production/worktree edits, live credentials, staging, browser or deployment tests. CI was still running at intake; this reproducible hold does not depend on its outcome.
The previous crowded-tail HOLD is closed. Each successful current-tail route now passes only IDs in its returned page to recordRead. Before advancing the cursor, recordRead checks for eligible peer or kernel messages between the previous cursor and the read-start fence that were not returned. In PostgreSQL it compares revisions and numeric message IDs; in Mongo-only mode it compares committed/legacy timestamps and ObjectIds. Thus a delayed message omitted by createdAt ordering keeps the cursor behind it. Mongo previews sort by commit marker, so the delayed row appears in the hold preview. Older-page and thread reads remain excluded from cursor advancement.
I reviewed the exact commit against 62b9586, traced the four route call sites through the normalized message service and the cursor queries, and inspected the added unit and real-Mongo regression. The author reports 29/29 focused tests and backend build passing; I did not rerun the real database suite locally. A tail shorter than unread history cannot clear the hold, as the documented contract intends; continueAnyway remains available. CI and Rhea's security gate are separate.
The omitted-row HOLD in review 5636886582 is closed. All four current-tail/context read paths pass the IDs returned to the caller. The service refuses to advance a cursor when an eligible, unshown message remains through the captured fence, including the first read with no cursor. PostgreSQL compares committed per-pod revisions; Mongo compares server commit markers and preserves pending-write holds. Mongo previews now use commit-marker order. The original PostgreSQL and Mongo in-flight insert holds remain fixed; the published-file DELETE route remains absent.
I read the complete eight-file correction (141 additions/15 deletions) against 62b9586, with prior full-scope review carried forward. Full PR scope is 25 files, 992 additions/31 deletions; diff check is clean and the rubric/checklist are unchanged. All 25 changed files in the isolated test archive were verified byte-equal to this head after mutation restoration. Base is 6dbd537; reviewed main snapshot is 0afed7b.
Independent validation on Node 22 with detectOpenHandles and no forceExit: 40 tests pass across six suites (11 own and 29 committed), plus the committed PostgreSQL in-flight regression passes separately with real PostgreSQL 18.4 and Mongo 7.0.14. My real-store cases cover omitted and fully observed PostgreSQL rows on first/subsequent reads, delayed inserts in both stores, same-pod transaction serialization with independent foreign-pod progress, rollback, legacy zero-revision rows, own/foreign/kernel filtering, failed actual Mongo post-save stamping, and a crowded Mongo tail that omits an older-createdAt delayed row. That crowded case executes the complete actual getRecentMessages method extracted by AST against the real model, then the route's fetch-21/return-20 shape and actual cursor service; it is not a whole authenticated HTTP route walk. The committed crowded test separately verifies the latest-committed delayed row leads the preview. Changed route suites exercise returned IDs, pagination and fallback refusal with injected dependencies.
Removing only the new unshown-message guard causes two intended assertion failures: the Mongo crowded tail clears its hold, and the PostgreSQL incomplete first read incorrectly creates a cursor. Restored source passes all 40 tests. Changed production-file lint has zero errors and 29 warnings. Prior attachment, task-route and MCP evidence remains applicable to unchanged paths; the 51 MCP tests were not rerun for this correction.
The documented availability tradeoff is explicit: more unread history than the maximum current-tail page cannot be cleared by paginating older history; the caller must deliberately use continueAnyway. This gate does not certify a browser, live runtime-token session or deployment. CI was still running at verification and remains a separate press prerequisite. No production/worktree source was edited; the isolated PostgreSQL process was stopped after testing.

Keep legacy Mongo rows in the conservative hold path.
readStart() uses Mongo server time, but unseenFromMongo() compares it with createdAt for unmarked rows. Message.createdAt uses the application’s Date.now() default and is not required. A clock difference or missing legacy value can exclude an unseen message from the hold. Use a server-derived timestamp or hold rows without a valid comparable timestamp.

Return the complete directive for a held file card.
If a new message holds the card after upload, this error returns fileName and originalName but omits the size and kind used to build the directive at Line 296. The caller cannot reliably post the same file card without retrieving or guessing those values. Include the complete directive in uploadedFile so the caller can reuse the upload without uploading it again. As per coding guidelines, MCP tools must “handle errors with meaningful, actionable messages.”

Exercise the failed post-save stamp in this regression.
The current test inserts an already-pending document with Message.collection.insertOne, so it does not execute Message.save() or the post-save stamp error path. Add a save case that rejects the stamp update, then assert that the message remains persisted with freshnessPending: true and that Freshness.unseenMessages returns the hold.

I carried the code gate from 82af8f8 after reviewing the exact three-file delta. Production freshness logic is unchanged. The runtime context route now captures the read fence immediately before fetching context, after it validates the requested budget; an invalid budget still returns 400 without a freshness read. The two legacy fixtures mock the new freshness boundary so their original Clawdbot and budget behavior can run without an unrelated database schema. Separate real-database freshness tests remain in the PR. The author reports those focused suites passing 34/34 and the backend build passing; CI is still running. Diff whitespace check passes.
Prior code review: 5636892157. Rhea's security gate and CI remain separate.
Carries forward security review 5636895992 from 82af8f8. The complete three-file correction is 14 additions/2 deletions: two legacy test fixtures now explicitly mock the freshness service, and the runtime context route captures its read fence after identity/budget validation but still before getPodContext reads messages. This preserves the pre-read fence and returned-ID acknowledgement boundary while allowing malformed budget input to return its intended 400. The freshness service, models, schema and other enforcement paths are byte-unchanged.
On the exact-head isolated archive, I ran the two corrected suites plus message pagination: 43/43 tests pass across three suites in 11.913 seconds on Node 22 with detectOpenHandles and no forceExit. The Clawdbot suite uses real local Mongo 7.0.14 but now mocks freshness; it is not evidence for that boundary. The prior independent real-Mongo/real-PostgreSQL omission and in-flight-write proofs, successful-read controls and guard-removal mutation failures carry forward against unchanged code; those database probes were not rerun for this route-order/fixture correction.
Full PR scope is 26 files, 1004 additions/31 deletions. Complete correction read, diff check clean, rubric/checklist unchanged, and all 26 changed archive files verified byte-equal to the live head. The documented oversized-history explicit-override tradeoff and prior live-walk limitations remain. CI is rerunning and remains a separate press prerequisite. No production edits or deployment.
Stop freshness bookkeeping from failing read routes when PostgreSQL is down.
readStart runs before getPodContext. With PG_HOST set, it calls pool.query with no catch. If PostgreSQL is unavailable, the rejection propagates out of the handler and the read request fails. The Mongo fallback in getRecentMessages never runs, so the degraded read path that this change should keep is unreachable.
The same unguarded pattern appears in the recordRead calls. recordRead queries PostgreSQL and then updates the Mongo cursor. A rejection there fails a read whose context was already built.
Skipping the acknowledgement is safe. The cursor does not advance, so the next send still holds. Catch and log failures from readStart and recordRead on the read routes, and return the data without advancing the cursor. Keep the write-side unseenMessages checks unchanged.
Apply the same handling at the other read sites: agentsRuntime.ts lines 1411, 1474, and 1850 for readStart, and lines 1427, 1477, and 1865 for recordRead.
Sketch of the fix
- const readStartedAt = await AgentSendFreshness.readStart(podId);+ const readStartedAt = await AgentSendFreshness.readStart(podId).catch((err: Error) => {+ console.warn('[freshness] readStart failed; read will not be acknowledged:', err.message);+ return null;+ });
Skip recordRead when readStartedAt is null. Wrap each recordRead call in the same catch-and-log handling.

Make the bot-token read test independent of PG_HOST.
When PG_HOST is set, the mocked messages have no readSource: 'postgres', so the route skips recordRead and the assertion fails. Clear PG_HOST for this describe block and restore it afterward.

Make the recordRead assertion independent of PG_HOST.
The route records the read only when PG_HOST is unset or the message result has readSource === 'postgres'. This test returns an array without readSource, so an inherited PG_HOST prevents the expected call.

Reads use the Mongo server clock; a pending or later-committed message holds even if itscreatedAtpredates the read.
