feat(messages): reconcile Mongo fallback messages into Postgres (TASK-313 part 2) - #2624
Closes the Mongo-fallback half of TASK-313. Part 2 approved by @lily-shen with conditions (pod message 83439); this implements them.
What changes
Schema (additive).messages.legacy_mongo_id TEXT, nullable. No table rewrite. Mongo pgFallback explicitly marks new availability writes from both human and agent paths; a partial compound index supports the scoped scan. Only marked rows are reconciled; unmarked pre-PG history and unmarked pre-deploy fallback rows are left untouched. No inferred history backfill is run.
Unique index without blocking writes. The index is built by the reconciler with CREATE UNIQUE INDEX CONCURRENTLY IF NOT EXISTS on its own autocommit connection, before the first insert. It is not in schema.sql, because that script runs as one transaction and CONCURRENTLY cannot run inside one. An INVALID index left by a failed build is dropped and rebuilt.
Reconciler (services/pgMessageReconcileService.ts, scheduled once a minute from server.ts):
runs only after SELECT 1 against Postgres succeeds;
bounded at 200 rows per pass, unattempted rows first, then deferred retries ordered by retry time and age;
inserts with ON CONFLICT (legacy_mongo_id) DO NOTHING, then stamps the Mongo row with updateOne({_id, reconciledAt: {$exists: false}}, …).
Mongo-side safety under two replicas. The stamp is set-once, so a second writer matches nothing. The PG insert is ON CONFLICT. The test runs two passes concurrently and asserts one PG row.
Refusals do not stall the scan. SQLSTATE classes 22/23 (except recoverable FK 23503), and row-shape TypeErrors are permanently stamped. FK failures attempt the existing Mongo pod sync and retry once. Remaining recoverable failures receive a retry time; unattempted rows sort ahead of retry rows so even a retry-ready full batch cannot starve new messages. A conflict whose follow-up lookup finds no PG row throws without a Mongo stamp.
Preserved content. Card payload and kernel author marker survive the move. Reconciled IDs are resolved in the shared PG model/thread validation path, covering human and agent writers. All-digit ObjectIds resolve before the numeric PG-ID case. Successful deletes attempt Mongo-copy cleanup so the fallback does not resurrect deleted content.
Index maintenance. A dedicated autocommit client holds an advisory lock through invalid-index repair and concurrent index creation. Another replica defers its pass if it cannot acquire that lock.
Id stability. A moved row returns its Mongo id as id, so clients keep the id they already hold. Reaction enrichment keys by the PG id (pgId).
Guard removed. The "NEVER RECONCILED" comment in messageController.createMessage is gone. Replies still return 503 during an outage. That is deliberate: a Mongo row has no reply edge to carry, and the reconciler does not reconstruct one. Changing that is a separate decision, flagged below.
Routes that take a message id (resolution)
PGMessage.resolveId maps a 24-hex Mongo id to its PG row. A numeric id passes through. Anything else returns null.

Not covered here (open):
POST /api/agents/runtime/messages/:messageId/claim and DELETE …/claim (routes/agentsRuntime.ts). They still take the id as sent. The claim key on an agent wake for a reconciled message must match the id the agent was handed. Follow-up required before reconciliation runs on a pod with live agent claims.
Socket fan-out of messageReaction events still carries the client id as sent. The client matches it, but only if it holds the Mongo id, which it does for moved rows.
Real Postgres 16 / Mongo 7 service suite: 19/19. Covers 203 older poison rows followed by a valid row, retry-ready orphan batches, pod synchronization, missing conflict-row IDs, stable set-once stamps, overlapping copies, index maintenance locking, kernel/card preservation, reply/thread ID resolution and cross-pod refusal, digit-only ObjectIds, deletes and reactions.
Focused controller/model/thread suites: 9 suites, 115/115. Includes delete cleanup, PG-outage refusal without mutation, and Mongo-only deletion.
Mutants: removing the set-once stamp guard changes the timestamp and fails its regression; disabling permanent failure stamps fails the poison-row assertion. Restored combined tree passes all 132 tests.
Scoped production lint: 0 errors (22 existing warnings across changed files); diff check clean. Full backend typecheck is not green locally: shared dependencies lack playwright-core and pre-existing utility scripts have type errors.
CodeQL's existing high alert was js/missing-rate-limiting on DELETE /api/messages/:id. That route now has the existing message limiter before auth. Fresh CI/CodeQL and both seat gates remain pending on this head.
Decisions for review
Reconciled rows stay visible in the Mongo fallback. The fallback reads Mongo only while PG is down, and PG is not read then, so a reconciled message cannot appear twice. Filtering reconciled rows out of the fallback would make them vanish during an outage. I did not filter.
Pre-PG history. Only explicit pgFallback rows are eligible. Historical unmarked rows stay in Mongo; any provenance-based backfill requires a separate reviewed migration, not a time cutoff that guesses their origin.
Replies during an outage still 503. Carrying a reply edge through Mongo is a separate design.
Tier and review
Tier 2 (schema). @Sable and @Rhea at the head, per Lily's ruling.
This PR does not add a transaction around the ordinary message INSERT and pod update. The TASK-313 history records part 1 (#2543) already merged: post-insert side effects no longer trigger a second Mongo copy. That prior fix is retained; no new atomic-write work is claimed here. Rhea’s SECURITY HOLD 5638330507 is addressed with an explicit fallback marker on both writers and in the scan. An unmarked 2020 row is never moved; removing the marker predicate makes that regression fail. Historical unmarked fallback rows require separate provenance-based review before any backfill. Agent claim-ID compatibility remains an explicitly open rollout restriction above. The concurrent delete-before-copy race remains open, as does the pre-existing explicit-thread PG-down response path; no claim is made that these are closed. Mongo delete cleanup is best effort; PG retention still does not purge the availability copy. No migration, live reconciliation or deploy has been run by Kai.
CI baseline repair and forward-only ruling
The only delta after c8623d4 is removing the now-fixed DELETE route from the rate-limit exception baseline. CI shard 3/3 caught that stale exception; the guard now passes 16/16 locally. Production files are identical to c8623d4, which had Sable CODE PASS 5638346261 and Rhea SECURITY PASS 5638343241. Fresh exact-head re-stamps and CI are pending.
Lily ruling 83643: forward reconciliation does not wait for legacy rows. SRE owns the read-only unmatched Mongo count; if nonzero, Kai files a separate one-shot dry-run-first import reusing copyOne. The scheduled pass stays flag-only.
Delete cleanup follow-up
At ab93e78, cleanup matches the PG row’s legacy Mongo ID as well as its reconciledPgId stamp. This covers deletion by Mongo or numeric PG ID when insertion committed but stamping did not. Two actual controller/PG/Mongo regressions prove both copies stay gone on the next pass; omitting legacy-ID cleanup fails both. Controller unit suite34/34; real-service suite19/19; scoped lint0errors and diff clean. This closes discussion_r4367232340; the separate deletion-before-insert race remains named. Fresh CI and both gates required.
…-313 part 2)
While Postgres is down, non-reply messages land in Mongo. Once it is healthy,
a bounded pass copies each unmoved row into messages and stamps the Mongo row
with the PG id it became. The moved row keeps its Mongo id as the client id
(messages.legacy_mongo_id), so clients holding it keep working.
- schema.sql: additive nullable legacy_mongo_id. The UNIQUE index is built by
the reconciler with CREATE UNIQUE INDEX CONCURRENTLY, outside schema.sql,
because the schema script runs as one transaction and CONCURRENTLY cannot.
- Idempotent: INSERT ... ON CONFLICT (legacy_mongo_id) DO NOTHING; the Mongo
stamp is set-once (filtered on reconciledAt absent). Two replicas can run
the pass at once.
- Bounded per pass (default 200, oldest first). A row the database refuses
permanently (FK 23503, pod not in PG) is stamped reconcileFailedAt and left
out of the scan so it cannot block the rows behind it.
- Id resolution: PGMessage.resolveId maps a 24-hex Mongo id to its PG row.
Used by findById, update, updatePayload, delete, reaction add/remove,
thread follow/unfollow/collapsed, the created-at backdate route, and the
message delete route. A delete of a reconciled message during a PG outage
returns 503 rather than deleting only the Mongo copy.
- Reaction enrichment keys by the PG id (pgId), not the client id.
- Removes the "NEVER RECONCILED" comment. Replies still 503 during an outage,
because Mongo rows have no reply edge to carry.
Scheduled once a minute per process from server.ts (in-process overlap guard).

Walkthrough
The change adds reconciliation for marked MongoDB fallback messages, stores their legacy IDs in PostgreSQL, and resolves those IDs for message operations. It also updates message deletion, reaction, thread-state, and backdate handling to use PostgreSQL row IDs.

Summary
Legacy ID storage and resolution backend/config/schema.sql, backend/models/pg/Message.ts, backend/services/pgMessageId.ts, backend/services/threadRootResolver.ts
PostgreSQL messages can store a legacy Mongo ID. Message and thread-root operations resolve client IDs before using PostgreSQL row IDs.
Mongo fallback messages are marked and carry reconciliation state. The service copies eligible messages in bounded passes, records success or failure state, and starts when PostgreSQL mounts. Integration tests cover pass behavior and related message operations.
Message retrieval and deletion, reactions, thread-state authorization, and backdating use resolved PostgreSQL IDs. The delete route applies a rate limiter.

Delete the Mongo copy when you delete a reconciled message.
For a reconciled message, only the PostgreSQL row is deleted. The Mongo row stays, with reconciledAt set and the full content. The Mongo fallback read returns it. After a user deletes the message, it reappears during any PostgreSQL outage. The Mongo copy is also never removed by PostgreSQL retention.
After PGMessage.delete(pgId), remove the Mongo copy on a best-effort basis. Do not fail the response if that cleanup fails.

Add tests for the new deleteMessage branches.
This change updates only the happy path with a numeric ID. The following behaviors have no unit test:
Deleting by a reconciled Mongo ID: PGMessage.delete is called with the resolved pgId, and MongoMessage.findByIdAndDelete is not called.
resolveId throws and the Mongo row has reconciledAt: the response is 503 MESSAGE_REQUIRES_POSTGRES. Assert that MongoMessage.findByIdAndDelete was not called.
An unreconciled Mongo row is deleted from Mongo only.
The 503 test is the one that fails if the guard is removed.
As per path instructions: "tests should fail when the behavior under test is reverted (rules 1 and 1a), and refusal-path tests should also assert that protected state remains unchanged (rule 2)."

Check the 24-hex pattern before the all-digits pattern in resolveId.
A Mongo ObjectId is 24 hex characters. It can consist only of digits, with a probability of roughly 1 in 75,000 per ID. /^\d+$/ matches such an ID first, so resolveId returns it as a PostgreSQL row ID. The lookup in legacy_mongo_id never runs.
The next query compares the value to an integer key. It fails with "value out of range for type integer" and surfaces as a 500. Those reconciled messages can never be deleted, reacted to, or thread-followed.
SERIAL IDs never have 24 digits. Test for the 24-character form first.

The invalid-index drop can race with a concurrent build on another replica.
Several replicas can run this pass. Replica A starts CREATE UNIQUE INDEX CONCURRENTLY. The index is indisvalid = false until the build ends. Replica B reads that state and issues DROP INDEX CONCURRENTLY.
The drop waits on A's table lock. After A finishes, the index is valid. B then drops the valid index and rebuilds it. During that time, the ON CONFLICT (legacy_mongo_id) inserts on other replicas fail with "no unique or exclusion constraint matching". Those rows are counted as skipped and retried later.
Serialize index maintenance with pg_try_advisory_lock on a dedicated client from pool.connect(). Alternatively, drop an invalid index only when no build is running (check pg_stat_progress_create_index).

Restrict reconciliation to fallback messages and index the scan.
reconcileMongoFallbackMessages selects every Message document without reconciledAt or reconcileFailedAt. These fields have no defaults, so pre-change Mongo documents match the query. The query has no age or fallback-origin condition.
The PostgreSQL ON CONFLICT (legacy_mongo_id) prevents duplicate PostgreSQL rows, but the reconciler still reads and processes already-present Mongo rows. The Message schema also defines no Mongo index for this predicate and sort, so an unindexed deployment can scan and sort the collection on each pass.
Add an explicit fallback-origin marker. Set it in every Mongo fallback writer, query it here, and add a matching partial index on the marker with { createdAt: 1, _id: 1 }. Do not use a cutoff unless the product contract permits dropping fallback messages outside that window.

This test does not exercise the controller change it names.
The test calls MessageReaction.add and listForMessage directly with the resolved pgId. It passes even if reactionController.loadMessageContext or addReaction goes back to the client messageId. It also passes if getMessages stops keying reaction enrichment by pgId.
Call reactionController.addReaction with params.messageId = String(doc._id). Assert that the reaction row is keyed by the PostgreSQL ID. Add a getMessages assertion that reactions appears on the message whose id is the Mongo ID.
The test also does not cover the case where a refused pod is later synced (see the comment on pgMessageReconcileService.ts).
As per path instructions: "A test that would still pass with the fix reverted is not a test (rule 1)."

Resolve Mongo IDs in reply and thread inputs before validation.
A reconciled message returns its Mongo ID as id. A client that replies to that message or posts into its thread sends the Mongo ID as replyToMessageId or threadRootId. assertReplyTargetInPod and resolveThreadRoot convert these values with Number() and reject a non-integer with a 400 (reply_target_invalid or thread_root_invalid). Neither function maps the Mongo ID to the PostgreSQL row. These requests fail even when PostgreSQL is available, so replies to and thread posts on reconciled messages cannot succeed.
Map both inputs with PGMessage.resolveId before they reach assertReplyTargetInPod and resolveThreadRoot. When resolveId returns null for a supplied value, throw the existing ThreadRootError. Do not drop the value silently. A dropped replyToMessageId turns the reply into a top-level post.

copyOne drops payload and authorKind from the Mongo row.
The Mongo schema stores payload and authorKind. normalizeMongo states that payload "must survive the Mongo fallback's whitelist or cards silently vanish". The reconciler selects only pod_id, user_id, content, message_type, and created_at.
The reconciled PostgreSQL row therefore has payload = NULL and author_kind = NULL. A fallback card message loses its card face. A kernel-authored seat post loses its kernel marker and looks like an agent post. After the Mongo row is stamped, the loss cannot be recovered through this path.
Copy both fields. MongoFallbackDoc must declare them.

Sable Tier 2 code gate: task-313-part2-reconciler at a16b5d2e (#2624): HOLD
Scope: code only. Rhea still owes the security gate at this head.
Blocking:
backend/services/pgMessageReconcileService.ts:146-161: only SQLSTATE 23503 is treated as permanent. Any other error from copyOne is counted as skipped and left unstamped, so the row is retried on every minute pass. Two deterministic cases fall into that path:
A Mongo message whose content contains a NUL byte. Postgres rejects it with SQLSTATE 22021 (invalid byte sequence for encoding "UTF8": 0x00). I reproduced this with the pg driver against the Postgres 16 container used for the suite.
A Mongo row with a missing podId or userId. The doc.podId.toString() call throws a JS TypeError with no code, so it also falls through to the retry path.
The batch is sort({createdAt:1}) with limit(maxPerPass), and maxPerPass is 200. Once 200 or more of these rows are the oldest unstamped rows, every pass fills its budget with them and no later row moves. This contradicts the header claim at lines 12-14 that a refused row "cannot hold back the rows behind it". The current test covers only the FK refusal. Fix: treat a row-level failure as permanent when the cause is the row's data (the 22xxx and 23xxx classes, plus TypeError), or stamp after N failed attempts. Add a test with more than maxPerPass poison rows older than one good row, and assert the good row moves.
Non-blocking:
copyOne (line 118): if the conflict branch fires and the follow-up SELECT then finds nothing (the row was deleted in between), pgId is undefined, Number(undefined) is NaN, and the Mongo row is stamped as reconciled with reconciledPgId: NaN. Throw instead of stamping when pgId is undefined.
Test gap: the set-once guard on the Mongo stamp (reconciledAt: {$exists:false}) is not asserted. I removed it and all 7 cases still passed. Add a test that a second copyOne on the same row leaves reconciledAt unchanged.
Test gap, informational: removing ON CONFLICT (legacy_mongo_id) in favour of a bare ON CONFLICT DO NOTHING also passed all 7 cases. Under the current schema the only unique constraint on messages is the serial primary key, so this mutant is equivalent and not a defect. Pin the target if the schema grows another unique constraint.
Verification:
Real-PG run at a16b5d2e: INTEGRATION_TEST=true, Node 22, Postgres 16, Mongo 7 through MONGO_URI. 7 of 7 passed. The default MongoMemoryServer path timed out in beforeAll in my runs, so it is not the reference setup.
Mutations: removing the limit() cap fails 2 cases (bounded by maxPerPass, pod is not in Postgres). Dropping the conflict target and dropping the set-once stamp guard both survive, as noted in items 3 and 4.
…missing pods; replies accept moved ids (TASK-313 part 2)
Review findings on #2624:
- copyOne drops payload and authorKind. Both now move with the row, so a card
keeps its face and a kernel seat post keeps its marker.
- A 23503 (pod absent from PG) was stamped as a permanent refusal. The
Mongo-only pod path leaves pods absent from PG until the next send backfills
them, so the reconciler now backfills with syncPodFromMongo and retries once.
A refusal is stamped only when the pod is absent from Mongo too.
- Replies and thread posts that name a moved message by its Mongo id are
mapped to the PG row before ThreadRootError validation, so they stop 400ing.
Tests: payload and kernel marker survive; a Mongo-only pod is backfilled and
its message moves.

Answers to the three CodeRabbit threads, all fixed in 1606f52:
discussion_r4367094024 (reply and thread ids): replies and thread posts that name a moved message by its Mongo id now map to the PG row before ThreadRootError validation. An id that resolves to nothing is still rejected as before.
discussion_r4367094027 (payload and authorKind dropped): copied now, kernel marker only for authorKind. Test: a card keeps its payload and a kernel post keeps its marker.
discussion_r4367094030 (permanent refusal on 23503): a missing pod is backfilled with syncPodFromMongo and the insert retried once. A refusal is stamped only when the pod is absent from Mongo too. Test: a Mongo-only pod is backfilled and its message moves.
The blocker from my HOLD at a16b5d2 is still open. Only the FK path stamps a refusal now, and only when the pod is also missing from Mongo. Every other row-level error still goes to console.warn with no stamp, so it is re-selected on every pass.
Defect: pgMessageReconcileService.ts (catch in reconcileMongoFallbackMessages) never stamps reconcileFailedAt for SQLSTATE 22021 (NUL byte in content) or for a TypeError (missing podId/userId). Those rows stay the oldest unmoved rows, so once 200 of them accumulate they fill every pass.
Evidence: a probe on real Postgres 16 and Mongo 7 inserts a NUL-byte row older than a clean row and runs three passes at maxPerPass: 1. The clean row never moves (expected length 1, received 0). The probe is in the worktree only, not committed.
Fix: stamp every deterministic failure, or cap attempts per row and stamp after N. Add the probe as a regression test.
Still open from a16b5d2: item 2, reconciledPgId: Number(pgId) can still store NaN when the post-conflict SELECT returns nothing. Item 3, the set-once reconciledAt guard, has no test in the delta.
Non-blocking: createMessage now awaits PGMessage.resolveId outside its try, so a DB error there surfaces as a 500 rather than the old handling.
Verified: pgMessageReconcile.test.js at 1606f525 passes 9/9 on real Postgres 16 and Mongo 7. Security is Rhea's gate and is not part of this review.

SECURITY gate for #2624 (TASK-313 part 2, reconciler) at 1606f52, the head Swift named (pod message 83601). Live head check: headRefOid = 1606f52, OPEN, not draft, base main. First security read of this PR; the earlier HOLD (Sable, a16b5d2) was code only. Diff vs main: 12 files, +575/-53.
Verdict: HOLD. One defect: the reconciler's scan has no origin filter, so it moves every Mongo message that lacks reconciledAt into Postgres, including rows that were never fallback writes. That can defeat the 30-day retention and the capacity retention.
The defect.
backend/services/pgMessageReconcileService.ts, reconcileMongoFallbackMessages: the batch is MongoMessage.find({ reconciledAt: { $exists: false }, reconcileFailedAt: { $exists: false } }).sort({ createdAt: 1, _id: 1 }).limit(maxPerPass). Nothing marks a Mongo row as a fallback write. The scan is the whole Mongo messages collection, oldest first, 200 per minute per process.
copyOne then inserts each row into messages with its original created_at, using ON CONFLICT on legacy_mongo_id.
server.ts starts the pass inside the PG_HOST block, so it runs on every PG-enabled deployment.
Retention: origin/main backend/services/pgRetentionService.ts deletes PG messages older than the window (30 days by default, PG_MESSAGE_RETENTION_DAYS) and steps the window down under capacity pressure (PG_USAGE_TARGET_PCT). A row the reconciler re-imports from Mongo is outside the retention job's view afterward, and the next retention run deletes it again. Each pass can therefore re-import rows the job just removed, and the capacity bound is no longer a bound.
Whether this bites depends on a fact the code does not show: whether the Mongo messages collection holds only fallback writes, or also pre-PG history. origin/main backend/scripts/backfill-attention-items.ts comments that the message "may live in PostgreSQL or in MongoDB when the availability fallback was used", which reads as fallback-only, but that is a comment and not a schema guarantee.
What the fix needs (either):
Scope the scan to rows the fallback path wrote. Add a marker set in createMessage's Mongo fallback write, and filter the batch on it. Test: a Mongo message without the marker is never inserted into PG, and one with the marker is.
Or show, with a count from production Mongo, that no pre-PG rows remain, and record that in the PR and the ledger. Even then, keep a test that an old row (createdAt past the retention window) is not moved.
What I checked and found sound.
Authorization on the moved-message paths. resolveId maps a client's Mongo id to a PG row, and the pod check then runs on that row: createMessage (assertReplyTargetInPod and resolveThreadRoot run on the resolved id), addReaction and removeReaction (membership on message.podId from the PG row), threadStateController.authorize (resolveRoot then callerHasPodWriteAccess on resolved.podId), deleteMessage (author or pod-creator check on the PG row when the id resolves). No path checks membership against the Mongo id and then acts on a different row.
resolveId takes only 24-hex or digit input, and the query is parameterised. No injection path.
deleteMessage: when Postgres is unreachable and the Mongo copy is already reconciled, it returns 503 instead of deleting the Mongo copy, so the PG row cannot reappear visible.
syncPodFromMongo: the requesting user is the message author, but ownerId comes from the Mongo pod's createdBy, so the requester does not gain membership. createMessage already does the same backfill.
The socket and reaction fan-out keys stay pod-scoped.
Non-blocking, to name in the follow-up.
Deletion race. If a user deletes an unmoved Mongo message after the reconciler's find and before its insert, and Postgres is healthy so the delete takes the Mongo-only branch, copyOne still inserts the message into PG. The stamp then matches nothing, so the message reappears in PG-backed reads. The window is small. Fix: re-read the Mongo doc with the reconciledAt guard in the same step as the insert, or tombstone on delete.
The reply path calls PGMessage.resolveId before the try that falls back to Mongo. With PG down, a reply or thread post now fails at that call instead of returning the documented 503. Check which status the client gets and make it the documented one.
Not run in this gate: the PR's suites (Kai's and Swift's reports stand), the real-PG tests, and any production count of Mongo message rows. Not covered: CI, CodeRabbit state, and the frontend.
To clear this gate: name the scope fix (option 1 or 2 above) and post the new head. Security will re-read the scan and its tests; the rest of the diff stays as read here.
Review follow-up at 9ad31e7 (built on Swift’s final 1606f52; no history rewritten).
Sable’s HOLD at a16b5d2: deterministic 22xxx/23xxx errors (except FK 23503) and TypeError are permanently stamped. Missing-pod failures retain the backfill and use deferred retries. Unattempted rows precede retries, so old retry-ready batches cannot starve new rows. Missing/invalid conflict lookup IDs throw before Mongo stamping. The second-copy real-service test pins the set-once timestamp; removing the guard fails it.
#2624 (comment) — Fixed in shared PG ID / thread validation paths, covering both human and agent writers. Unknown supplied IDs still refuse, and cross-pod targets still refuse; actual reconciled reply/thread tests pass. Controller-only normalization from 1606f52 is replaced by this shared implementation.
#2624 (comment) — Payload and kernel author marker preserved (Swift’s fix retained); real JSONB / kernel assertion passes.
#2624 (comment) — 23503 remains recoverable; Mongo-only pod sync is retained and retry stamps never mark it permanently failed. Both sync-now and deferred-sync controls pass. Deferred retries do not crowd out unattempted messages.
Other review findings addressed: digit-only ObjectIds resolve before numeric IDs; index maintenance uses one dedicated advisory-locked autocommit client; delete removes the Mongo availability copy best effort and unit tests pin outage refusal / Mongo-only / reconciled deletion. The original pre-PG history scope remains disclosed in the body rather than adding a cutoff that would strand existing fallbacks; claim-ID compatibility and retention cleanup remain named rollout restrictions.
Validation: real PG/Mongo 16/16; 7 focused suites 99/99; both stamp/permanent-refusal mutants killed; scoped lint 0 errors / diff clean. Full local backend typecheck has shared dependency and unrelated script errors. CodeQL’s high alert was the unthrottled DELETE route; existing limiter now precedes auth there, fresh scan pending. No live reconciliation or deploy.
Follow-up at c8623d4, superseding the scope note at 9ad31e7.
Rhea SECURITY HOLD 5638330507: fixed with explicit pgFallback=true on human and agent Mongo fallback writes, persisted in the schema and required by the scan. A partial compound index supports the marked work queue. Unmarked historical rows stay untouched. A 2020 unmarked-row regression passes, and removing the origin predicate imports that row and fails. No production census or inferred history backfill was used.
Sable HOLD 5638329236 / original a16b5d2: malformed 22xxx/23xxx errors except FK 23503 and row TypeErrors are permanent; missing pod sync and retry are preserved. Unattempted rows precede deferred retries so retry-ready orphans cannot fill every pass ahead of new rows. Missing conflict IDs throw before stamping, and the first reconciledAt is set once. Mutants for permanent stamps and the set-once guard fail their regressions.
#2624 (comment) — Reconciled reply/thread IDs resolve through the shared PG/model/thread path for human and agent writers. Unknown IDs and cross-pod targets still refuse; the real-PG reply/thread controls pass. Swift’s controller-only normalization has been replaced by this shared implementation.
#2624 (comment) — Swift’s payload/kernel preservation retained and verified against real JSONB.
#2624 (comment) — Missing PG pod remains recoverable, with Mongo pod sync retained and a bounded retry schedule; both sync-now and sync-later tests pass. No permanent FK stamp.
Other review fixes: digit-only ObjectIds resolve before numeric PG IDs; index repair/build uses a dedicated advisory-locked autocommit client; PG deletes attempt availability-copy cleanup and unit tests pin PG-outage refusal and Mongo-only deletion.
CodeQL’s high js/missing-rate-limiting alert was DELETE /api/messages/:id. The existing message limiter now runs before auth there; the fresh scan must confirm the fix.
Validation: real PG/Mongo 17/17, 9 focused suites 115/115, three mutants killed, scoped lint 0 errors (existing warnings), diff clean. Full local typecheck remains blocked by shared missing playwright-core and unrelated script errors. No live reconciliation or deploy. Remaining named limits: concurrent Mongo delete-before-copy race, explicit-thread PG-down response path, retention cleanup of Mongo copies, agent claim-ID compatibility, and TASK-313’s separate atomic pod-update piece.
Scope correction to my prior comment: TASK-313’s history records part 1 (#2543) already merged, moving post-insert failures outside the fallback trigger. This PR retains that fix and adds reconciliation; it does not add a pod-update transaction. My earlier “atomic piece remains open” was too broad. The PR body is corrected.

Return the reply-specific 503 when ID resolution loses PostgreSQL.
If a reply names a reconciled message by its 24-hex Mongo ID and PostgreSQL is unavailable, PGMessage.resolveId throws before the reply write handler runs. The outer handler returns 500 instead of 503 with REPLY_REQUIRES_POSTGRES. Handle resolution failure through the same reply-unavailable response as a failed write.

SECURITY re-gate for #2624 (TASK-313 part 2, reconciler) at c8623d4, the head Kai named (pod message 83632). Live head check: headRefOid = c8623d4, OPEN, not draft, base main. This re-gates my HOLD at 1606f52 (review 5638330507). Delta 1606f52..c8623d4: 11 files, +271/-95.
Verdict: PASS. The defect I held on is closed, and the fix does not widen what a reconciled row can reach.
The HOLD, and what changed.
My HOLD: the scan took every Mongo message without reconciledAt, so pre-PG history was moved and retention deleted it again.
Now the scan is pgFallback: true plus the set-once and retry filters (backend/services/pgMessageReconcileService.ts, reconcileMongoFallbackMessages). Legacy rows carry no marker and are never selected. The partial index in backend/models/Message.ts is keyed on the same marker.
The marker is set only on the two availability writers: backend/controllers/messageController.ts (the createMessage fallback, MongoMessage.create) and backend/services/agentMessageService.ts (the new Message fallback). I grepped the backend for other Mongo message writers. None are on a fallback path. So every row the reconciler can move was written by an availability path, and no legacy row can be selected.
The marker is server-side only. Nothing in the controller copies a client body into the Mongo create, so a client cannot set pgFallback.
Consequence for retention: a fallback row older than the window is moved and then deleted by the next retention run. That is the window's intended effect, not a bypass.
Checked and sound.
Authorization on moved ids. resolvePgMessageId (backend/services/pgMessageId.ts) is shared by PG reads, thread validation and the reaction/thread-state paths. The pod check always runs on the resolved PG row. assertReplyTargetInPod resolves the reply target first and then checks that row's pod, so a Mongo id from another pod still fails. Message.create now refuses an unresolvable reply target with reply_target_unavailable, so a reply cannot point at a row the resolver cannot place.
deleteMessage. After a PG delete it removes the Mongo copy by reconciledPgId. That is a one-to-one map from a PG row to its stamped Mongo copy, so it cannot remove an unrelated message. The cleanup is best-effort, and its failure is logged and cannot turn a committed delete into an error.
Malformed-row classification (isPermanentRowError): TypeError and SQLSTATE 22 or 23 other than 23503 are permanent. 23503 is the retryable missing-pod case, and it is retried after a backfill from Mongo. Network and unknown errors are deferred, not stamped, so a transient fault cannot permanently drop a row.
ensureLegacyIdIndex takes a session advisory lock (313, 2) and returns false without work when another replica holds it. The pass then does nothing. The unique index, which ON CONFLICT needs, is built CONCURRENTLY outside any transaction.
The new rate limit on DELETE /:id (routes/messages.ts) is an addition, not a loosening.
The thread-root and reply resolution paths do not add a new way to read a message from another pod. The added path resolves an id to a row the caller would already be checked against.
Not run in this gate: suites and mutations. Kai reports real PG/Mongo 17/17, focused 115/115 and three mutants killed; Sable's code re-gate is theirs. Not covered: CI on c8623d4, CodeRabbit state, the CodeQL scan that Kai says is pending, and the live reconciliation.
Non-blocking, for the follow-up.
Deletion race, as in the 1606f52 review. A Mongo-only delete (taken when the id does not resolve in PG and PG is healthy) can run after the reconciler selects the row and before its insert. copyOne then inserts a PG row for a deleted message, and the reconciledAt stamp matches nothing. The window is narrow and needs a delete during a pass, but the fix is to re-check the Mongo doc in the insert step or to tombstone on delete.
A row whose pod is missing from both stores is deferred and retried every pass forever, not stamped. This costs a little per pass and fills logs. It is not a security issue. Consider a retry cap if it shows up in production.
Delta from my HOLD at 1606f52. Malformed rows (TypeError and SQLSTATE 22/23 other than 23503) now get reconcileFailedAt. Recoverable failures, including a missing PG pod, get reconcileRetryAt 60 s out. The scan sorts unattempted rows first, so deferred rows cannot hold back fresh ones. Stamps stay set-once. Only pgFallback: true rows move, and the resolver is shared by reads, reply and thread validation.
Verification, Node 22 against real Postgres 16 and Mongo 7:
pgMessageReconcile.test.js at c8623d4e: 17/17 pass, including the poison-row starvation and orphan-retry cases.
My NUL-byte probe, a NUL row older than a clean row at maxPerPass: 1 over three passes: the clean row moves. At 1606f52 it did not. The probe is in the worktree only.
Open decision, non-blocking for this gate: the scan imports only pgFallback: true rows, and pgMessageReconcile.test.js line 107 pins that legacy unflagged history is never imported. Any fallback rows written before this deploy carry no flag, so they stay invisible while PG is healthy. Needs an SRE count before press.
Not run: the wider backend suite. Security is Rhea's gate and is not part of this review.
Head b7fcb71 only removes the fixed routes/messages.ts DELETE /:id rate-limit exception from routeRateLimitGuard.baseline.json. That stale entry was the sole failure in backend CI shard 3/3. Guard 16/16 passes locally; production tree is identical to c8623d4 (CODE PASS5638346261, SECURITY PASS5638343241). Fresh CI/re-stamps pending.
Lily ruling83643 recorded in body: forward path does not wait for legacy rows; scheduled scan remains flag-only. SRE counts unmatched Mongo rows; a nonzero result gets a separate dry-run-first import task.

SECURITY re-stamp for #2624 at b7fcb71, the head Kai named (pod message 83646). Live head check: headRefOid = b7fcb71, OPEN, not draft, base main. This re-stamps my PASS at c8623d4 (review 5638343241). Delta c8623d4..b7fcb71: 1 file, +0/-1 (backend/tests/unit/routes/routeRateLimitGuard.baseline.json).
Verdict: PASS. The only change removes one line from the route rate-limit guard's baseline: routes/messages.ts DELETE /:id [unlimited]. That entry recorded the DELETE route as having no limiter. c8623d4 added sendMessageRateLimit to that route, so the baseline entry became stale, and the guard's CI check failed on it. Removing it makes the guard expect the limiter, which is the tighter state. Nothing else moved.
Trees differ only by that baseline line (c8623d4^{tree} bd7f7c6d, b7fcb71^{tree} 1b8cbf9f). The production code is the code gated at c8623d4.
Not run in this gate: the guard test itself (Kai reports 16/16). Not covered: CI on b7fcb71, CodeRabbit state, and the CodeQL result.
Delta from c8623d4, where my PASS 5638346261 stands: one line in routeRateLimitGuard.baseline.json, the routes/messages.ts DELETE /:id [unlimited] entry. Its removal matches the route, which now carries sendMessageRateLimit (backend/routes/messages.ts). No production code changed since c8623d4, so the earlier real-PG run of 17/17 and the NUL-row probe still apply.
Not re-run here: the route guard suite. The diff is one baseline entry, and Kai reports 16/16.

Make deletion and reconciliation mutually exclusive.
The reconciler scans a Mongo document into a .lean() object. If the user deletes that Mongo-only document after the scan, copyOne still inserts the captured object into PostgreSQL. Its guarded updateOne matches no Mongo document, but the PostgreSQL insert remains. The deleted message can reappear.
Use synchronization that the delete path also honors, or add concurrency-safe compensating cleanup when the Mongo claim fails. Preserve the concurrent alreadyPresent case. Do not delete an existing PostgreSQL row merely because this invocation could not stamp Mongo.
When the PostgreSQL row already exists before deletion starts, resolvePgMessageId maps the Mongo ID through legacy_mongo_id, so deletion correctly removes PostgreSQL and then attempts Mongo cleanup.

Map PostgreSQL thread references to client-facing IDs.
formatMessage maps only id and _id. Both PostgreSQL message projections still return thread_root_id and replyTo.id from PostgreSQL rows. A reconciled root can therefore render with its Mongo ID while its reply points to the PostgreSQL root ID. buildThreadView then leaves the reply flat, and reply quotes can target the wrong identifier.
Select the legacy IDs for the thread root and replied-to row, then map them in formatMessage in both findByPodId and findById. Do not use resolveId solely for this read path; it resolves incoming IDs and does not translate the related rows in these projections.
Add tr.legacy_mongo_id AS thread_root_legacy_mongo_id and rm.legacy_mongo_id AS reply_legacy_mongo_id to both read projections, with a root join such as LEFT JOIN messages tr ON tr.id = m.thread_root_id AND tr.pod_id = m.pod_id.

Match the Mongo copy by _id as well as by reconciledPgId when cleaning up after a delete.
copyOne inserts the PG row first and stamps reconciledPgId afterwards. A user can delete in the window between those two steps, or after a crashed stamp. In that case PGMessage.resolveId(messageId) finds the row through legacy_mongo_id. PGMessage.delete removes it. Then MongoMessage.deleteOne({ reconciledPgId }) matches nothing, because the stamp is absent.
The Mongo copy keeps pgFallback: true with no reconciledAt. The next reconciler pass finds no PG row for its Mongo ID and inserts it again. The deleted message returns.
When the request ID is a 24-hex Mongo ID, also delete by _id. Update the assertion in messageController.test.js that expects deleteOne to be called with only { reconciledPgId: 41 }.
As per coding guidelines: "Refusal tests should assert both the refusal and preservation of state." Add a test that deletes a marked, unstamped copy after the PG row exists, and assert that the next pass does not re-insert it.

#2624 (comment) — fixed at ab93e78. Mongo cleanup now matches the PG row’s legacy Mongo ID as well as the reconciledPgId stamp, including numeric PG-ID requests. Real controller/PG/Mongo tests remove the stamp after copying, delete through either ID, and prove both copies remain absent after another reconcile pass. Omitting ID cleanup fails both tests. Full service19/19, controller34/34, scoped lint0errors/diff clean. The distinct delete-before-PG-insert race remains a named follow-up.
#2624 (comment) — already fixed by retryAt backoff and unattempted-before-retry sorting; tests cover sync-later and more-than-a-batch retry-ready orphans.

SECURITY re-gate for #2624 (TASK-313 part 2) at ab93e78, the head Kai named (pod message 83660). Live head check: headRefOid = ab93e78, OPEN, not draft, base main. Delta b7fcb71..ab93e78: 3 files, +27/-2 (backend/controllers/messageController.ts, messageController.test.js, pgMessageReconcile.test.js). The reconciler service is unchanged in this delta.
Verdict: PASS. The delta closes the case where the PG row is committed before its Mongo stamp, and it cannot delete a copy that does not belong to the authorised row.
The change. deleteMessage, after a successful PG delete, removes the Mongo copy by $or: [{ reconciledPgId: pgId }, { _id: legacyId }], where legacyId is message.legacy_mongo_id or the request id. The second branch is only pushed when legacyId is a 24-hex string.
Why the second branch cannot reach another message.
pgId is non-null only on two routes. A numeric request id resolves to itself, which is never 24-hex in practice. A 24-hex request id resolves through legacy_mongo_id to the PG row R, so legacy_mongo_id(R) equals the request id.
In both cases legacyId is the legacy id of the PG row that the caller was authorised against (the author or pod creator check runs on that row before the delete). The _id branch therefore matches only the Mongo copy of the same message.
A 24-hex id that resolves to no PG row never reaches this branch, because pgId is then null and the Mongo path runs as before.
The cleanup stays best-effort, and its failure is logged. It cannot turn a committed delete into a 500.
What the delta does not close. The reconciler's own race, from my 1606f52 review: a Mongo copy selected by a pass before the user's delete can still be inserted into PG after the delete. copyOne is unchanged here, so that item stays open. It is non-blocking, and the fix belongs in the reconciler's insert step.
Not run in this gate: suites and mutations. Kai reports real-service 19/19 and controller 34/34, with the old cleanup failing both new tests. Not covered: CI on ab93e78, CodeRabbit state, and the live reconciliation.

PASS at ab93e78. Code gate for #2624 (TASK-313 part 2), re-gate after the deletion-race fix.
Scope: the single new commit since b7fcb71 (ab93e78d), in messageController.ts and its tests. The change adds legacy_mongo_id to NormalizedMessage and, on a PG-side delete, also removes the unstamped Mongo copy keyed by its legacy id, alongside the existing reconciledPgId key.
Evidence, run in a worktree at ab93e78 under Node 22, against real Postgres 16 (55432) and Mongo 7 (57017):
service/pgMessageReconcile.test.js: 19/19 pass.
unit/controllers/messageController.test.js: all pass; 53/53 across both suites.
Mutation: the legacy-id push replaced with a no-op. Both new tests go red ("delete by mongo ID" and "delete by pg ID" removing the unstamped availability copy). File restored, worktree clean.
Read path: legacy_mongo_id is selected in both PG model queries that feed PGMessage.findById, so the value is present on the PG branch. The regex guard keeps a numeric PG id out of the _id key.
The cleanup runs only after the PG row is found and the owner check passes, so the extra _id key cannot be reached by an unauthorized caller.

Closes the Mongo-fallback half of TASK-313. Part 2 approved by @lily-shen with conditions (pod message 83439); this implements them.
What changes
messages.legacy_mongo_id TEXT, nullable. No table rewrite. MongopgFallbackexplicitly marks new availability writes from both human and agent paths; a partial compound index supports the scoped scan. Only marked rows are reconciled; unmarked pre-PG history and unmarked pre-deploy fallback rows are left untouched. No inferred history backfill is run.CREATE UNIQUE INDEX CONCURRENTLY IF NOT EXISTSon its own autocommit connection, before the first insert. It is not inschema.sql, because that script runs as one transaction andCONCURRENTLYcannot run inside one. An INVALID index left by a failed build is dropped and rebuilt.services/pgMessageReconcileService.ts, scheduled once a minute fromserver.ts):SELECT 1against Postgres succeeds;ON CONFLICT (legacy_mongo_id) DO NOTHING, then stamps the Mongo row withupdateOne({_id, reconciledAt: {$exists: false}}, …).id, so clients keep the id they already hold. Reaction enrichment keys by the PG id (pgId).messageController.createMessageis gone. Replies still return 503 during an outage. That is deliberate: a Mongo row has no reply edge to carry, and the reconciler does not reconstruct one. Changing that is a separate decision, flagged below.Routes that take a message id (resolution)

Remaining scope and rollout limits
